[Surgical treatment of aneurysms of the thoracic aorta. Presentation of personal cases].

Six cases of aneurysm of the thoracic aorta, four located in the 1st segment and two in the 3rd, are reported. The four cases in the ascending aorta presented a different aetiology: a) aneurysm on aortic stenosis (case 1); b) atheromatous aneurysm (case 2); c) aortic dissection (case 5); d) aneurysm due to dystrophy of the media in a patient with Marfan's disease (case 6). 3 tubular prosthetic substitutions of the ascending aorta were made (with a valvular substitution and commissural relocation associated with right aorto-coronary venous by-pass) and one plastic intervention on the aorta with

Left Main Stenosis Stenting Normalises Wall Shear Stress of Ascending Aorta in Bicuspid Aortic Valve.

INTRODUCTION: Bicuspid aortic valve (BAV) is associated with dilation and dissection of the ascending aorta. The high shear forces within the ascending aorta lumen seem to have a pivotal role on the development of such complications. We describe the time-averaged wall shear stress (TAWSS) forces in a patient with normally functioning BAV and significant ostial/mid-shaft left main (LM) stenosis using computational fluid dynamic analysis (CFD). CASE REPORT: A 47-year-old female patient with normally functioning BAV with fusion of right and non-coronary cusps was investigated for unstable angina

Should sinus of Valsalva be preserved in patients with bicuspid aortic valve and aortic dilation?

BACKGROUND: It is recommended that dilated ascending aortas (≥45 mm) should be replaced at the time of aortic valve replacement (AVR) for bicuspid aortic valve (BAV). The risk of progressive sinus of Valsalva dilatation after AVR and ascending aorta replacement is less clear. METHODS: We identified 156 patients (age 56.2±10.8 years, 46 females) who underwent AVR and ascending aorta replacement in our institution from 2010-2014, 124 (79%) of whom had BAV. Aortic root and ascending aorta sizes were determined from preoperative and the most recent echocardiograms. The mean follow-up time was 34.

Transseptal guidewire stabilization facilitates stent-graft deployment for persistent proximal ascending aortic dissection.

PURPOSE: To report endovascular repair of a DeBakey type I ascending aortic dissection. CASE REPORT: Five months after aortocoronary bypass grafting, a 56-year-old woman demonstrated a persistent ascending aortic type I dissection whose entry tear proximal to the innominate artery spiraled down the descending thoracic aorta into the iliofemoral arteries. Repair was facilitated with a guidewire passed through a transseptal sheath in the right femoral vein. The guidewire traversed the right and left atria and the left ventricle and then exited into the ascending aorta and was exteriorized throu

[Spontaneous Rupture of the Ascending Aorta;Report of a Case].

A 66-year-old woman was transferred to our hospital on an emergency basis complaining of a sudden onset of chest discomfort. An emergency contrasted computed tomography showed a ruptured the ascending aorta near the right pulmonary artery with a hematoma and notch in the aortic wall. An emergency operation of the graft replacement was performed via standard median sternotomy under the cardiac arrest with aortic clamp. Upon the operation, there was a 1.5 cm rupture site in the central portion of the ascending aorta. There was no specific evidence of aortic aneurysm or dissection, therefore a sp

[Diagnostic goals in aortic dissection. Value of transthoracic and transesophageal echocardiography].

The combination of different ultrasound techniques like transthoracic, suprasternal, subcostal and transesophageal echocardiography have a high sensitivity and specificity in the diagnosis of aortic dissection. The limitation of this combined ultrasound technique is related to the visualization of the ascending part of the aortic arch which, cause of the interposition of the trachea, can not be visualized completely. The beginning or the end of a dissection in this part of the aorta may be misinterpreted. False negative results are rare. False positive results due to artefacts resulting from r

Gasdermin D deficiency attenuates development of ascending aortic dissections in a novel mouse model.

BACKGROUND: Thoracic aortic dissection (TAD) is a silent killer. Approximately two-thirds of the cases occur in the ascending aorta (i.e. type A dissection) and majority of them are unrelated to genetic mutations. However, animal models of spontaneous type A dissection are not widely available. In the present study, a novel mouse TAD model was created. Further, the role of gasdermin D (GSDMD) in TAD development was evaluated. METHODS: TADs were created by treating ascending aorta of adult mice (C57BL/6J) with active elastase (40.0 U/ml) and β-aminopropionitrile (Act E+BAPN). The temporal prog

Unexpected Intraoperative Diagnosis of Acute Type A Aortic Dissection in an Octogenarian: Tailored Repair.

BACKGROUND: Acute type A aortic dissection (ATAAD) is a high-mortality surgical emergency, particularly in octogenarians, and may be overlooked with overlapping cardiac symptoms. CASE SUMMARY: An 86-year-old woman with severe aortic stenosis and multivessel coronary artery disease presented with chest pain. Three months earlier, she underwent evaluation for transcatheter aortic valve replacement, but no intervention was pursued. Because of persistent chest pain, surgical aortic valve replacement and coronary bypass were planned. Intraoperatively, the ascending aorta appeared abnormal, and tra

Endovascular Stent Graft Repair of Localized Acute Aortic Intramural Hematoma: A Case Report and Literature Review.

Aortic intramural hematoma (IMH) is a variant of acute aortic syndrome, which can be life-threatening. Ascending aorta IMHs, particularly accompanied by penetrating aortic ulcer (PAU), can cause dissection, rupture, and cardiac tamponade. Therefore, early surgical treatment is recommended for IMHs of the ascending aorta. Herein, we present the case of a 60-year-old male patient who was on warfarin sodium treatment and in whom an IMH localized to the arcus aorta was detected incidentally via computed tomography, with the suspicion of pulmonary embolism, and an endovascular stent graft was inser

Swinging motion of intimal flap through the aortic valve in acute aortic dissection.

The purpose of this article is to present a very rare case of Stanford type A acute aortic dissection featuring a swinging motion of the cylinder-shaped intimal flap through the aortic valve. The patient was a 62-year-old male suffering from severe cardiogenic shock. A transthoracic echocardiogram revealed aortic dissection and severe aortic regurgitation. A transesophageal echocardiogram demonstrated that the aortic dissection in the ascending aorta was circumferential and the proximal portion of the intimal flap was swinging through the aortic valve, ie., falling into the left ventricle duri

Aortic band: a rare mimicker of aortic dissection.

We report a case of 21 years old male with Tetralogy of Fallot who underwent electrocardiogram-gated cardiac computed tomography angiography (CCTA) as part of preoperative evaluation. CCTA revealed a linear hypodense band within the ascending aorta, just proximal to origin of right brachiocephalic trunk, connecting the anterior and posterior walls. This case highlights the importance of CCTA in identifying such uncommon anatomical entities which can mimic aortic dissection on imaging and may predispose to unwarranted emergent surgical explorations, despite their indolent nature.
