Development and evaluation of a procedure for fitting multi-channel compression hearing aids.

Hearing aids with multi-channel compression are often fitted on the basis of loudness scaling data obtained using narrow bands of noise or tones. Here, we report the development and evaluation of an alternative fitting procedure based on the use of speech signals. The parameters of the hearing aid (the gains in each channel for high and low input levels) are adjusted adaptively under computer control on the basis of the listener's responses. The goal is that speech at 85 dB SPL should be judged as 'loud', speech at 60 dB SPL should be judged as 'quiet', and speech at both levels should be judg

Diagnosing and Prognosing Bone Metastasis in Prostate Cancer: Clinical Utility of Blood Biomarkers.

Bone metastasis (BM) may occur in any type of cancer. The diagnosis of patients with BM has increased due to improved imaging technologies and advances in cancer drug therapy that have prolonged the survival time of cancer patients. BM may be asymptomatic in the early stages; however, as the disease progresses, it causes pain, fracture, neurological symptoms associated with spinal cord compression, hypercalcemia, and other specific symptoms that significantly impair the patient's quality of life (QOL). Imaging modalities have disadvantages, such as high cost, radiation exposure, and bone pain

Comparison of Frequency Transposition and Frequency Compression for People With Extensive Dead Regions in the Cochlea.

The objective was to determine the effects of two frequency-lowering algorithms (frequency transposition, FT, and frequency compression, FC) on audibility, speech identification, and subjective benefit, for people with high-frequency hearing loss and extensive dead regions (DRs) in the cochlea. A single-blind randomized crossover design was used. FT and FC were compared with each other and with a control condition (denoted 'Control') without frequency lowering, using hearing aids that were otherwise identical. Data were collected after at least 6 weeks of experience with a condition. Outcome m

Integrated Wireless Sensor System Featuring Liquid-Repellent Interfaces for Reliable Pressure Ulcer Monitoring.

Continuous interface pressure monitoring is essential for proactive pressure ulcer (PU) prevention, yet the reliability of flexible capacitive sensors is frequently compromised by the accumulation of sweat and biofluids in clinical settings. The fundamental challenge arises from the significant dielectric mismatch between aqueous fluids and sensor materials, where fluid infiltration leads to severe signal distortion. Here, we present a liquid-immune wireless sensor system that leverages a robust superhydrophobic interface to eliminate this interference. The sensor unit features a hierarchical

[Interventionnal bronchoscopy for the treatment of tracheobronchomalacia].

Tracheobronchomalacia is usually characterized by more than 50% expiratory narrowing in diameter of the trachea and the bronchi. The expiratory collapse includes two entities: (1) the TBM related to the weakness of the cartilaginous rings, and (2) the Excessive Dynamic Airway Collapse (EDAC) due to the excessive bulging of the posterior membrane. Patients have nonspecific respiratory symptoms like dyspnea and cough. Diagnosis is confirmed by dynamic tests: flexible bronchoscopy and/or computed tomographic scan of the chest. There are different forms of tracheobronchomalacia in adults: primary

[Deep vein thrombosis and pulmonary embolism : Diagnosis and treatment].

This review summarizes current evidence and guideline recommendations concerning diagnosis and treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE). For the diagnostic pathway, evidence-based algorithms should be employed, based on the assessment of pretest clinical probability. D‑dimer tests may reduce the need for subsequent diagnostic procedures. Clinical management of PE is guided by risk stratification according to early mortality. Lactate levels may be helpful for further risk stratification. The acute treatment of venous thromboembolism (VTE) is commenced with intensified

SPINAL ANAESTHESIA-INDUCED HYPOTENSION IN OBSTETRICS: PREVENTION AND THERAPY.

Regional centro-axial anaesthesia, primarily spinal block, is the preferred method of anaesthesia for elective caesarean section because it entails fewer risks for the mother and the foetus compared to general anaesthesia. The most common side effect associated with spinal block is hypotension due to sympatholysis, occurring in up to 75% of cases. Spinal block-induced sympatholysis leads to vasodilatation and consequently causes maternal hypotension, which may compromise uterine blood flow and foetal circulation, and thus cause foetal hypoxia, bradycardia and acidosis. The selection of the mos

Early diagnosis of femoral neck stress fractures may decrease incidence of bilateral progression and surgical interventions: A case report and literature review.

INTRODUCTION: Early intervention in femoral neck stress fractures (FNSFs) can be self- limiting, but they have an insidious presentation. High index of suspicion for an occult fracture is necessary to avoid bilateral progression and/or operative interventions. CASE PRESENTATION: A 83-year-old female, non-athlete, presents with multiple comorbidities and progressive bilateral FNSFs. Initially the patient presented with groin pain and radiographs demonstrated a non-displaced compression type fracture of the right femoral neck without any inciting events. Subsequently, during a post-operative vi

An efficient hybrid spectral-compact difference scheme for rod-coil diblock copolymers in slit confinement.

Self-consistent field theory simulations of rod-coil diblock copolymers in slit confinement present significant numerical challenges due to sharp density gradients near hard walls. To rigorously resolve these systems utilizing the Gaussian and wormlike chain models, a hybrid spectral-compact finite difference scheme is developed on a non-uniform Chebyshev-Gauss-Lobatto grid. Shen's Chebyshev spectral method is employed for the flexible blocks. For the semiflexible blocks, a second-order upwind compact scheme together with an L-stable TR-BDF2 contour-stepping algorithm is adopted. This hybrid f

Machine learning-driven multi-objective optimization of sustainable engineered cementitious composites: Balancing performance and CO2 reduction via municipal solid waste incineration bottom ash (MSWIBA).

This study addresses the dual challenge of enhancing the mechanical performance and environmental sustainability of Engineered Cementitious Composites (ECC) by replacing natural sand with Municipal Solid Waste Incineration Bottom Ash (MSWIBA). Fifteen ECC mixtures were experimentally tested across varying W/B ratios (0.24-0.30) and BA replacement levels (0-100%). Results showed BA inclusion decreased flowability and strength, with compressive strength dropping from 74.1 MPa (control) to 32.7 MPa (full replacement at W/B = 0.24). Optimal experimental performance, characterized by robust strain-

Quantification of the effect of osteolytic metastases on bone strain within whole vertebrae using image registration.

The vertebral column is the most frequent site of metastatic involvement of the skeleton with up to 1/3 of all cancer patients developing spinal metastases. Longer survival times for patients, particularly secondary to breast cancer, have increased the need for better understanding the impact of skeletal metastases on structural stability. This study aims to apply image registration to calculate strain distributions in metastatically involved rodent vertebrae utilizing µCT imaging. Osteolytic vertebral lesions were developed in five rnu/rnu rats 2-3 weeks post intracardiac injection with MT-1

Comparison of deep LSTM and machine learning models for predicting compressive strength of fly ash/slag-based geopolymer concrete.

In the production of geopolymer concrete (GPC), using ground granulated blast furnace slag (GGBFS) and fly ash (FA) can reduce the carbon dioxide footprint and decrease the amount of waste materials released into the environment. Finding the compressive strength (fc) of GPC through experiments is time-consuming and costly; thus, applying artificial intelligence models can expedite this process. This study aims to compare the performance of deep Long Short-Term Memory (LSTM) and the machine learning (ML)-based algorithms in predicting the fc of FA/GGBFS-based GPC. Artificial neural networks (AN

Design, characterisation, and clinical evaluation of a novel porous Ti-6Al-4V hemipelvic prosthesis based on Voronoi diagram.

Three-dimensional printed Ti-6Al-4V hemipelvic prosthesis has become a current popular method for pelvic defect reconstruction. This paper presents a novel biomimetic hemipelvic prosthesis design that utilises patient-specific anatomical data in conjunction with the Voronoi diagram algorithm. Unlike traditional design methods that rely on fixed, homogeneous unit cell, the Voronoi diagram enables to create imitation of trabecular structure (ITS). The proposed approach was conducted for six patients. The entire contour of the customised prosthesis matched well with the residual bone. The porosit
