BACKGROUND: Neonatal hypernatremic dehydration (NHD) is a severe condition associated with neonatal morbidity and mortality.
PURPOSE: The present study evaluated maternal risk factors, including duration of maternal hospitalisation, primiparity, caesarean section, and pregnancy complications, as well as social factors, such as depression, fatigue, and inadequate support for NHD.

Five cases of moderately severe hypernatraemic dehydration were identified within a 5-month period between two regional hospitals in Hong Kong. Unlike previous reported cases, these exclusively breast-fed infants presented with the unusual triad of fever, absence of overt signs of dehydration and within the first week of life. Patients with acute low-tone sensorineural hearing loss were treated by rheology or dehydration after recording of the summation and compound action potentials by electrocochleography. Patients treated in the acute phase, as well as patients with chronic disease (that is the interval between onset of hearing loss and therapy was more than 1 week) clearly responded better to dehydration than to conventional rheological treatment. This was especially striking in those patients showing an enhanced SP/AP ratio indicating endolymphatic hydrops.

We studied the role of the sodium-potassium pump in erythrocytes of 12 patients with sickle cell anemia (SS). Ouabain-binding sites per cell and pump-mediated Rb/K uptake were significantly higher in SS patients than in white or black controls. Ouabain-resistant Rb/K influx was also greater than in normal controls or patients with sickle cell trait. This prospective study comprises 40 infants with severe hypernatremic dehydration due to gastroenteritis. During the first 24h, natremia was closely monitored and infusion rates were adjusted so as to keep the rate of fall in natremia below 0.5 mEq/l/h. This could be achieved by giving a 70 mEq/l Na solution at the rate of 120 ml/kg/24 h. Rehydration was uneventful in all cases, and no convulsions were observed.
