Low risk of variant Creutzfeldt-Jakob disease transmission from blood transfusions in Aotearoa New Zealand suggests donor exclusion policies can be relaxed.

Aotearoa New Zealand currently excludes potential blood donors who lived in the United Kingdom (UK) for 6 months or more between 1980 and 1996. This action is due to the potential for variant Creutzfeldt-Jakob disease (vCJD) following blood transfusions from preclinical vCJD cases, who themselves mostly developed disease from the consumption of cattle with bovine spongiform encephalopathy (BSE) during this period, or from those incubating the misfolded prion proteins that cause disease. This donor exclusion policy led to 10% of New Zealand's active blood donors in 2000 being excluded, and it r

Left Lateral Sector In Situ Split Liver Transplantation Technique: Step-by-Step Video Demonstration.

The split liver technique enables transplanting 2 recipients with one single graft (typically an adult-child pair). It facilitates small recipients' access to liver transplantation and reduces mortality on the waiting list. However, splitting is technically demanding and may increase peri- and postoperative complications. To be able to obtain comparable outcomes to a full graft liver transplantation, careful donor-recipients selection, experienced surgeons, and logistic planning are paramount. The video shows an in situ split liver procedure from a 32-year-old brain stem death donor to generat

Impact of COVID-19 on blood donor deferral patterns during the COVID-19 pandemic: A retrospective analysis.

BACKGROUND AND OBJECTIVES: Blood donor deferral is an essential tool for blood safety. The ongoing COVID-19 pandemic has adversely affected blood transfusion services all over the world. But its impact on donor deferral rate and the pattern is unclear in light of the new donor deferral policy due to the COVID-19 pandemic. MATERIALS AND METHODS: This retrospective study was divided into pre-COVID and COVID (15 March 2019-14 March 2021). All the deferred donors were divided into six different categories: (1) medical causes, (2) surgical causes, (3) drugs and vaccination, (4) risk of transfusion

Rethinking incompatibility in kidney transplantation.

Donor/recipient incompatibility in kidney transplantation classically refers to ABO/HLA-incompatibility. Kidney paired donation (KPD) was historically established to circumvent ABO/HLA-incompatibility, with the goal of identifying ABO/HLA-compatible matches. However, there is a broad range of donor factors known to impact recipient outcomes beyond ABO/HLA-incompatibility, such as age and weight, and quantitative tools are now available to empirically compare potential living donors across many of these factors, such as the living donor kidney donor profile index (LKDPI). Moreover, the detrimen

Emerging tick-borne diseases and blood safety: summary of a public workshop.

Tick-borne agents of disease continue to emerge and subsequently expand their geographic distribution. The threat to blood safety by tick-borne agents is ever increasing and requires constant surveillance concomitant with implementation of appropriate intervention methods. In April 2017, the Food and Drug Administration organized a public workshop on emerging tick-borne pathogens (excluding Babesia microti and Lyme disease) designed to provide updates on the current understanding of emerging tick-borne diseases, thereby allowing for extended discussions to determine if decisions regarding miti

Is the Kidney Donor Risk Index a step forward in the assessment of deceased donor kidney quality?

The allocation of deceased donor kidneys has become more complex because of the increasing spectrum of donors and recipients age and comorbidities. Several scoring systems have been proposed to evaluate the donor quality of deceased donor kidneys, based on clinical, pathological or combined parameters to predict the risk of renal allograft failure. Nonetheless, besides the dichotomous extended criteria donor (ECD) score, none of the others have been used in clinical practice because of numerous reasons, ranging from lack of robust validation to the technical challenges associated with the eval

Blood supply safety in Afghanistan: a national assessment of high-volume facilities.

BACKGROUND: Little information is available regarding blood supply safety in Afghanistan. The purpose of this study was to assess blood safety through serologic and observational measures in Afghanistan. STUDY DESIGN AND METHODS: This cross-sectional assessment included the 40 highest-volume facilities collecting and transfusing blood nationally identified in a previous survey. At each facility, study representatives completed a standardized instrument assessing staff performance of transfusion-related activities and performed rapid testing for human immunodeficiency virus, syphilis, and hepa

Eager blood donor.

Blood donor questionnaire is an integral component of blood safety, designed to identify and disqualify donors who do not fit certain criteria. One of the criteria is the time interval between 2 consecutive donations because repeated blood donors are exposed to adverse effects of frequent donations. We describe a 19-year-old healthy Israeli Defense Forces male soldier who has been detected by the Israeli National Blood Services only after his fifth voluntary blood donation within 12 weeks. The soldier was asymptomatic, yet blood examination revealed iron deficiency anemia with a low ferritin l

The in vitro bioassay systems for the amplification and detection of abnormal prion PrP(Sc) in blood and tissues.

Prion diseases or transmissible spongiform encephalopathies, including Creutzfeldt-Jakob disease (CJD) and variant CJD (vCJD), are chronic neurodegenerative diseases characterized by accumulation of abnormal infectious prions known as PrPSc. Infectious prion is emerging as a blood transfusion-transmissible pathogen and is present at extremely low levels in the blood of asymptomatic patients, which is not detectable by current standard methodologies. As such, prion-related diseases impose a huge challenge to blood transfusion services around the world. Rapid, sensitive, and specific assays are

Using auxotrophic donor strains to explore pQBR57 plasmid host range among environmental soil bacterial isolates.

Plasmid host range (PHR) plays a key role in the spread of ecologically important genes, alongside applications in microbiome engineering and environmental biotechnology. PHR is a complex trait arising from the combination of plasmid, donor and recipient properties. Most studies of PHR use a single donor strain, leaving the role of the donor unexplored and often require genetically tagged recipient strains for counter-selection, which limits the use of non-genetically tractable strains. Here, we applied auxotrophic donor counter-selection in a relatively high-throughput and accessible screenin

Expanding criteria for the use of living donors: what are the limits?

The cadaver organ shortage has pushed the transplant community to extend the boundaries beyond the traditional criteria used for living donor transplantation. This new liberal policy involves: (1) the type of donor, such as emotionally related individuals, the direct or indirect interchange of donors, anonymous as well as rewarded donation; (2) challenging immunological criteria, using incompatible ABO blood types and or transplantation across a positive cross-match; (3) relaxing clinical criteria related to elderly, hypertensive, or obese donors, or patients with nephrolithiasis, fibromuscula

Liver transplantation: current and future.

PURPOSE: The aim of this paper is to summarize the allocation challenges facing the field of liver transplantation while providing examples of the expansion of indications for the procedure. METHODS: UNOS allocation policy was reviewed as well as the recent literature describing expanded criteria for recipient candidate selection. RESULTS: Liver allocation policy changes for deceased-donor organs remain gridlocked in legal and bureaucratic red tape. Meanwhile, the indications for liver transplantation are being expanded to include acute alcoholic hepatitis, intrahepatic cholangiocarcinoma, a

Role of human leucocyte antigens matching in renal transplantation.

Matching human leucocyte antigens between donors and recipients of solid organ transplantation is essential for short and long term graft survival. Some physicians believe that human leucocyte antigens matching is not essential in renal transplantation. The beneficial effects of human leucocyte antigens-B antigen matching are seen if graft is functioning at least 2 years or more and benefits of human leucocyte antigens-A antigen matching seen if graft is functioning at least for 3 years or more. The one year graft survival rate in first transplant (UNOS registry, USA) being 95% in human leucoc

Marginal parental donors for pediatric living donor liver transplantation.

PURPOSE OF REVIEW: Living donor liver transplantation (LT) has been increasingly recognized as an effective treatment modality with excellent patient survival. Indications for LT have evolved not only for cholestatic liver disease, but also metabolic liver diseases. Living donor selection, particularly for pediatric inherited disease, is essential to prevent morbidity, both in the donor and recipient. RECENT FINDINGS: Based on 30 years of experience in pediatric living donor LT in Japan, we could identify marginal parental living donors who have potential risks following LT, including heteroz…

Living-unrelated kidney donor transplantation: Legalization in exceptional circumstances?

The prevalence of end-stage renal disease (ESRD) morbidity and mortality is mounting. Kidney transplantation offers a good means of survival and improves longevity of patients with ESRD. However, not everyone is fortunate to benefit from this lifesaving renal replacement therapy due to the lack of available kidneys, one of the many reasons. It eventually expands the number of patients on waiting list of kidney transplantation. At present, deceased and living-related kidney donor transplantation models are widely used, but with limited success to keep up with the pace of burgeoning ESRD. A deba

Genomic information for hematologic malignancies is now routinely available in clinical practice, supporting the adaptation of hematopoietic … (MDS), and myeloproliferative neoplasms (MPN). It also presents an up-to-date framework for optimal donor selection based on donor genome information, addressing both donor clonal hematopoiesis of indetermined significance and the risk that related donor candidates may carry the same hereditary predisposition. Finally, it discusses research showing that patient a …

Blood donor management in china.

Despite a steady increase in total blood collections and voluntary non-remunerated blood donors, China continues to have many challenges with its blood donation system. The country's donation rate remains low at 9%o, with over 60% of donors being first-time donors. Generally there is a lack of adequate public awareness about blood donation. The conservative donor selection criteria, the relatively long donation interval, and the small donation volume have further limited blood supply. To ensure a sufficient and safe blood supply that meets the increasing clinical need for blood products, there…
