Lateral cephalographs from 220 normal adolescent white orthodontic patients, and of individuals with craniofacial dysostosis and oculodento-osseous dysplasia were studied. Variations were observed in the morphology of the posterior margins of the superior articular processes, vertebral artery canals being present in 8%. Posterior arch dehiscence was found in eight individuals, including seven cases in the midline. Three patients had accessory ossicles above the posterior arch of the atlas, and two showed evidence of fusion of the second and third cervical vertebrae. The profile of the atlas wa

Anatomically, the vertebral artery courses through six foramina transversaria of the cervical vertebrae, passing through the groove on the surface of the arch of the atlas and then penetrating the dura mater. Because of this anatomical course, the vertebral arteries are often affected by head motion. Stenotic change of the vertebral artery can occur at the atlantoaxial level in head rotation. Such a special type of stroke was named "bow hunter's stroke" by Sorensen. We report three cases of bow hunter's stroke and discuss the angiographic examinations. As surgical treatment, we performed parti

The article reports on the first detailed vertebral and rib morphology of anguine taxon Pseudopus apodus using micro-computed tomography. A comparison shows significant morphological differences of vertebrae of Pseudopus relative to those of Anguis and Ophisaurus. Usually, there are 55 presacral vertebrae, two sacral, and 95-97 caudal vertebrae. Pseudopus apodus can be defined by 23 diagnostic features concerning the vertebral column. Although zygapophyseal articulation between atlas and axis is well developed in limbed anguid gerrhonotine lizards like Abronia or Barisia, it is absent in the e

We report the autopsy of a traffic accident victim with a rare atlas fracture. The deceased was 52-year-old man found dead on the road because of a traffic accident. We performed a postmortem computed tomography and an autopsy on the body. The sagittal reformation image of postmortem computed tomography showed a vertically separated anterior arch of atlas, suggesting a "horizontal fracture." Therefore, we assumed that cervical hyperextension may have been one of the forces that affected him during the accident. Minor spinal cord injury was also noted. In this case, postmortem computed tomograp

We present an unusual presentation of unstable atlanto-occipital dissociation as locked-in syndrome. Traumatic atlanto-occipital dissociation is a severe injury that accounts for 15-20% of all fatal cervical spinal injuries. A disruption occurs between the tectorial ligaments connecting the occipital condyle to the superior articulating facets of the atlas, resulting in anterior, longitudinal, or posterior translation, and it may be associated with Type III odontoid fractures. Furthermore, the dissociation may be complete (atlanto-occipital dislocation) or incomplete (atlanto-occipital subluxa

A 6-month-old male-castrated domestic shorthair cat was presented shortly after being bitten on the neck by a large breed dog. On presentation the cat was non-ambulatory tetraparetic with preserved deep pain perception. Plain radiographs of the neck did not show any abnormalities. Computed tomography was performed and showed a right-sided depressed fracture of the dorsal lamina of the atlas. A partial dorsal laminectomy was performed to alleviate the spinal cord compression. The cat made a full recovery within 12 weeks of surgery.

The imaging features of four dogs with atlanto-occipital overlapping are described. This malformation appeared to play a role in the development of neck pain, ataxia, variable cerebellar involvement, medullary kinking, and possibly syringomyelia. Using cervical radiographs, three of the four dogs were initially diagnosed with an atlanto-axial malformation. Because this disorder could not account for all clinical signs, magnetic resonance and computed tomography images were also acquired. These provided a more complete evaluation of the craniocervical junction, allowing detection of atlanto-occ

Achondroplasia has a known association with foramen magnum stenosis that can result in cervicomedullary compression, which is most often due to a hypertrophied posterior occipital rim and an undersized transverse diameter. The authors present a unique case of a child with achondroplasia with symptomatic craniocervical compression from marked overgrowth of his opisthion anterior to the posterior arch of the atlas. This 22-month-old child with achondroplasia presented with severe respiratory and motor disabilities, including progressive quadriparesis and apneic episodes requiring continuous posi

OBJECTIVE: To investigate the classification of atlas pedicles and the methods of the pedicle screw fixation.
METHODS: To study the classification of atlas pedicles, 48 dry adult atlas specimens were measured. By atlas 3D-CT reconstruction, two transverse sections were established by going through the one third of the lateral atlas pedicle and 2 mm below the vertebral artery sulcus. By setting 3.50 mm and 1.75 mm as the standardized diameter and radius for the screw and according to the thickness of bone substance of vertebral artery sulcus that went through the one third of the lateral atlas

Fractures of the atlas constitute 4% to 12% of all bony injuries of the cervical spine; most are treated successfully by a cervical orthosis. Nonunion may be associated with neck or scalp pain on movement and is treated conventionally by some form of craniocervical fusion, which restricts head movement. The authors describe a case in which direct repair of the bony ring with a titanium plate and screws allowed bone healing, relieved the symptoms and maintained a full range of neck movements. The titanium plate interfered little with postoperative MR and CT imaging.

For the purpose of understanding the acute instability of a burst (Jefferson) fracture of the atlas, the authors produced the fractures experimentally and measured multidirectional flexibilities in seven cadaveric C0-C3 specimens. The flexibilities were measured by the authors' standardized method: they applied six types of physiologically pure moments (up to 1.5 Nm) and recorded the ensuing C0-C2 motions by stereophotogrammetry. The flexibility tests were performed before and after the production of the fracture. The greatest increase in flexibility due to the injury was in flexion-extension

Congenital clefts of the anterior and posterior arches of the atlas were demonstrated in a cervical spine CT performed in a case of suspected trauma. Intact cortical margins and lack of adjacent soft tissue swelling are CT indices distinguishing congenital clefts from fractures.

A bilateral aplasia of the dorsal arches of the atlas with persisting tuberculum posterius has been observed by the authors. Since the patient had suffered a severe cranio-cerebral trauma, it became necessary to exclude a possible fracture by differential diagnosis. This was possible by means of CT and conventional x-ray diagnosis. According to the classification of dorsal atlas arch aplasia by von Torklus and Gehle (1975) into 6 types, the diagnosis was, in the case under report, Keller's type of aplasia of the arch of the atlas.

The stability of a burst fracture of the atlas (Jefferson's fracture) should be determined in order to decide upon the appropriate treatment. Unstable fractures include those with a rupture of the transverse ligament, or those in which union has been incomplete despite treatment. An occiput-to-C2 or in atlantoaxial arthrodesis may be indicated.
