Behcet's disease and AA-type amyloidosis.

Bacterial communities in and on wild hosts are increasingly appreciated for their importance in host health. Through both direct and indirect interactions, bacteria lining vertebrate gut mucosa provide hosts protection against infectious pathogens, sometimes even in distal body regions through immune regulation. In house finches (Haemorhous mexicanus), the bacterial pathogen Mycoplasma gallisepticum (MG) causes conjunctivitis, with ocular inflammation mediated by pro- and anti-inflammatory cytokines and infection triggering MG-specific antibodies. Here, we tested the role of gut bacteria in ho

[The experience with the supervision and treatment of the patients presenting with external otitis].

This clinical study was designed to estimate the efficacy and tolerability of the locally applied Pimafucort preparation for the treatment of diffuse otitis of bacterial and mycotic origin. The results of dynamic bacteriological investigation in conjunction with clinical observations give reason to recommend the local therapy as the sole treatment of uncomplicated forms of diffuse external otitis.

What is causing active trachoma? The role of nonchlamydial bacterial pathogens in a low prevalence setting.

PURPOSE: In low prevalence settings, clinically active follicular trachoma (TF) is often found in the absence of detectable Chlamydia trachomatis. The reasons for this persistent follicular phenotype are not well understood; one possible explanation is that other bacterial species are provoking the inflammatory response. This study investigated the relationship between TF, C. trachomatis, and nonchlamydial bacterial infection. METHODS: A cross-sectional survey was conducted in a trachoma endemic village in Tanzania. All available children were examined for trachoma and swabs were collected fo

[Gonococcal conjunctivitis in a young woman].

We describe the management of a female patient who developed an uncomplicated unilateral purulent conjunctivitis with no other clinical signs. The typical clinical presentation and Gram stain of the discharge suggested gonococcal conjunctivitis, allowing treatment to be initiated. Indeed, a strain of Neisseria gonorrhoeae resistant to penicillin and tetracycline was isolated. In collaboration with the patient's primary care physician, management included lavage of the infected eye, systemic antibiotic treatment with erythromycin and topical antibiotic treatment with azithromycin, followed by l

Escherichia hermannii as the sole isolate from a patient with purulent conjunctivitis.

Escherichia hermannii was isolated in pure culture from a patient with acute purulent conjunctivitis after a minor ocular injury. This is the first report of E. hermannii isolated as the sole pathogen from an infected site without prior antibiotic exposure, confirming the pathogenic potential of the microorganism.

How do GPs diagnose and manage acute infective conjunctivitis? A GP survey.

OBJECTIVE: To determine GPs' diagnosis and management of acute infective conjunctivitis (AIC)-one of the commonest but least researched acute infections seen in primary care. METHODS: A postal questionnaire survey of 300 GPs from two Health Authorities in Southern England. RESULTS: 236 (78%) GPs returned the questionnaire. 92% of those responding felt confident or very confident in the diagnosis of AIC. 95% usually prescribe topical antibiotics for AIC despite 58% stating that they thought at least half of the cases they see are viral in origin and only 36% believing that they could discrimi

Presence of cytomegalovirus and herpes simplex virus in middle ear fluids from children with acute otitis media.

Twenty-seven (10%) of 271 infants and children with acute otitis media (AOM) were found to be infected with cytomegalovirus (CMV) or herpes simplex virus type 1 (HSV). CMV or HSV, alone or in combination with bacteria or other viruses, was isolated from the middle ear fluid (MEF) of 10 patients. In three cases, CMV alone was isolated from the MEF, and in one case, HSV alone was isolated. One of the CMV cases involved an acute primary or reactivation of CMV infection, with CMV-bacterial otitis and conjunctivitis as major manifestations. One patient with AOM and stomatitis had purulent otitis as

[Determination of resistance and antibiotic therapy in bacterial eye diseases of childhood].

From June 1982 to December 1983 children aged between 3 days and 5 years (average 2-1/2 years) with bacterial conjunctivitis, blennorrhea or perforated corneal ulcers were examined microbiologically. The results of 30 antibiotic susceptibility tests were compared with those of adults. Prevalence of Escherichia coli (6%) and Staphylococcus aureus haemolyticus (17%) was observed; Candida species was not found in the young children. Pediatric diseases revealed themselves to be more pronounced in general. The authors diagnosed 3 chlamydial blennorrheas in newborn aged 7, 8 and 10 days. Mixed infec

A Diagnostic Challenge: Periorbital or Orbital Cellulitis?

The authors present the case of a 39-year-old male who presented to the hospital with worsening eye pain, swelling, and blurred vision of the left eye. His symptoms grew worse despite initial over-the-counter medication and a trip to the urgent care center. A physical exam was concerning for a possible orbital cellulitis given the appearance of the eye and the amount of discomfort, as well as their immunocompromised status, necessitating imaging and workup to confirm the diagnosis. The patient was ultimately diagnosed with periorbital cellulitis and bacterial conjunctivitis, and he received in

A progressive and severe rash - Answers.

A 73-year-old female patient with epilepsy presented to hospital with a progressive, diffuse macular rash over the trunk and limbs with associated mucosal blistering and discharge. Ocular symptoms initially predominated and she was treated for presumed bacterial conjunctivitis by her General Practitioner the previous day. On the acute medical unit supportive management was initiated for suspected adverse drug reaction (ADR) to a recent lamotrigine dose increase. Skin biopsy confirmed a diagnosis of toxic epidermal necrolysis. We present this case to highlight the importance of medication histo

Identification of Moraxella lacunata from pulmonary abscesses in three zoo herbivores.

Although Moraxella lacunata causes conjunctivitis, keratitis, endocarditis, and otolaryngitis in humans, its infection is rare in animals. We report three cases of asymptomatic pulmonary abscesses caused by M. lacunata in zoo herbivores, including two elks (Cervus canadensis) and a common eland (Taurotragus oryx). In all cases, macroscopic findings included coalescence of lung lobes and severe pulmonary abscesses filled with cheese-like materials in cysts. Microscopic findings included pneumonia characterized by marked fibrin exudates in alveolar spaces and infiltration of inflammatory cells.

Chlamydia trachomatis Infection in Pregnancy: The Global Challenge of Preventing Adverse Pregnancy and Infant Outcomes in Sub-Saharan Africa and Asia.

Screening and treatment of sexually transmitted infections (STIs) in pregnancy represents an overlooked opportunity to improve the health outcomes of women and infants worldwide. Although Chlamydia trachomatis is the most common treatable bacterial STI, few countries have routine pregnancy screening and treatment programs. We reviewed the current literature surrounding Chlamydia trachomatis in pregnancy, particularly focusing on countries in sub-Saharan Africa and Asia. We discuss possible chlamydial adverse pregnancy and infant health outcomes (miscarriage, stillbirth, ectopic pregnancy, pret

Methicillin-resistant Staphylococcus aureus induced reactive arthritis treated with systemic corticosteroid.

Reactive arthritis consists of the classic clinical triad of arthritis, urethritis and conjunctivitis generally occurring within 6 weeks of an infection, typically of the gastrointestinal or genitourinary systems. Staphylococcus aureus is not usually implicated in this condition. Staphylococcal septicemia, while frequently associated with arthralgia, has rarely been associated with a sterile arthritis, although it infrequently results in septic arthritis. It is important to consider reactive arthritis, rather than solely an infectious cause of joint effusions, and arthropathy in a patient with

Stevens-Johnson Syndrome triggered by chemical hair relaxer: a case report.

This case report describes a 41-year-old Afro-Caribbean lady presenting with a constellation of pyrexia, conjunctivitis, arthralgia, sterile dysuria, apthous ulceration, labial crusting and widespread erythema multiforme. A diagnosis of Stevens-Johnson Syndrome was made. She had taken no medications recently (the most common precipitant of Stevens-Johnson Syndrome) and a full screen for the common and atypical bacterial and viral triggers was negative. The identified trigger was the use of a chemical hair relaxant treatment a few days previously. With supportive measures and a course of oral p

Conservative Management of Eyelid Necrotizing Fasciitis in a Pediatric Patient With Concurrent Parinaud's Oculoglandular Syndrome.

Parinaud's oculoglandular syndrome is an uncommon manifestation of Bartonella henselae infection presenting with granulomatous conjunctivitis, marked preauricular lymphadenopathy, and occasional necrotizing lesions. The most common cause of Parinaud's oculoglandular syndrome is due to direct contact with contaminated cat saliva or scratches near the eye. While Parinaud's oculoglandular syndrome has been linked to cat scratch disease, it has not been associated with necrotizing soft tissue infections due to a distinct bacterial process, such as necrotizing fasciitis due to Group A Streptococcus

Design and evaluation of proniosomes as a carrier for ocular delivery of lomefloxacin HCl.

The current investigation aims to develop and evaluate novel ocular proniosomal gels of lomefloxacin HCl (LXN); in order to improve its ocular bioavailability for the management of bacterial conjunctivitis. Proniosomes were prepared using different types of nonionic surfactants solely and as mixtures with Span 60. The formed gels were characterized for entrapment efficiency, vesicle size, and in vitro drug release. Only Span 60 was able to form stable LXN-proniosomal gel when used individually while the other surfactants formed gels only in combination with Span 60 at different ratios. The opt
