Single cell transcriptional profiling of monocytes from asthma patients show a predisposition for an inflammatory response.

BACKGROUND: Asthma is the most common chronic lung disorder among children and young adults worldwide. Despite effective new biologic therapies, ∼10% of patients remain poorly controlled. Monocytes contribute to the proinflammatory milieu in asthmatic lungs but also suppress inflammation and promote tissue healing. AIMS: Compare the transcriptome of blood monocytes in asthma patients and healthy controls and assess the impact of an infusion of interferon y-activated mesenchymal stromal cells (yMSCs) on the transcriptome of asthmatic monocytes. METHODS: scRNA-seq was conducted on blood monocy

Elimination of routine chest X-rays after lung resection: a quality improvement initiative.

BACKGROUND: Performing a postoperative chest X-ray (CXR) is a routine practice following lung surgery. Despite studies suggesting that CXRs may be unnecessary for most patients, no initiative has been taken to discontinue this practice. Our initiative aims to discontinue routine CXR after lung resections and evaluate the safety of this approach to improve the quality of care. METHODS: We conducted a single-center, retrospective, controlled before-after study. Patients who underwent lung resection between January 2022 and May 2023, excluding pneumonectomy and spontaneous pneumothorax, were inc

Prevention and Treatment of Hyponatremia in Patients with Subarachnoid Hemorrhage: A Systematic Review.

BACKGROUND: Current guidelines for the management of hyponatremia in patients with subarachnoid hemorrhage (SAH) are not based on a systematic assessment of the literature. We evaluated published studies on the efficacy and safety of current preventative and treatment strategies for hyponatremia in patients with SAH. METHODS: We searched the Cochrane Central Register of Controlled Trials, Embase, MEDLINE, and PubMed for relevant studies. Primary outcomes of interest included neurologic functional outcomes, symptomatic vasospasm, and others. Secondary outcomes included measures of sodium and v

[Clinical study PEPCA : The effect of standardized preoperative patient education for patient-controlled regional analgesia on postoperative pain].

BACKGROUND: Patient-controlled analgesia (PCA) is a well-established form of postoperative pain management. One form of administration is patient-controlled regional analgesia (PCRA), where local anesthetics are administered via peripheral regional catheters; however, a prerequisite is that the patients are instructed on its use. A multitude of sources recommend that these instructions are given before surgery as preoperative training on pain management procedures has been shown to significantly reduce patients' postoperative pain and increase their well-being. OBJECTIVE: The aim was to asses

Effects of calcium infusion on blood pressure in hypertensive and normotensive humans.

Disorders of calcium and parathyroid hormone homeostasis have been reported in subjects with essential hypertension. In many of these studies, dietary intakes of sodium and calcium were not carefully controlled. The present study was designed to compare calcium and parathyroid hormone homeostasis in normal and hypertensive subjects on controlled dietary sodium and calcium intakes and to examine the impact of dietary sodium loading on hemodynamic and metabolic responses to infused calcium. Seven subjects with essential hypertension and seven age-matched and sex-matched controls were studied whi

Prevention of postoperative delirium in elderly patients planned for elective surgery: systematic review and meta-analysis.

Introduction: Vulnerable or "frail" patients are susceptible to the development of delirium when exposed to triggers such as surgical procedures. Once delirium occurs, interventions have little effect on severity or duration, emphasizing the importance of primary prevention. This review provides an overview of interventions to prevent postoperative delirium in elderly patients undergoing elective surgery. Methods: A literature search was conducted in March 2018. Randomized controlled trials (RCTs) and before-and-after studies on interventions with potential effects on postoperative delirium in

Sanguinate's effect on pial arterioles in healthy rats and cerebral oxygen tension after controlled cortical impact.

Sanguinate, a polyethylene glycol-conjugated carboxyhemoglobin, was investigated for cerebral vasoactivity in healthy male Sprague-Dawley rats (Study 1) and for its ability to increase brain tissue oxygen pressure (PbtO2) after controlled cortical impact (CCI) - traumatic brain injury (TBI) (Study 2). In both studies ketamine-acepromazine anesthetized rats were ventilated with 40% O2. In Study 1, a cranial window was used to measure the diameters of medium - (50-100μm) and small-sized (<50μm) pial arterioles before and after four serial infusions of Sanguinate (8mL/kg/h, cumulative 16mL/kg IV)

Gastroesophageal Reflux Disease Following Roux-en-Y Gastric Bypass.

Background: Gastroesophageal reflux disease (GERD) is one of the most important obesity-related comorbidity, with prevalence >50% in obese population. Roux-en-Y gastric bypass (RYGB) is considered the gold standard for metabolic surgery in obese patients with GERD, but in a subgroup of patients this pathological GERD may be not really controlled after this technique. Aims of this article are to discuss surgical and endoscopic options to manage refractory GERD after RYGB. Materials and Methods: We realized a literature review using the PubMed database and searching articles published before Dec

Metabolic Signatures of Performance in Elite World Tour Professional Male Cyclists.

BACKGROUND AND OBJECTIVE: Metabolomics studies of recreational and elite athletes have been so far limited to venipuncture-dependent blood sample collection in the setting of controlled training and medical facilities. However, limited to no information is currently available to determine if findings in laboratory settings are translatable to a real-world scenario in elite competitions. The goal of this study was to define molecular signatures of exertion under controlled exercise conditions and use these signatures as a framework for assessing cycling performance in a World Tour competition.

Effect of clinical guidelines in nursing, midwifery, and the therapies: a systematic review of evaluations.

BACKGROUND: Although nursing, midwifery, and professions allied to medicine are increasingly using clinical guidelines to reduce inappropriate variations in practice and ensure higher quality care, there have been no rigorous overviews of their effectiveness, 18 evaluations of guidelines were identified that meet Cochrane criteria for scientific rigor. METHODS: Guideline evaluations conducted since 1975 which used a randomised controlled trial, controlled before and after, or interrupted time series design were identified through a combination of database and hand searching. RESULTS: 18 stud

Effect of fusion following decompression for lumbar spinal stenosis: a meta-analysis and systematic review.

PURPOSE: The surgical methods of degenerative lumbar spinal stenosis include spinal decompression with or without instrumented or non-instrumented spinal fusion. Previous meta-analysis and systematic reviews have reported the contrast between surgical management and nonsurgical management for degenerative lumbar spinal stenosis, while no literature did among surgical managements. And it is evidenced that whether fusion should be added to spinal decompression in patients of lumbar spinal stenosis is still divisive. So our purpose is to identify whether spinal fusion with or without decompressio

Living-donor liver transplantation for moderate or severe porto-pulmonary hypertension accompanied by pulmonary arterial hypertension: a single-centre experience over 2 decades in Japan.

BACKGROUND: Candidates for orthotopic liver transplantation (OLT) often have porto-pulmonary hypertension (PPHTN) with pulmonary arterial hypertension (PAH). Poor outcomes of PPHTN contraindicate OLT. There are no guidelines for living-donor liver transplantation (LDLT) in PPHTN patients. METHODS: We present our experiences of LDLT in six patients with moderate or severe PPHTN, along with our institutional guidelines. Three had liver cirrhosis and three were non-cirrhotic. Catheterization studies were undertaken before, during and after LDLT, and the mean pulmonary arterial pressure (mPAP), c

ILEOSTIM trial: a study protocol to evaluate the effectiveness of efferent loop stimulation before ileostomy reversal.

PURPOSE: A protective loop ileostomy is the most useful method to reduce sequelae in the event of an anastomotic leakage (AL) after rectal cancer surgery. However, it requires an additional stoma reversal surgery with its own potential complications. Postoperative ileus (POI) remains the most common complication after ileostomy reversal, which leads to an increase in morbidity, length of hospital stay (LOS) and overall healthcare costs. Several retrospective studies carried out in this field have concluded that there are insufficient evidence-based recommendations about the routine application

Effect of Bilateral Ultrasound-Guided Deep Parasternal Intercostal Plane Block Before Extubation on Early Recovery After Cardiac Surgery via Median Sternotomy: A Randomized Controlled Trial.

INTRODUCTION: Postoperative pain remains a major concern after cardiac surgery via median sternotomy and may impair early recovery. Deep parasternal intercostal plane (DPIP) block is a regional analgesic technique targeting the parasternal intercostal nerves, but the effect of DPIP block performed before extubation on postoperative recovery has not been well defined. We therefore evaluated whether bilateral ultrasound-guided DPIP block administered before extubation could improve early recovery after median sternotomy cardiac surgery. METHODS: In this single-center, prospective, randomized, s

Comparison of effectiveness of different surgical treatments for meralgia paresthetica: Results of a prospective observational study and protocol for a randomized controlled trial.

INTRODUCTION: Various surgical procedures can be applied in the treatment of meralgia paresthetica. The two main ones are neurolysis and neurectomy of the lateral femoral cutaneous nerve. To date, no prospective or randomized controlled trial has compared the effectiveness of these procedures with standardized outcome measures. In this study we present our results for two prospectively followed cohorts and we present the protocol for a double blind randomized controlled trial (RCT). METHODS AND ANALYSIS: All patients that had an indication for surgical treatment of idiopathic meralgia paresth

Efficacy of Biomedical Materials for the Prevention of Lymphocele after Lymph Node Dissection for Gynecological Malignancies: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

UNLABELLED: <p>Introduction: Lymphocele is one of the complications after lymphadenectomy for female pelvic malignancies. In recent years, some studies indicated that the use of biomedical materials during surgery for gynecological malignancies may help to reduce the occurrence of lymphatic cysts, but results were inconsistent. The aim of the study was to evaluate the correlation between the use of biomedical materials intraoperatively and formation of lymphocele after lymph node dissection in patients with gynecological malignancies. METHODS: We conducted a systematic search of 4 electronic
