Dacryocystantrostomy. A preliminary report.

A new operative technique is described in which the lacrimal sac is drained into the maxillary sinus. The advantages of the procedure, especially of avoiding a facial incision, are emphasized. Five patients suffering from long-standing epiphora underwent surgery. Two patients had chronic dacryocystitis. Two other patients presented with a mucocele of the lacrimal sac. In the fifth patient, there was obstruction in the nasolacrimal duct. Short term follow-up results suggest its usefulness as a first line of surgical treatment in cases of epiphora due to obstruction in the lacrimal sac or the na…

Congenital Dacryocystocele: A Major Review.

PURPOSE: To provide a systematic review of the literature on congenital dacryocystoceles (CDCs) and summarize their presentations, investiga … t 7 days of birth. Initial conservative treatment can be a viable option in the absence of an acute dacryocystitis or a respiratory distress. Endoscopy-assisted probing appears to have better outcomes as compared with the in-office probing. Congenital dacryocystoceles with acute dacryocystitis are preferably managed with intravenous antibiotics and an early prob …

Preliminary experience with exoscope in lacrimal surgery.

BACKGROUND: Endoscopic dacryocystorhinostomy (e-DCR) is the mainstay for lacrimal sac/duct conditions. The purpose is to investigate the role of the exoscope as assisting tool in e-DCR. METHODS: Primary endpoint were symptoms resolution (epiphora/dacryocystitis) and time for surgery. Qualitative features of the exoscope were analyzed: a questionnaire administered to the surgical team allowed to evaluate the perceived quality of this technology. CONCLUSIONS: The exoscope is a new tool that may support e-DCR. It has comparable results on symptoms outcomes and time for surgery than classic e-DC…

Surgical treatment of familial dacryocystocele and lacrimal puncta agenesis.

Bilateral agenesis of the lacrimal puncta and enlargement of the nasolacrimal canal are rare anatomic variants. The authors present 2 familial cases: a 39-year-old woman with bilateral dacryocystocele and lacrimal puncta agenesis and her 46-year-old brother, who had a long history of epiphora and recurrent dacryocystitis, and also had bilateral lacrimal puncta agenesis and a left dacryocystocoele. The authors report the endoscopic, CT, and MRI findings, and describe the surgical treatments, by endoscopic dacryocystorhinostomy in the first case, and conjunctivodacryocystorhinostomy with Jones t…

Expulsion of a lacrimal dacryolith by sneezing: a rare event.

PURPOSE: To describe the case of a man presenting with intermittent epiphora of the right eye and a low grade dacryocystitis due to a dacryolith. METHODS/RESULTS: He was treated conservatively and was offered a Dacryocystorhinostomy. A few weeks before surgery, he developed swelling and pain over the right medial canthus followed by violent sneezing with spontaneous extrusion of the dacryolith. DISCUSSION: The authors propose that the dacryolith fragmented spontaneously and once it reached the nasal cavity, was expelled by violent sneezing to the nostril, a mechanism not previously reported.…

Symptomatic lacrimal duct diverticulum associated with closed nasal fracture.

Diverticula of the nasolacrimal collecting system are rare. Their cause may be congenital, inflammatory, or traumatic. A diverticulum of the nasolacrimal duct that presented 26 years after nasal trauma is described. The location of this diverticulum is extremely uncommon, and a case occurring after closed nasal trauma has not yet been presented. Evaluative studies and pertinent anatomy are reviewed. In any patient with recurrent dacryocystitis after nasal fracture, the diagnosis of a lacrimal system diverticulum should be considered. A detailed history and physical examination combined with da…

Dacryocystorhinostomy with nasolacrimal duct prostesis.

For many years the standard surgical treatment for chronic dacryocystitis has been the anastomosis of the lacrimal sac mucosa to nasal mucosa over the margins of an ostium through the lacrimal bone. Failures of this standard procedure have been attributed to many factors and complications frequently require reoperation. The procedure described in this paper was developed as one means of overcoming some of these difficulties. It consists of the implantation of a specially shaped nasolacrimal duct prosthesis from the base of the sac through the interosseous canal into the inferior nasal meatus. …

Retrograde Flow of Fluid Through the Nasolacrimal Duct System Under General Anesthesia: A Case Report.

This case report describes a rare incident of fluid exhibiting retrograde flow from the nasopharynx through the nasolacrimal duct and accumulating within the eye guard while under general anesthesia. The patient was in a steep Trendelenburg position for several hours and received multiple liters of intravenous fluid. The patient did not have a history of sinusitis, sinus surgery, cocaine use, dacryostenosis and dacryocystitis, or nasolacrimal duct irrigation and probing. This case provides insight into the potential ophthalmic implications of surgical and anesthetic management. While the patie…

In this case report, two patients were initially presented with orbital symptoms due to undiagnosed sinonasal disease. An eight-year-old girl was suspected of preseptal cellulitis due to swelling and redness around the right eye for a month. MRI of the orbit showed a change in the orbit suspected to be a tumour. Regression in symptoms was seen after three months and a mucocele was suspected. The other patient was a 57-year-old woman suspected of left-side acute dacryocystitis with a palpable mass above the medial canthus for a month. MRI was performed due to atypical presentation and showed tu…

Hereditary angioedema type 2 presented as an orbital complication of acute rhinosinusitis.

Hereditary angioedema is an autosomal dominant and life-threatening disorder characterized by recurrent episodes of non-pitting edema affecting the skin, respiratory system and digestive tracts and caused by a congenital deficiency or function defect of the C1 esterase inhibitor. Preseptal cellulitis is defined as an infection of the tissues of the anterior orbital septum. It is generally caused by complications from an upper respiratory tract infection, dacryocystitis, dermal infection, and, rarely, sinusitis. The disease presents with orbital pain, edema on the eyelids, erythema, and fever. …

Progress in screening and treatment of common congenital eye diseases.

Congenital eye illnesses are caused by congenital ocular malformations and are a primary cause of poor visual acuity and blindness in infants. Early diagnosis and treatment of congenital eye illnesses are of great significance for affected infants, their families, and even society as a whole. This study describes the current situation for prenatal and infant screening for congenital eye diseases and briefly summarizes novel progress in the treatment of the five most common eye diseases (congenital dacryocystitis, congenital cataract, retinopathy of prematurity, congenital glaucoma and retinobl…

Nasolacrimal drainage system cyst in an adult.

A cyst of the nasolacrimal drainage system (NLDS) is rare in an adult. We report a case in a 29-year-old man of a mucous retention cyst of the NLDS, which appeared 2 years after the patient developed dacryocystitis coincident with an aggravation of chronic sinusitis. The lesion was successfully managed by removing the bony wall of the NLDS at the interface with the ethmoid and nasal cavity, as well as the membranous closure of the ostium of the common canaliculus. We speculate that the cyst development might have been initiated by inflammatory change such as mucosal adhesion in the NLDS result…

Congenital nasolacrimal duct obstruction in Japanese infants: its incidence and treatment with massage.

We examined the patency of 670 nasolacrimal ducts in 335 newborn infants--42 patients (62 ducts) had obstructions; the involvement was unilateral in 22 and bilateral in 20. No sexual predilection was noted. All affected patients were treated with hydrostatic massage at the area of the lacrimal sac. About two-thirds of the ducts became patent by 2 months after birth, while the remainder became patent by 9 months. Only one patient who had acute dacryocystitis at age 5 months required a probing of the duct and a regimen of systemic antibiotics to achieve good results.

Complications by liquid plastics in ophthalmic surgery. Histopathologic study.

Retrospective morphological studies of ocular tissues after treatment with liquid plastics reveal many complications induced by the material. Cyanoacrylates for punctal occlusion glide into the lacrimal sac and lead to obstructive dacryocystitis and even to reactive xanthogranuloma. The use of this glue in combination with a hard contact lens as 'artificial epithelium' provokes a remarkable stromal and endothelial cell loss of the cornea. Intraocular silicone oil sometimes induces a massive epiretinal macrophage activity with storage of the material or reactive fibrosis when it gets into the s…

Sarcoidosis of the lacrimal sac.

Two women with acute dacryocystitis were found to have noncaseating granulomatous inflammation of the lacrimal sac and nasal mucosa. One patient had chronic uveitis and the other had skin lesions and generalized lymphadenopathy histologically compatible with sarcoidosis. The diagnosis was supported by elevation of serum levels of angiotensin-converting enzyme. To our knowledge, these are the sixth and seventh histologically confirmed cases of sarcoidosis of the lacrimal sac. They differ from previously reported cases in the absence of classic roentgenographic changes of sarcoidosis. Analysis o…
