Knowledge of the inverted "T" incision--used in plastic, oncologic and orthopedic surgery--has allowed its adaptation for the diagnostic assessment and therapeutical approach of acral, nodular lesions. The authors describe the use of this technique for the surgical approach of a patient with a plantar nodular lesion, further diagnosed as a calcified angioleiomyoma.

BACKGROUND: Barbed sutures provide a novel technique for knotless wound closure. They have mainly been used in plastic and general surgery. There are no reports about its use in the foot.
METHODS: We present a case series of 11 feet in 8 patients whose wounds were closed with barbed sutures. Ten had primary hallux valgus correction and one had a revision correction.

Bronchogenic cyst is a benign congenital developmental abnormality of the embryonic foregut. The most common extrapulmonary location of these lesions is the mediastinum. Over 50 cases of cutaneous bronchogenic cyst were reported in the English literature. Nevertheless, it is poorly recognized by clinicians and is sometimes confused with branchial cyst. A procedure for soft tissue release through double incisions in clubfoot is described. It has been practiced in 287 cases of severe, neglected, resistant, and relapsed clubfeet in the last 7 years. Generally, it has been found to be a useful approach without risking any skin necrosis.

Severe swelling and skin necrosis are not uncommon following foot and ankle surgery, particularly trauma surgeries. A skin suture technique is presented that has the advantages of being simple, providing good skin eversion and apposition with minimal tension and risk of ischemia, and being easy to remove. A new method of treating syndactyly is described. The fingers are separated by graduated steps in a special apparatus using skeletal fixation of the phalanges with wires. Skin grafting is then carried out. Scarring and deformity are avoided. The operation can be used for very young children and our patients were aged from 5 months to 30 years. The results of 60 operations are reviewed: 56 were good, 3 satisfactory and 1 unsatisfactory. The advantages of the procedure are discussed.

In a three-year study, the scalpel Nd:YAG laser was compared with conventional surgery in 18 patients who underwent oculoplastic surgery. The postoperative follow-up was at least eight months. A method of primary closure following excision of pilonidal cysts and sinus was used in 38 cases. The criteria for patient selection are described as is the technique and procedure. Special attention must be placed on the obliteration of all "dead spaces" to eliminate accumulation of serum and blood. The dressing is not changed for a minimum of eight days, and pressure must be maintained for that time to assure success. In all cases complete closure was obtained. The advantages of this technique over the open procedure is explained.

OBJECTIVE: Various skin incision methods have been reported for reduction mammoplasty and mastopexy. This report describes a new incision method that may improve on conventional methods, particularly with respect to prevention of hypertrophic scars.

In mammary tumors that are large relative to breast-size, complex reconstructive methods are required. We describe an alternative system of closure.
