Efficacy of dupilumab in atopic comorbidities associated with moderate-to-severe adult atopic dermatitis.

BACKGROUND: Dupilumab is an anti-IL-4Rα antibody used in the treatment of patients with moderate-to-severe atopic dermatitis (msAD). This study explored the potential benefit of dupilumab in perennial allergic rhinoconjunctivitis (PAR) and perennial allergic asthma (PAA) caused by indoor allergens in adults with msAD. METHODS: This multicentric, prospective, observational, real-life study included adult patients with msAD who had been treated with dupilumab in 16 Italian care centres. Efficacy outcomes regarding AD, PAR and PAA were collected at baseline and 16 weeks. Safety was also assessed

Economic burden of atopic manifestations in patients with atopic dermatitis--analysis of administrative claims.

BACKGROUND: Atopic dermatitis (AD) has been associated with atopic manifestations (AMs), such as food allergies, asthma, allergic rhinitis, and allergic conjunctivitis. OBJECTIVES: To (1) compare the risk of developing AMs in patients with AD versus those without AD, (2) estimate the incremental costs attributable to AMs in patients with AD, and (3) examine the factors associated with incremental costs. METHODS: In this retrospective cohort study, the authors used MarketScan research databases containing medical and pharmacy claims with dates of service from January 1, 1999, to December 31,

Ocular findings in atopic dermatitis with special reference to the clinical features of atopic cataract.

44 patients with atopic dermatitis who had had active generalized skin lesions, especially of the face, following transient remission after eczema in infancy were examined ophthalmologically. Their ocular findings have been correlated with the clinical features of atopic dermatitis. 25% of the patients with this clinical course had cataracts. However, the clinical features of the patients without cataracts were almost the same as those of the patients with. Thus, cataracts in patients with atopic dermatitis may not be due to the clinical symptoms and course but to other factors such as genetic

[Translated article] Drug Survival in Cyclosporine Treatment for Moderate to Severe Atopic Dermatitis: Analysis of the Spanish Atopic Dermatitis Registry (BIOBADATOP).

BACKGROUND: The past 5 years have seen a proliferation of new treatments for atopic dermatitis (AD). We analyzed recent drug survival data for cyclosporine in this setting. Because the Spanish National Healthcare system requires patients with AD to be treated with cyclosporine before they can be prescribed other systemic treatments, drug survival for cyclosporine may be shorter than in other diseases. MATERIAL AND METHOD: Multicenter, observational, prospective cohort study using data from the Spanish Atopic Dermatitis Registry (BIOBADATOP). Data from the Spanish Registry of Systemic Treatmen

Atopic dermatitis results in intrinsic barrier and immune abnormalities: implications for contact dermatitis.

Atopic dermatitis (AD), as well as irritant contact dermatitis (ICD) and allergic contact dermatitis (ACD), are common skin diseases. These diseases are characterized by skin inflammation mediated by activated innate immunity or acquired immune mechanisms. Although AD, ICD, and ACD can be encountered in pure forms by allergists and dermatologists, patients with AD often present with increased frequency of ICD and ACD. Although a disturbed barrier alone could potentiate immune reactivity in patients with AD through increased antigen penetration, additional immune mechanisms might explain the in

Type I sensitization in adolescents: prevalence and association with atopic dermatitis. The Odense Adolescence Cohort Study on Atopic Diseases and Dermatitis (TOACS).

The prevalence of Type I sensitization and its relationship to atopic dermatitis were assessed in a cohort of 1501 8th grade schoolchildren (aged 12-16) in Odense, Denmark. The protocol included a questionnaire, a clinical examination, IgE measurements and skin prick tests. A history of atopic dermatitis was found in 21.3%, allergic asthma in 6.9% and allergic rhinitis in 15.7% of the adolescents. One or more positive specific IgE measurements (CAP FEIA) were found in 29.6% of the schoolchildren (inhalant allergens 28.4%, food allergens 8.5%, pityrosporum ovale 1.5%) and a considerable proport

The ACVD task force on canine atopic dermatitis (X): is there a relationship between canine atopic dermatitis and cutaneous adverse food reactions?

In humans, allergies to foods are known to induce skin lesions in some patients with atopic dermatitis. This is particularly evident in infants with severe atopic dermatitis. Food allergy in humans is an IgE-mediated hypersensitivity in most cases, and thus has the same (or very similar) pathogenic mechanism of disease induction as environmental allergen-induced atopic dermatitis. Cutaneous adverse food reactions and atopic dermatitis in dogs are often indistinguishable from each other on historical and clinical grounds alone. Limited current evidence suggests that dogs with cutaneous adverse

The Role of and Therapeutic Strategies for Eosinophils in Atopic Dermatitis.

Atopic dermatitis (AD) is a chronic inflammatory skin disease driven by immune dysregulation and epidermal barrier dysfunction, in which eosinophils act as key effector cells contributing to tissue damage and persistent inflammation. This comprehensive review elucidates the multifaceted contributions of eosinophils to the progression of AD. Driven by key type 2 cytokines (notably IL-4, IL-5, and IL-13) and specific chemokines, eosinophils infiltrate lesional skin and undergo IgE-mediated degranulation. The subsequent release of cytotoxic granule proteins, including major basic protein (MBP), e

Defining the Potential for Disease Modification in Atopic Dermatitis.

Atopic dermatitis is a common inflammatory disorder affecting the skin, often associated with a strong disease burden and a long-term impact on patients' quality of life. Although most treatments for atopic dermatitis have been mainly focusing on alleviating symptoms, emerging therapies and approaches may offer the potential to modify the disease course, thereby leading to off-treatment remission and prevention of comorbidities. However, a consensus on the definition of disease modification in atopic dermatitis is yet to be reached. The aim of this article is to review the concept of disease m

Off-label treatment of atopic dermatitis and psoriasis.

OBJECTIVES: Off-label prescribing is a common and often necessary practice in dermatology, as approved treatment options frequently fail to meet the diverse needs of patients. This is particularly relevant in the management of two prevalent inflammatory dermatoses - atopic dermatitis and psoriasis. Despite the widespread use of off-label therapies, their application often lacks formal guidance, highlighting the need for updated clinical recommendations and consideration of expanded drug indications. METHODS: This retrospective study analyzed medical records of 5072 patients with atopic dermat

Special Considerations in Atopic Dermatitis in Young Children.

Atopic dermatitis (AD) begins in early childhood in the majority of children. Addressing AD in small children includes recognition of the early presentations of disease in all skin types, triggers, comorbidities, and therapeutics. These include risk of medication absorption, more xerosis, infections, and creating management plans that are acceptable to parents/caregivers, while offering safety. Vaccination efficacy, safety on systemic agents, growth and development, tactile sensory development, and teething-related facial eruptions of early childhood are additional concerns. Prevention of long

Precision medicine in atopic dermatitis: Present and future.

Atopic dermatitis (AD) is a chronic, multifactorial inflammatory skin disease characterized by barrier dysfunction, immune dysregulation, genetic predisposition, and environmental factors, including microbiome and allergens. As more is learned about its pathophysiology, treatment evolves using precision medicine-leveraging phenotype, endotype, biomarkers, disease severity, and response predictors, molecular profiling, and omics. Oral and topical corticosteroids may be effective and most accessible, but their use has been associated with various adverse effects. Cyclosporine also has proven eff

Biological drugs for the treatment of children with atopic dermatitis.

INTRODUCTION: Atopic dermatitis (AD) is a chronic inflammatory skin disease that frequently affects children and is associated with significant morbidity and impaired quality of life. Recent advances in our understanding of AD immunopathogenesis have led to the development of targeted biological and small-molecule therapies. AREAS COVERED: This review focuses on the available and emerging targeted therapies for pediatric AD, linking mechanistic rationale to currently available and emerging biologics and JAK inhibitors and highlighting the limits of pediatric evidence, real-world data, and tre

Patient Experience and Preferences for Treatments in Atopic Dermatitis.

INTRODUCTION: Although type 2 inflammatory pathway dysfunction is a hallmark of atopic dermatitis (AD), disease symptoms in many patients are also driven by other immune/inflammatory pathway dysfunctions, all of which lead to varied treatment response profiles of targeted systemic therapies for individual patients. Adding complexity to the process of identifying which systemic therapy to prescribe for an individual patient is that these treatments have varied safety profiles, routes of administration, monitoring requirements, and costs-all of which are reviewed during today's shared decision-m

The significance of component-resolved diagnostics in atopic dermatitis.

Atopic dermatitis (AD) is a common and chronic inflammatory skin disease characterized by an abnormal skin barrier, resulting in skin dryness, pruritus, and an increased risk of allergies and secondary infections. AD patients often show hypersensitivity to both food and airborne allergens. Component-resolved diagnostics (CRD) offers sIgE testing of individual allergen molecules and provides additional insights, especially in polysensitized patients, in case of sensitization to allergens with low abundance, low stability, or associated with risks of anaphylaxis. It enables the detection of genu
