Laparoscopic surgery for benign lesions of the pancreas.

BACKGROUND: Minimally invasive pancreatic surgery, although known to be feasible and safe, is still not considered a standard procedure. We … mean postoperative hospital stay was 5.5 +/- 1.2 days (range, 5-8 days) for the laparoscopic cases. Conversion to open surgery was necessary in 6 patients because of: closeness of the lesion to the portal/mesenteric vein (n = 3), inadequate intraoperative tumor localization (n = 2), or stapler device dysfunction (n = 1). In these patients, open enucleation (n = 1), middle s …

[Endoscopic Retrograde Cholangiopancreatography before Gallbladder Removal - an Unholy Alliance?].

Endoscopic retrograde cholangiopancreaticography (ERCP) is, among other indications, performed in conjunction with cholecystectomy (CHE) for suspected bile duct stones. Due to its invasiveness, ERCP is an endoscopic examination with considerable morbidity and mortality and influence the perioperative outcome after CHE.In this retrospective and monocentric study, all patients who underwent CHE in Baden-Baden from 01.07.2018 to 31.12.2022 were consecutively examined. In this exploratory study, p values < 0.05 were assessed as statistically significant.942 patients with gallbladder surgery (CHE)

[Laparoscopic treatment of hepatic hydatid cysts: techniques and post-operative complications].

INTRODUCTION: We evaluate our experience in the surgical laparoscopic treatment of hepatic hydatid cysts with the same criteria that we use in open surgery. MATERIAL AND METHODS: A retrospective study of 8 operated patients and their intra- and postoperative complications. RESULTS: We performed the scheduled surgery on 7 patients; bleeding was the reason for conversion to open surgery in the remaining one. We made 4 complete peri-cystectomies, 3 de-roofing and 1 hepatic resection. Two patients had postoperative bile leaks: the first one had an external leak that needed an endoscopic sphincte…

Preoperative Risk Factors for Conversion from Laparoscopic to Open Cholecystectomy: A Systematic Review and Meta-Analysis.

Laparoscopic cholecystectomy is a standard treatment for patients with gallstones in the gallbladder. However, multiple risk factors affect the probability of conversion from laparoscopic cholecystectomy to open surgery. A greater understanding of the preoperative factors related to conversion is crucial to improve patient safety. In the present systematic review, we summarized the current knowledge about the main factors associated with conversion. Next, we carried out several meta-analyses to evaluate the impact of independent clinical risk factors on conversion rate. Male gender (OR = 1.907

Laparoscopic cholecystectomy for acute inflammation of the gallbladder.

OBJECTIVE: The aim of this study was to prospectively assess the results of laparoscopic cholecystectomy in patients with acute inflammation of the gallbladder. SUMMARY BACKGROUND DATA: Laparoscopic cholecystectomy has become the standard treatment for symptomatic gallbladder disease. Its role in the surgical treatment of acute cholecystitis has not been defined, although a number of recent reports suggest that there should be few contraindications to an initial laparoscopic approach. METHODS: All patients presenting with symptomatic cholelithiasis from October 1990 until June 1992 were eval

Laparoscopic cholecystectomy in acute cholecystitis: C-reactive protein level combined with age predicts conversion.

INTRODUCTION: The aim was to enable prediction of risk for conversion in early laparoscopic cholecystectomy for acute cholecystitis. METHODS: Multivariate analysis and receiver operating characteristic curve analysis were used to define independent predictors for conversion and optimal cutoffs. Using those, a scoring system was created to predict conversion. RESULTS: In 261 patients, conversion to open cholecystectomy was necessary in 62 cases (24%). Multivariate analysis revealed age and C-reactive protein (CRP) level to be independent predictors for conversion (odds ratio 1.02; P=0.02 and

Robotic emergency surgery in geriatric care.

BACKGROUND: Older adults present unique challenges in emergency general surgery (EGS) due to increased comorbidities and frailty. While minimally invasive surgery (MIS), including laparoscopic (lap) and robotic techniques, reduces perioperative morbidity, the role of minimally invasive EGS in geriatric patients remains unclear. This study compares outcomes for geriatric EGS patients following robotic, lap, and open procedures. We hypothesize that MIS approaches would improve perioperative outcomes in geriatric patients. METHODS: We conducted a retrospective analysis of the ACS-NSQIP database

Multimodal analgesia in abdominal sepsis: a case report of liposome bupivacaine in ultrasound-guided rectus sheath blocks after unexpected open hemi-colectomy.

OBJECTIVE: Contrasted with open laparotomy, the risk-benefit ratio of epidural anesthesia in laparoscopic surgery frequently disfavors its use. The presence of abdominal sepsis also affects utilization with concerns about the risks of epidural abscess and hypotension. In the unplanned conversion to open laparotomy, postoperative pain control becomes more difficult. We discuss utilization of rectus sheath blocks with liposome bupivacaine as a component of multimodal analgesia in a patient with abdominal sepsis from a perforated appendix. CASE REPORT: A 33-year-old healthy male Soldier presente

The short-term outcomes of laparoscopic multivisceral resection for locally advanced colorectal cancer: our experience of 39 cases.

PURPOSE: Laparoscopic surgery for locally advanced colorectal cancer suspected of direct invasion to adjacent organs or structures remains controversial because of its high conversion rate, inadequate oncologic clearance and surgical outcomes. This study retrospectively evaluated the short-term outcomes of laparoscopic multivisceral resection for colorectal cancer and investigated the factors influencing conversion to open surgery. METHODS: Between 2010 and 2015, 39 patients who underwent laparoscopic multivisceral resection for colorectal cancer intraoperatively suspected of direct invasion …

Hepatic Pedicle Occlusion with the Pringle Maneuver During Difficult Laparoscopic Cholecystectomy Reduces the Conversion Rate.

BACKGROUND: In the presence of cholecystitis or portal hypertension, hemorrhage is common during laparoscopic cholecystectomy (LC) because the vessels of Calot's triangle are fragile and tortuous. Bleeding can obstruct surgical field visibility and increase conversion rates and risk of common bile duct injury. The Pringle maneuver is a simple occlusion approach that could limit blood flow from the hepatic pedicle, thus controlling bleeding to provide a clear surgical field to reduce conversion rate. In this study, we aimed to investigate the feasibility, effectiveness and safety of hepatic ped
