Shoulder-arm pain from cervical bands and scalene muscle anomalies.

Fourteen patients were identified with (1) pain and sensory changes in a brachial plexus distribution, (2) aggravation of pain with use of the affected extremity, and (3) pain on palpation over the brachial plexus. All patients had minimal or no intrinsic hand muscle atrophy. Only one patient had cervical ribs. Nerve conduction studies were normal, and electromyography (EMG) showed mild chronic neuropathic changes in 2 patients. None of the patients responded to conservative therapy over a prolonged period (7-12 months). A compressive brachial plexopathy from abnormally attached or enlarged sc…

Thoracic outlet syndrome: a review.

Thoracic outlet syndrome (TOS) is a rare condition (1-3 per 100,000) caused by neurovascular compression at the thoracic outlet and presents with arm pain and swelling, arm fatigue, paresthesias, weakness, and discoloration of the hand. TOS can be classified as neurogenic, arterial, or venous based on the compressed structure(s). Patients develop TOS secondary to congenital abnormalities such as cervical ribs or fibrous bands originating from a cervical rib leading to an objectively verifiable form of TOS. However, the diagnosis of TOS is often made in the presence of symptoms with physical ex…

Digital ischaemia of the upper limbs in middle age: consider arterial thoracic outlet syndrome until proven otherwise!

OBJECTIVES: Patients presenting with digital upper limb ischaemia are occasionally referred to rheumatology services to rule out vasculitis. We aimed to present two cases of delayed diagnosis of arterial thoracic outlet syndrome (aTOS) in middle-aged patients presenting with digital ischaemia in order to raise awareness of this important pathology that requires timely surgical intervention. METHODS: Two cases of progressive ischaemia of the right upper extremity caused by primarily undiagnosed compression of the subclavian artery by an accessory cervical rib are presented. The case notes, rad…

Thoracic vertebral count and thoracolumbar transition in Australopithecus afarensis.

The evolution of the human pattern of axial segmentation has been the focus of considerable discussion in paleoanthropology. Although several complete lumbar vertebral columns are known for early hominins, to date, no complete cervical or thoracic series has been recovered. Several partial skeletons have revealed that the thoracolumbar transition in early hominins differed from that of most extant apes and humans. Australopithecus africanus, Australopithecus sediba, and Homo erectus all had zygapophyseal facets that shift from thoracic-like to lumbar-like at the penultimate rib-bearing level,

BACKGROUND: Vascular thoracic outlet syndrome (VTOS), encompassing both venous and arterial subtypes, arises from extrinsic compression of t … -limb edema (48.6%) and heaviness with activity (46.2%) were the most frequent presenting symptoms. Cervical ribs were present in 32.3% of cases. All patients underwent first-rib resection and scalenectomy; 32.0% required additional cervical rib excision with or without venolysis, sympathectomy, or thrombectomy. The mean operative time was 54.9 min (range: 50– …

Homeobox genes: molecular link between congenital anomalies and cancer.

Homeobox-containing genes play a major role in the control of segmental identity during embryonic development in Drosophila. Abnormalities of these genes have been shown to produce a wide variety of congenital anomalies in invertebrates and in vertebrates. Many transgenic mice, which are mutant for homeobox genes, show a specific skeletal abnormality, similar to the human cervical rib. In humans, a relationship exists between malformations and tumours. Human cervical rib has been shown to be associated with an increased incidence of malignancy. Recent evidence indicates that homeobox genes mig…

Can a spinal cord tumor cause brachioradial pruritus?

A woman had burning pruritus on both arms and thenar regions for 1 year. The location of pruritus was consistent with C5-C6 dermatomes. Brachioradial pruritus caused by spinal cord tumor was diagnosed. Although cases of brachioradial pruritus were reported in cervical osteoarthritis and cervical rib previously, to our knowledge, our patient is the first reported case of brachioradial pruritus caused by a tumor.

Continuing experience with transaxillary excision of the first rib for thoracic outlet syndrome.

The results of transaxillary excision of the first rib for thoracic outlet syndrome are reported. During a 3-year period, 40 transaxillary rib resections were performed on 32 patients. The symptoms in 33 limbs were completely relieved and in a further four symptoms were improved. These results confirm that transaxillary excision of the first rib is the operation of choice in the management of thoracic outlet syndrome.

Reverse Flow Thromboembolism From Distal Subclavian Artery Aneurysm Due to Arterial Thoracic Outlet Syndrome and Posterior Circulation Stroke-Role of Dynamic Doppler Imaging.

INTRODUCTION: Arterial thoracic outlet syndrome (aTOS) is the least common among the 3 subtypes of thoracic outlet syndrome and can be the cause of posterior circulation infarction due to thrombus from a secondary thrombosed subclavian-axillary artery aneurysm. CASE REPORT: Here, we report a case of a 51-year-old man who presented to our institute with sudden onset vertigo, dizziness, vomiting, gait imbalance, and visual field defects immediately after strenuous exercise. Computed tomography angiography revealed bilateral cervical ribs with aneurysmal dilatation of the left distal subclavian …

Anatomical Variants Identified on Computed Tomography of Oropharyngeal Carcinoma Patients.

Background and Objectives: Anatomical variations in the head, neck and chest are common, and are observed as occasional findings on computed tomography (CT). Although anatomical variations are mostly asymptomatic and do not cause any negative influence on the body function, they may jeopardize diagnosis and may be confused with pathological conditions. The presence of variations may also limit surgical access during tumor removal. The aim of this study was to investigate the prevalence of six anatomical variations-os acromiale, episternal ossicles, cervical rib, Stafne bone cavity, azygos lobe…

An examination of the spectrum of anatomic defects and variations found in eight cases of trisomy 13.

We report the anatomic variations found in four additional cases of trisomy 13. Data from these and four previous cases [Colacino and Pettersen, 1978] are utilized to define a muscle phenotype. Previously unreported defects include the bilateral presence of cervical ribs and the bilateral absence of 12th thoracic ribs in five of the eight cases. One unusual developmental defect of the great vessels is also described. The findings suggest that a definitive diagnosis of trisomy 13 can be made on the basis of six muscle variations.

Cervical magnetic stimulation of the phrenic nerves in bilateral diaphragm paralysis.

Cervical magnetic stimulation (CMS) produces a greater twitch transdiaphragmatic pressure (TwPdi) than electrical stimulation. This may be because CMS produces rib cage muscle activation, thus producing an inspiratory action independent of the diaphragm. Alternatively, CMS could merely stiffen the rib cage, allowing the diaphragm to act efficiently, by contracting against a stable rib cage. To examine these two hypotheses we studied five patients with isolated bilateral diaphragm paralysis using CMS and bilateral electrical phrenic stimulation (BES). TwPdi, twitch esophageal pressure (TwPes),

[The management of patients with thoracic outlet syndrome (TOS) and an assistant diagnosis to discriminate between TOS and cervical spondylosis].

Between 1984 and 2007, 20 patients, admitted to our hospital for the treatment of thoracic outlet syndrome (TOS), were analyzed. Of these 20 patients, 2 subsequently entered rehabilitation to strengthen the muscle power surrounding the shoulder, 5 were followed by a brachial plexus anesthesiological block for the purpose of both diagnosis and treatment of TOS, and the other 13 patients underwent a total of 18 surgeries. Surgical methods consisted of a supraclavicular first rib resection with anterior and middle scalenectomy (SCFR + AMS) in 16 surgeries, and a transaxillary first rib resection
