A 63-year-old man had symptoms and a history suggestive of a manic episode. Meningeal cryptococcosis with normal pressure hydrocephalus was later found to be the underlying cause. Antifungal treatment resulted in sustained improvement of all mental functions.

A case of pulmonary cryptococcosis initially diagnosed by cytologic examination of sputum is presented. The cytologic findings are reviewed, with a brief discussion of the clinical settings in which pulmonary cryptococcosis occurs. The role of the cytologist in the early detection of this important and often clinically occult pulmonary mycotic disease is emphasized.

Widely disseminated cryptococcosis was found on autopsy in a 50-year-old alcoholic man. The spleen, lungs, CNS, liver, kidney, and lymph nodes were all involved. In his clinical course, the patient showed no signs of immunologic anergy. His terminal hospital course resulted directly from end-stage liver disease and renal failure. The cryptococcal infection was of the nongranulomatous, diffuse type with little or no inflammatory response, which probably explains the lack of symptoms.

A case of cerebral cryptococcosis associated with pulmonary and bone lesions is described. While discussing some general data of the literature, the authors outline the rarity of granulomatous CNS Cryptococcosis. From a clinical standpoint the similarity with tuberculous processes is emphasized. The methods of histopathologic diagnosis both intraoperatively and in permanent sections are discussed.

Immunologic studies were performed in two patients with sarcoidosis, who developed cryptococcosis. Polymorphonuclear leukocyte function, complement, and serum antibodies were normal. Both patients had depressed cell-mediated immunity (cmi) to Cryptococcus neoformans and other antigens that persisted after therapy for their infection. These findings suggest that the patients' impaired CMI predisposed them to cryptococcal infection, which complicated their sarcoidosis. Evaluation of sarcoidosis cases should include studies of immune function, and the possibility of a secondary infection should b…

A new differential colour medium prepared with an extract of potatoes and carrots produced ochre-brown pigmentation in Cryptococcus neoforma … in the contaminated specimens was credited largely to the differential medium used. Twelve cases of cryptococcosis were investigated between 1962 and 1968, five of which presented involvement of the respiratory tract only.

PURPOSE OF REVIEW: Cryptococcus neoformans was recently declared a top priority fungal pathogen because of its propensity to cause a life-threatening meningoencephalitis, which continues to have high mortality and morbidity despite antifungal therapy. Our goal is to review recent developments in antifungal therapy while synthesizing how these are integrated into advances in understanding the pathophysiology of cryptococcosis.
RECENT FINDINGS: The therapeutic outcomes for cryptococcosis continue to improve but the disease still carries an unacceptably high mortality and morbidity. Advances in …

There is a consensus that the antifungal repertoire for the treatment of cryptococcal infections is limited. Standard treatment involves the administration of an antifungal drug derived from natural sources (i.e., amphotericin B) and two other drugs developed synthetically (i.e., flucytosine and fluconazole). Despite treatment, the mortality rates associated with fungal cryptococcosis are high. Amphotericin B and flucytosine are toxic, require intravenous administration, and are usually unavailable in low-income countries because of their high cost. However, fluconazole is cost-effective, wide…

This is a case of disseminated cryptococcosis with gastrointestinal and thyroid involvement. This case illustrates that gastrointestinal and thyroid involvement may be an under-recognized phenomenon. Suspicion should be high in an immunocompromised patient with odynophagia and a thyroid nodule and warrants further investigation by endoscopy and fine needle aspiration biopsy.

Cryptococcus neoformans is an encapsulated yeast found worldwide.1 Patients with immunosuppression, including individuals with HIV/AIDS, tra … itative efforts. A post-mortem diagnosis revealed Cryptococcus neoformans fungemia and disseminated cryptococcosis involving multiple organs. Disseminated cryptococcosis primarily affects the central nervous system3, and thus this report presents a rare case of disseminated cryptococcosis involving multiple organs.

The encapsulated fungus Cryptococcus neoformans (Cn) causes cryptococcal meningitis (CM). There are ~180,000 deaths per year worldwide attributed to CM, which is the most common cause of meningitis in adults with HIV in sub-Saharan Africa. HIV infection with advanced immunodeficiency is the most important predisposing risk factor for CM, highlighting the critical role that T cell mediated immunity plays in disease prevention. Numerous studies in the past decade demonstrate that antibody immunity also plays a role in resistance to CM, although its role has taken more time to establish. In mice,

PURPOSE OF REVIEW: To perform an extensive review of recent literature and provide an update on the current epidemiology, clinical features and management of cryptococcal disease with a focus on the differences between patients depending on their immune status.
RECENT FINDINGS: Emerging literature has highlighted the inflammatory pathophysiology and varied manifestations of cryptococcal infections in patients who are apparently healthy but paradoxically have a more critical clinical course compared to their immunosuppressed counterparts.
SUMMARY: Non-HIV cryptococcal meningitis has greater m

Isavuconazole is a broad-spectrum azole that is FDA-approved for the treatment of aspergillosis and mucormycosis; data on the use of isavuconazole for the treatment and prevention of other invasive fungal infections are limited. Here, we report a patient with pulmonary cryptococcosis treated with isavuconazole who experienced progression to disseminated infection with Cryptococcus while on isavuconazole. Caution is advised when using isavuconazole in situations where there is a paucity of data to recommend its use.
