Factors associated with the absence of cocaine craving in treatment-seeking individuals during inpatient cocaine detoxification.

BACKGROUND: Anecdotal evidence suggests a substantial proportion of individuals with cocaine use disorder do not report craving during inpatient detoxification. OBJECTIVE: To examine prevalence and clinical correlates of consistent absence of cocaine craving among inpatients during detoxification. We hypothesized that craving absence would be associated with less severity of cocaine use, depression, and anxiety. Alternative explanations were also explored. METHODS: Craving absence (i.e., non-cravers) was defined as a daily score of zero across two separate craving visual analogue scales in e

Effects of a Smartphone-Based Approach-Avoidance Intervention on Chocolate Craving and Consumption: Randomized Controlled Trial.

BACKGROUND: Repeatedly pushing high-calorie food stimuli away based on joystick movements has been found to reduce approach biases toward these stimuli. Some studies also found that such avoidance training reduced consumption of high-calorie foods. OBJECTIVE: This study aimed to test effects of a smartphone-based approach-avoidance intervention on chocolate craving and consumption, to make such interventions suitable for daily use. METHODS: Within a 10-day period, regular chocolate eaters (n=105, 86% female) performed five sessions during which they continuously avoided (ie, swiped upward) c

Effects of repeated transcranial direct current stimulation on smoking, craving and brain reactivity to smoking cues.

Recent studies have shown that transcranial direct current stimulation (tDCS) may reduce craving and smoking. However, little is known regarding brain correlates of these behavioral changes. We aimed to evaluate whether 10 sessions of tDCS modulate cigarette consumption, craving and brain reactivity to smoking cues in subjects with tobacco use disorder (TUD). In a double blind parallel-arms study, 29 subjects with TUD who wished to quit smoking were randomly assigned to receive 10 sessions of either active or sham tDCS applied with the anode over the right dorsolateral prefrontal cortex (DLPFC

Craving Behavior from Opioid Addiction Controlled with Olanzapine in an Advanced Cancer Patient: A Case Report.

Opioid addiction, although uncommon in cancer patients, can be a significant challenge for optimal pain management in certain patients. We present a case of a 59-year-old man with advanced colon cancer whose compulsive craving for the buccal tablet of fentanyl citrate (BTFC) was improved with the use of olanzapine. He was hospitalized for abdominal pain caused by disease progression. He had visited several times at outpatient follow-up to obtain a prescription for BTFC because he took all medications before the appointed times. After admission, intravenous infusion of oxycodone and opioid rota

Transcranial direct current stimulation of the frontal-parietal-temporal area attenuates cue-induced craving for heroin.

BACKGROUND: Transcranial direct current stimulation (tDCS) is an effective approach to modulate brain region functions. We assessed if a single tDCS session over the bilateral frontal-parietal-temporal (FPT) areas would reduce cue induced craving in heroin addicts. METHODS: Twenty non-treated, long-term heroin-addicted subjects were randomly assigned to receive either real tDCS (1.5 mA, cathodal over bilateral FPT for 20 min) or control tDCS stimulation (turning off the stimulation after 30 s). The participants received heroin cue exposure (containing both injection and inhalation procedures)

Symptoms of posttraumatic stress predict craving among alcohol treatment seekers: results of a daily monitoring study.

Alcohol use disorders (AUDs) and Posttraumatic Stress Disorder (PTSD) commonly co-occur. Craving for alcohol is a common aspect of AUD, with and without PTSD, and is one of the key predictors of continued problematic alcohol use among treatment seekers. The present study sought to investigate the self-medication hypothesis using daily Interactive Voice Response (IVR) reports to examine the relationships between PTSD symptomatology and both same-day and next-day alcohol craving. Twenty-nine individuals with an AUD (26 of whom screened positive for PTSD) entering AUD treatment provided daily IVR

Time-dependent neuroendocrine alterations and drug craving during the first month of abstinence in heroin addicts.

RATIONALE: Heroin use and withdrawal cause abnormality in the endocrine system. However, the time course of neuroendocrine alterations in heroin addicts during pharmacologically unassisted withdrawal is still unclear. OBJECTIVES: To investigate alterations in cortisol, adrenocorticotrophic hormone (ACTH), beta-endorphin (beta-EP), leptin, and neuropeptide Y (NPY) during the first month of abstinence in heroin addicts. METHODS: Twelve heroin addicts and eight matched healthy control subjects were recruited for this study. The neuroendocrine alterations and self-reported heroin craving, anxiet

Investigating craving as an indicator of early response to buprenorphine treatment among adults with opioid use disorder.

OBJECTIVE: Reduction of craving is a key mechanism of buprenorphine treatment for opioid use disorder but has not been examined as an indicator of early response versus nonresponse. We aimed to (a) identify cut points for craving in the first week of buprenorphine treatment that best predicted later sustained opioid use and (b) examine associations between early nonresponse based on craving and opioid-related, retention, and psychosocial outcomes. METHOD: Data were from two buprenorphine trials (N = 562; Mage = 34 years; 38% female). Craving was rated on a 0-10 visual analog scale in each of

Acute effects of aerobic and resistance exercise on craving and cognitive function in individuals with heroin use disorder.

Heroin use disorder (HUD) presents significant global health challenges, with cravings, emotional dysregulation, and cognitive impairments being key barriers to successful recovery. This study explores the acute effects of aerobic and resistance exercise on craving, emotional state, attentional bias, and executive functions in individuals with HUD. A crossover design was employed with 30 male participants diagnosed with HUD, each completing three interventions: high-intensity aerobic exercise (AEC), resistance exercise (REC), and a sedentary control condition (SCC). Assessments were conducted

Naltrexone Is Superior to Placebo for Abstinence and Craving Reduction in Alcohol-Associated Cirrhosis: NAL-CI Trial.

BACKGROUND AND AIMS: Alcohol use disorder (AUD) coexisting with cirrhosis carries high morbidity and mortality, with no approved pharmacotherapy for AUD. We evaluated the safety and efficacy of naltrexone, an opioid receptor antagonist, in patients with compensated alcohol-associated cirrhosis (AaC) and AUD. METHODS: One hundred patients with compensated AaC and DSM-5 AUD were randomised 1:1 to naltrexone (50 mg/day) or placebo for 12 weeks. The primary endpoint was point-prevalence abstinence at 12 weeks, defined as no alcohol use in the four preceding weeks. Secondary endpoints included cra

Associations between posttraumatic cognitions and cannabis cravings among trauma-exposed individuals using cannabis.

OBJECTIVE: Trauma-exposed individuals with posttraumatic stress disorder (PTSD) symptoms are at risk for problematic cannabis use. However, modifiable risk factors associated with cannabis use in this population are less clear. Posttraumatic cognitions (PTC; negative cognitions about the self, self-blame, and negative cognitions about the world) have recently been investigated as potential motivators for substance use, particularly to cope with distress, but extant research is limited. The current study tested whether PTC are associated with increased coping-oriented cannabis cravings using an

Role of Drinking Attitudes and Trait Impulsivity in Prenatal Alcohol Craving and Consumption in Mothers of Reproductive Age.

Recognizing the severe consequences of alcohol consumption during pregnancy, such as fetal alcohol spectrum disorders (FASDs), the present study explored the role of drinking attitudes, trait impulsivity, and decision-making toward instant gratification in alcohol craving and consumption during pregnancy among mothers of reproductive age. Utilizing participants from Amazon Mechanical Turk (N = 141), we first categorized mothers into three groups: those who neither craved nor consumed alcohol during their last pregnancy, those who craved but did not consume, and those who craved and consumed al

The role of craving in gambling behavior: examining its relevance and links to psychological distress, personality, and demographics.

BACKGROUND: Several studies have sought to investigate gambling craving and its potential contribution to gambling problems. One aim of this line of research has been to further examine whether craving should be considered a separate diagnostic criterion for gambling disorder, which it currently is not. The present study aimed to investigate the relationship between craving and other risk factors—such as mental health symptoms, affect, and personality traits—and to determine whether craving makes a unique contribution to problematic gambling when controlling for these factors. METHODS: In tota

Naltrexone and its Effects on Craving and Alcohol Use among Patients with Alcohol Dependence Syndroms: A Report.

BACKGROUND: Naltrexone is a Food and Drug Administration (FDA)-approved anti-craving agent for the long-term treatment of alcohol dependence syndrome (ADS). However, it may not be equally effective in all patients. This study aims to assess naltrexone treatment response over four weeks in a national-level tertiary care setting. METHODS: Male patients with ADS (n=100) who were initiated on naltrexone were included in the study. The clinical data, including the drinking pattern and craving, were recorded at baseline. At the end of the one-month follow-up, the drinking status and compliance with

Dopamine transmission at D1 and D2 receptors in the nucleus accumbens contributes to the expression of incubation of cocaine craving.

Relapse represents a consistent clinical problem for individuals with substance use disorder. In the incubation of craving model of persistent craving and relapse, cue-induced drug seeking progressively intensifies or 'incubates' during the first weeks of abstinence from drug self-administration and then remains high for months. Previously, we and others have demonstrated that expression of incubated cocaine craving requires strengthening of excitatory synaptic transmission in the nucleus accumbens core (NAcc). However, despite the importance of dopaminergic signaling in the NAcc for motivated

Distinct influences of nicotine and sensorimotor stimuli on reducing cravings to smoke and vape among dual users.

INTRODUCTION: When considering the clinical efficacy of e-cigarettes for smoking cessation, non-pharmacologic influences, such as conditioned reinforcers (e.g. sensorimotor stimuli), must be considered in addition to nicotine. The present study parsed the influences of nicotine delivery and sensorimotor stimuli (i.e, similarity to smoking) on cravings and other immediate outcomes of e-cigarette use. METHOD: Participants (N = 127 dual users) completed an experimental ad-lib vaping session in one of 4 conditions: Drug (open label nicotine vs non-nicotine e-cigarettes) crossed with delivery appa
