The rehabilitation of patient function and esthetics is essential from a social and psychological point of view. After a maxillary resection, the supporting structures that help provide retention or stability to a removable prosthesis are lacking. An effective, functional, straightforward, comfortable, and esthetic prosthetic rehabilitation of a patient with a resected maxilla using a digital workflow is presented. The rehabilitation was done using a computer-aided design and computer-aided manufactured bar and clip-supported overdenture prosthesis over quad zygomatic implants.

The effect of five different treatments for retained permanent molars was evaluated in 59 patients to develop a rational guideline for adequate therapy. The results showed that a prosthetic buildup is the proper treatment if retention develops after the growth spurt, because in these cases the extent of infraocclusion is slight and relatively stable. To prevent caries and periodontal disease in overdenture abutment teeth, daily application of chlorhexidine gel is recommended. In order to get insight into the use of the gel, 29 overdenture wearing patients were interviewed. In addition the patients were requested to show the use of the gel. It was concluded that at any time the patients were instructed about the use of the gel. Nevertheless, a majority of the patients did not use the gel daily and in accordance with the instructions given. Application of the gel in the abutment depressions of the overdenture appeared to be a heavy task.

The results are reported on 34 edentulous patients, who underwent interposed autogenous bone graft augmentation in the symphysis of the mandible, combined with bone-hydroxylapatite onlay augmentation of the area posterior to the mental foramina. Two to four implants were placed in the grafted symphysis after 3-5 months. An overdenture was constructed three months later. The follow-up period ranged from one to seven years. An average loss of mandibular bone height of 10-13% was observed. The data provided no evidence of a further time-dependent resorption from two-and-half to seven years.

In implant patients who have extended soft tissue deficiencies, split skin grafts have been performed for the prevention of peri-implant inflammation.
