Role of bacillus Calmette-Guérin in the treatment of advanced melanoma.

Early trials of Bacillus Calmette-Guérin (BCG)-based immunotherapy for melanoma consistently show a trend toward improved clinical outcomes in patients treated with BCG compared with observation alone. As an extension of these findings, investigators have initiated the Malignant Melanoma Active Immunotherapy (MMAIT) trials in patients with stage III (MMAIT-III) and stage IV (MMAIT-IV) disease. The overall survival of the patients receiving BCG plus placebo was much better than expected in both studies, thus suggesting a potential for BCG as an adjuvant after the resection of advanced disease.

Targeted therapies for thyroid tumors.

Systemic chemotherapies for advanced or metastatic thyroid carcinomas have been of only limited effectiveness. For patients with differentiated or medullary carcinomas unresponsive to conventional treatments, novel therapies are needed to improve disease outcomes. Multiple novel therapies primarily targeting angiogenesis have entered clinical trials for metastatic thyroid carcinoma. Partial response rates up to 30% have been reported in single-agent studies, but prolonged disease stabilization is more commonly observed. The most successful agents target the vascular endothelial growth factor r

WITHDRAWN: Eletriptan for acute migraine.

BACKGROUND: Eletriptan (Relpax) is a new triptan soon to be made available by prescription for the treatment of acute migraine. Currently five triptans are available by prescription and more are under development. In light of the many drugs for treating acute migraine, there is a need for evidence-based assessments to help determine the relative efficacy and harm of these treatments. OBJECTIVES: To determine the efficacy of eletriptan for treating a single migraine attack using the outcomes of headache response and pain-free response at 0.5, 1, 2 and 4 hours, and sustained relief over 24 hour

Intensified tuberculosis treatment to reduce the mortality of HIV-infected and uninfected patients with tuberculosis meningitis (INTENSE-TBM): study protocol for a phase III randomized controlled trial.

BACKGROUND: Tuberculous meningitis (TBM) is the most lethal and disabling form of tuberculosis (TB), particularly in sub-Saharan Africa. Current anti-TB treatment is poorly effective since TBM mortality reaches 40% in HIV-negative patients and up to 70% in HIV-co-infected patients. To reduce TBM-induced morbidity and mortality, the INTENSE-TBM trial evaluates two interventions in both HIV-infected and uninfected patients: an anti-TB treatment intensification using oral high-dose rifampicin (35 mg/kg daily) and linezolid (1200 mg daily and then 600 mg daily) during the first 8 weeks of the anti

Assessment of Length and Readability of Informed Consent Documents for COVID-19 Vaccine Trials.

IMPORTANCE: Informed consent is a fundamental element of research ethics. The COVID-19 vaccine trials are high profile trials that have enrolled more than 100 000 participants. Consent documents must be succinct and understandable to ensure informed voluntary participation. OBJECTIVE: To assess how well informed consent documents of the COVID-19 vaccine trials achieve the ideal of being succinct and understandable, and to create a shorter, more readable document. DESIGN, SETTING, AND PARTICIPANTS: This quality improvement study collected and analyzed the informed consent documents used in 4

Drug re-purposing to improve outcomes in the management of prostate cancer - aims, outcome measures and design of current phase III trials.

Prostate cancer remains a major cause of cancer morbidity and mortality and is rising in incidence across the world. Although a succession of randomised controlled trials have improved outcomes across all stages of disease, there remain significant clinical challenges and unmet needs. Repurposed drugs have attracted interest in the treatment of prostate cancer, where a number of agents have been proposed with a range of mechanisms of action suggesting potential benefits. A major advantage of such drugs is the wealth of pre-existing patient safety data typically available, meaning repurposed ag

Old Drug, New Data: Broadening the Therapeutic Armamentarium for Desmoid Tumors.

Desmoid tumors are a locally invasive neoplasm that can exhibit unpredictable clinical behavior. Although treatment options are numerous, few are supported by level 1 evidence. A recent phase III trial evaluated the efficacy of long-used pegylated liposomal doxorubicin in patients with progressive desmoid tumors. See related article by Xu et al., p. 2198.

Vaccine efficacy estimands and power considerations.

The ICH E9(R1) addendum stresses the importance of clearly pre-specifying clinically interpretable treatment effect measures (estimands) and proposes different strategies to deal with intercurrent events. In this paper, we consider different estimands of the vaccine efficacy that are interpretable even when the proportional hazard assumption is not met, including estimands based on the average hazard ratio, the cumulative incidence ratio, and the restricted mean time lost ratio. Under the assumption that different estimands target relevant clinical questions, power becomes a crucial factor in

Cumulative Logit Ordinal Regression With Proportional Odds Under Nonignorable Missing Responses-Application to Phase III Trial.

Missing data are inevitable in clinical trials, and trials that produce categorical ordinal responses are not exempted from this. Typically, missing values in the data occur due to different missing mechanisms, such as missing completely at random, missing at random, and missing not at random. Under a specific missing data regime, when the conditional distribution of the missing data is dependent on the ordinal response variable itself along with other predictor variables, then the missing data mechanism is called nonignorable. In this article, we propose an expectation maximization based algo

Documentation of patient withdrawals, retention strategies, and postwithdrawal data practices in cancer clinical trials.

BACKGROUND: The withdrawal of patients from cancer clinical trials with unspecified rationale introduces selection bias, compromises study validity, and reduces generalizability. Excluding these data can lead to informative censoring, masking treatment toxicity, or inflating efficacy estimates. Whereas regulatory agencies emphasize documenting reasons for withdrawal and retaining data after withdrawal, adherence to these guidelines is unclear, raising concerns about trial integrity. The objectives of this study were to determine the prevalence of withdrawal with unspecified rationale, evaluate

Investigating the Latest Evidence from Phase III Trials Supporting Treatment Options for De novo Clinically Lymph Node-Positive Hormone-Sensitive Prostate Cancer.

PURPOSE OF REVIEW: The introduction of PSMA-PET/CT scans is expected to increase the incidence of clinically lymph node-positive metastatic hormone-sensitive prostate cancer (mHSPC). The 8th AJCC-TNM classify disease with metastasis limited to pelvic nodes (cN1M0) and nonregional lymph nodes (M1a) as stage IV. To date, there is limited prospective evidence for management of this subgroup. Additionally, no specific recommendations currently exist for managing M1a as a distinct condition but as a part of CHAARTED low volume disease (LVD). Our review examines relevant results from phase III trial

Expanding the frontiers of therapeutic options in hidradenitis suppurativa: The valid contribution of bimekizumab.

Hidradenitis suppurativa (HS) is a chronic inflammatory cutaneous disorder with a significant negative impact on quality of life. Th17 axis has a central role in the pathogenesis of HS. Kimball et al. demonstrated the efficacy and safety of bimekizumab in two double-blind, placebo-controlled phase 3 studies (BE HEARD I-II), adding a new targeting option to the therapeutic armamentarium of HS.1.

Dynamic Risk Prediction of Treatment Discontinuation Using Patient-Reported Outcomes Data in the Phase III NSABP B-35 Trial.

UNLABELLED: Predicting an individual's risk of treatment discontinuation is critical for the implementation of precision chemoprevention. We developed partly conditional survival models to predict discontinuation of tamoxifen or anastrozole using patient-reported outcome (PRO) data from postmenopausal women with ductal carcinoma in situ enrolled in the NSABP B-35 clinical trial. In a secondary analysis of the NSABP B-35 clinical trial PRO data, we proposed two models for treatment discontinuation within each treatment arm (anastrozole or tamoxifen treated patients) using partly conditional Cox

What Can IBD Specialists Learn from IL-23 Trials in Dermatology?

BACKGROUND AND AIMS: The advent of biologic drugs revolutionised the treatment of many chronic inflammatory diseases in rheumatology, dermatology, and gastroenterology. The introduction of different targeted agents closely followed the increase in knowledge of pathogenic mechanisms. The identification of IL-23 as a master regulator of 'pathogenic' inflammation and the consequent efficacy of IL-23 blocking agents were first proofed in psoriasis and then in other inflammatory diseases such as psoriatic arthritis and Crohn's disease. METHODS: We reviewed all available results from anti-Il-23 cli

Safety evaluation of ruxolitinib for treating myelofibrosis.

INTRODUCTION: In 2005, the JAK2 V617F mutation was identified and found to be highly prevalent in the 'Philadephia Chromosome-negative' Myeloproliferative neoplasms (MPN). This led to new diagnostic criteria for MPN in addition to the development of the first targeted therapy for myelofibrosis (MF), ruxolitinib . AREAS COVERED: Ruxolitinib was approved within 5 years of 'first-in-man' trials; it has been assessed in two large Phase III trials, and to date, several thousand patients have been prescribed this drug. This article reviews the latest data from the Phase III trials concerning effica
