Varicella Pneumonia in an Immunocompetent Middle-aged Adult Male: A Case Report.

UNLABELLED: Varicella pneumonia is uncommon among adults and can present as potentially life-threatening complications of varicella. Here we report a case of a 43-year-old man with no known history of chronic disease and no allergic history who presented to our hospital emergency department with widespread skin eruptions over the entire body and hemoptysis. Varicella pneumonia was diagnosed based on the patient being in contact with his 6-year-old son who had contracted chickenpox 10 days back, typical cutaneous lesions, pulmonary symptoms and radiographic findings. The patient was treated wit…

Herpes zoster may be a marker for COVID-19 infection during pregnancy.

Varicella-zoster virus infection causes 2 distinct forms of disease: varicella (commonly known as chickenpox) and herpes zoster (HZ)(commonly known as shingles). Primary varicella-zoster virus infection results in the diffuse vesicular rash that is characteristic of chickenpox. Following primary infection, varicella-zoster virus remains dormant in the dorsal root ganglia. This latent phase usually lasts for several decades before reactivation occurs. Varicella-zoster virus reactivation normally presents as HZ in middle-aged adults. A number of cutaneous skin manifestations have appeared in rel…

[Prenatal diagnosis of fetal varicella in the second trimester of pregnancy].

The first case of prenatal diagnosis of congenital varicella by amniotic fluid viral culture and PCR is reported. Chickenpox is a benign disease in children, but it can lead to severe complications in the adult, especially in the pregnant woman. Five percent of women in childbearing age are not immunised, and the incidence of gestational chickenpox is between 1 and 7 per 10,000. The consequences of this primary infection during pregnancy can be severe for the mother, because of the risk of serious varicella pneumonia, and for the fetus. The fetal infection depends on the gestational age at whi…

Diffuse pulmonary micronodules related to prior VZV infection.

BACKGROUND/INTRODUCTION: Chickenpox is a common viral infection caused by Varicella-zoster virus (VZV). Pneumonia is an infrequent complication of chickenpox infection. Rarely, multiple calcified pulmonary nodules can be the sequela of healed VZV pneumonia. CASE PRESENTATION: A middle-aged female individual was found to have diffuse incidental pulmonary micronodules. By further history inquiry and diagnosis of exclusion, her pulmonary micronodules were determined to be likely associated with prior VZV infection. CONCLUSIONS: VZV infection can cause calcified pulmonary nodules related to gran…

Cerebellar mutism caused by primary varicella infection in an immunocompetent child.

Varicella (chickenpox) is a common childhood infection caused by the varicella-zoster virus, which is often self-limiting and usually benign. Although uncommon, neurologic complications of varicella have been documented that include postinfectious cerebellar ataxia, meningoencephalitis, Reye syndrome, myelitis, optic neuritis, stroke, Guillain-Barré syndrome, seventh cranial nerve palsy, and Ramsay-Hunt syndrome. In this case study, the authors describe a 7-year-old girl who presented with varicella skin rash with unsteady gait and anarthria on day 2, and her condition was attributed to cerebe…

Varicella-Zoster Scar Treatments: A Tertiary Review.

Chickenpox is a highly communicable disease caused by Varicella Zoster Virus. Varicella rash commonly evolves into permanent depressed scars, documented in up to 18% of post varicella patients, leaving life-long cosmetic issues for patients. Although there is a lot of reviews on depressed scars, the viral etiology and the unique scar morphology of post varicella scar discriminate it from other depressed scars. Therefore it is required to assess the efficacy of scar removal modalities on these scars, specifically. Yet, despite the prevalence, there is no comprehensive review on chickenpox scars…

Diagnosing smallpox: would you know it if you saw it?

The intentional release of anthrax in the United States in 2001 and other recent acts of terrorism have highlighted the possibility of inten … n diagnosing and evaluating rashes, knowledge of the key differential diagnostic characteristics of chickenpox and smallpox, and the diagnostic interpretation of color photographs of patients with smallpox or chickenpox. Responses were summarized as a perceived comfort score, a differential diagnosis score, and a picture score. Of 266 eligible physicians, 17 …

Magnetic resonance imaging for early diagnosis of necrotizing fasciitis.

One of the most serious complications of chickenpox is varicella gangrenosa, a form of necrotizing fasciitis. A confounding factor is the frequency of more superficial skin infections during chickenpox, often caused by group A beta-hemolytic streptococcus. Because the progression from cellulitis to fasciitis is so insidious, severe tissue necrosis often has occurred before the correct diagnosis is made on the basis of clinical examination. Even today, limb amputation is occasionally required for treatment of gangrenous varicella. In this report, we demonstrate that magnetic resonance imaging c…

Atypical Presentation of Painless Herpes Zoster in an Elderly Male: A Case Report.

Shingles, also known as herpes zoster, is a reactivation of the chickenpox virus that causes a painful, blistering rash. After a chickenpox infection, the virus lies dormant in nerve cells. When reactivated, usually in older adults or those with weakened immune systems, it travels along nerves, typically affecting a single strip of skin called a dermatome. Shingles usually presents as a painful, one-sided rash that may involve nerves in the head, face, and body. While it typically resolves on its own, it can lead to complications, especially in older adults. Antiviral medications are effective…

Varicella in the neonatal ICU due to the Varicella vaccine Oka strain.

BACKGROUND: Varicella vaccination of non-immune post-partum women is recommended to reduce the risk of chickenpox in mothers and their infants. Though rare, transmission of the varicella vaccine strain vOka can occur from recent vaccinees to non-immune contacts who usually develop mild chickenpox. METHODS/RESULTS: Here we describe an infant hospitalized in the neonatal ICU with vaccine-strain varicella due to transmission from their mother who received the varicella vaccine post-partum. We describe the infection prevention and control strategies implemented to prevent further transmission. C…

Management of varicella-zoster virus infection during pregnancy and the peripartum.

This paper reviews the important concepts about varicella-zoster virus (VZV) infection, varicella (chickenpox), and herpes zoster (shingles, zoster) during pregnancy and the peripartum period. The majority of the U.S. population has had chickenpox during childhood, leaving only about 10% of adults over the age of 15 susceptible to the virus. However, nonimmune adults, including pregnant women, are at greater risk for complications and mortality when they contract varicella. The virus is also teratogenic. The implication of VZV infection during pregnancy and the perinatal period are presented. …

Bullous Varicella in an Immunocompetent Adult: A Case Report.

Chickenpox is a common viral infection that usually runs a benign course. Bullous varicella is an atypical presentation of chickenpox, which is usually seen in immunocompromised individuals. We describe a 54-year-old immunocompetent woman who presented to the emergency room with a 10-day history of fever and painful, fluid-filled lesions on her face, trunk, and extremities. Upon examination, the face, trunk, and extremities showed many vesicles, pustules, and bullae over an erythematous base. Multinucleated giant cells (MNGs) with acantholytic cells were visible in the Tzanck smear. A diagnosi…

Perspectives on the History and Epidemiology of the Varicella Virus Vaccine and Future Challenges.

The varicella attenuated virus vaccine, developed in Japan in the 1970s, has dramatically reduced the number of pediatric chickenpox cases over the past 30 years due to its widespread use. However, a small number of cases of chickenpox, shingles, aseptic meningitis, and acute retinal necrosis caused by vaccine strains have been reported. There are also issues that need to be addressed, such as breakthrough infections and the persistence of the preventive effect of vaccination. In addition, there is the possibility of the emergence of revertants or mutations in the vaccine strain. In recent yea…

Optic neuritis caused by varicella infection in an immunocompetent child.

Chickenpox may lead to several neurologic complications, but optic neuritis has rarely been described. We report on a 6-year-old immunocompetent patient who presented with unilateral optic neuritis and severe visual loss because of varicella infection. A week after varicella eruption, the child experienced blurred vision. An examination revealed decreased visual acuity of his right eye, a right pupil poorly reactive to light, and almost no color vision in his right eye. A history of chickenpox, the fundus examination, and the measurement of visual-evoked potentials allowed us to make a diagnos…

[Post-varicella acute retinal necrosis].

A man aged 43 in good health complaints of sudden blurred vision in his right eye, 12 days after a generalized chickenpox eruption. Examination shows an intraocular inflammation with retinal necrosis in temporal periphery. The serum antibodies against varicella-zoster are positive for the IgM and IgG, confirming a recent infection by varicella zoster. Bacterial serology is negative, as well as the serology for the HSV, HIV and CMV. An intraocular production of anti varicella-zoster antibodies is also found by an anterior chamber puncture (Goldmann-Witmer ratio = 1338). A general treatment by a…

Oral Manifestations of Varicella and Their Contribution to Clinical Assessment in Hospitalized and Outpatient Patients.

BACKGROUND: Oral lesions, particularly enanthema, may accompany chickenpox and represent an important but often underrecognized component of the clinical presentation. Their timely identification is especially relevant in dental practice, as oral manifestations may be more frequent in patients with a more severe clinical course. This study aimed to describe characteristic oral cavity changes in hospitalized and outpatient patients with chickenpox, to identify patterns in the occurrence of oral findings in relation to disease severity, and to support clinical assessment in practice. METHODS: A…
