Jail-based reentry programming to support continued treatment with medications for opioid use disorder: Qualitative perspectives and experiences among jail staff in Massachusetts.

BACKGROUND: Individuals with opioid use disorder released to communities after incarceration experience an elevated risk for overdose death. Massachusetts is the first state to mandate county jails to deliver all FDA approved medications for opioid use disorder (MOUD). The present study considered perspectives around coordination of post-release care among jail staff engaged in MOUD programs focused on coordination of care to the community. METHODS: Focus groups and semi-structured interviews were conducted with 61 jail staff involved in implementation of MOUD programs. Interview guide develo

Developing patient-reported measures of serious illness care quality: synthesis of input from patients, family caregivers, health care professionals, and other experts.

BACKGROUND: Experts have highlighted the need for standardized measures of patient and family experiences of serious illness care. To address this need, we developed surveys that can be used to derive patient-reported quality measures for use in assessment of home-based serious illness programs and community-based serious illness care. METHODS: In preparation for field testing of draft patient survey instruments, we conducted an environmental scan of existing surveys and measures; convened an expert panel of primary care providers, survey research experts, patient representatives, leaders of

School-going adolescent girls' preferences and views of family planning services in Phalombe district, Malawi: A descriptive, cross-sectional study.

BACKGROUND: Low uptake of family planning services by adolescent girls remains a public health concern. An estimated 120 out of every 1,000 girls aged 15 to 19 years are having unplanned pregnancies in the sub-Saharan region. Between January and June 2020, the Phalombe District of Malawi reported 3,030 adolescent pregnancies. At this stage, most Malawian schools were closed due to the COVID-19 pandemic. The high rate of adolescent pregnancies prompted the Ministry of Health to provide emergency contraceptives to reduce the number of unplanned pregnancies among adolescents. The provision of eme

Determinants of unmet need for contraceptive method among young married women in Ethiopia: Multilevel analysis of Ethiopia Demographic and Health Survey 2016.

BACKGROUND: The notion of unmet need for family planning indicates the gap between women's contraceptive practice and their reproductive intention. Although universal access to sexual and reproductive health services including contraceptive methods is a bedrock for sustainable development goals, the unmet need for contraception is high among young women in low-income countries including Ethiopia. The unmet need for contraception is associated with unintended pregnancy which most of the time end in unsafe abortion. Hence, this study aimed to assess the determinants of unmet need for family plan

Subtypes and service utilization among opioid use disorder patients at a community health center: findings from a medically underserved urban area of the Northeastern United States.

BACKGROUND: Opioid use disorder often co-occurs with other mental health and substance use disorders. Identifying clusters of individuals receiving treatment for opioid use disorder based on co-diagnosed conditions, healthcare plans, and service utilization over a seven-year treatment period provides insight into service needs. Objectives included [1] characterizing the sample [2], examining subtypes of the sample using cluster analysis, and [3] identifying differences in Current Procedural Terminology by subtype to examine service utilization among identified clusters. METHODS: This study us

Improving maternal and child healthcare programme using community-participatory interventions in Ebonyi State Nigeria.

In Nigeria, the government is implementing the Free Maternal and Child Health Care Programme (FMCHCP). The policy is premised on the notion that financial barriers are one of the most important constraints to equitable access and use of skilled maternal and child healthcare. In Ebonyi State, Southeastern Nigeria the FMCHCP is experiencing implementation challenges including: inadequate human resource for health, inadequate funding, out of stock syndrome, inadequate infrastructure, and poor staff remuneration. Furthermore, there is less emphasis on community involvement in the programme impleme

"So what's gonna happen when I get out of here?" Qualitative barriers and facilitators to MOUD continuity post-release from jail.

BACKGROUND: Medication for opioid use disorder (MOUD) treatment is increasingly available within jails, but little is known about patient experiences regarding treatment continuity post-release. METHODS: Between 2021 and 2022, we interviewed 38 recently-released people who received MOUD in Massachusetts county jails. We asked participants about their experiences with reentry and MOUD continuity post-release. Thematic analyses were framed by the social-ecological model. RESULTS: At the individual-interpersonal level, participants reported the importance of self-advocacy during incarceration t

Participant feedback on a health care transition conference for youth with medical complexity.

INTRODUCTION: Each year in the United States, an estimated 1.2 million youth with chronic conditions or functional limitations reach adulthood and must transition from pediatric to adult health care, a process often fragmented by service loss, limited adult providers, and complex waiver and financial systems. The University of Alabama at Birmingham developed the Staging Transition for Every Patient (STEP) Program which includes an annual Healthcare Transition Conference. Here we describe the conference and analyze two years of attendee feedback. METHODS: Conference planning followed an iterat

Lessons Learned from the Pathways to Community Health Study to Evaluate the Transition of Care from Jail to Community for Men with HIV.

Time spent in jail can provide opportunities to deliver comprehensive medical care, including screening and treatment for HIV; however, engagement in HIV care postrelease is often fragmented. Identifying ways to improve the transition of care from jail to community for people with HIV (PWH) may help with engagement in HIV care postrelease. We evaluated the current HIV care transition processes of one jail in Massachusetts and identified change ideas to facilitate improving the transition of care from the jail to the community for PWH. We conducted qualitative interviews in 2018-2019 with incar
