We have studied differences in reperfusion between the pancreas and the duodenum after 6 hr of cold storage in a porcine whole-organ pancreaticoduodenal allograft transplantation model. Two different flush-out and storage solutions, Perfadex and EuroCollins, were compared. Graft duodenal mucosal pH (pHi) was measured as an indicator of duodenal mucosal reperfusion. Many secretory studies reported an increase in gastric acid secretion by the duodenal ulcerogen cysteamine. A detailed analysis of these experiments, especially the results from rats with chronic gastric fistula suggest that direct stimulation of gastric acid secretion may not be the primary mechanism of the duodenal ulcerogenic action of cysteamine. Data on 323 patients operated upon for penetrating duodenal ulcers (resection of the stomach) are presented. New procedures are described which elevate hermeticity of the duodenal stump as well as prophylactic measures against incompetence of sutures of the duodenum stump. Six patients had incompetent sutures of the duodenum stump which was of limited character and was completed by the formation of a duodenal fistula. Lethality from this complication was not noted.

Histological investigations were carried out upon the state of the gastric and duodena mucosa in 27 young patients with duodenal ulcer, seven out of them being in the state of exaverbation of the ulcer and 20-in remission. While the duodenal mucosa in the majority of the cases (88.9%) was established to be affected, in 85.2 percent of the patients the gastric mucosa remained intact. INTRODUCTION: Cytomegalovirus (CMV) is known to be opportunistic in immunocompromised patients. However, there have been emerging cases of severe CMV infections found in immunocompetent patients. Gastrointestinal (GI) CMV disease is the most common manifestation affecting immunocompetent patients, with duodenal involvement being exceedingly rare. Cytomegalovirus (CMV) remains one of the most important infections in kidney transplantation. Only a handful of images have been reported in the literature thus far. We present classic pathologic and gross images of CMV duodenitis in an immunosuppressed patient more than one year post-renal transplantation.

Causes involved in progression of duodenal lesions have been analyzed in 55 adolescents with chronic duodenitis. Five-year follow-up revealed factors with potential influence on the outcome: sex, acid-peptic factor, mucosal contamination with Helicobacter pylori, an initial morphological form of duodenitis. Sixty-eight horses with colic caused by small intestinal disease were allotted into 2 groups of 34 on the basis of recorded findings during exploratory celiotomy, necropsy, or response to medical treatment alone. Fifty patients with active peptic ulcers on endoscopy were randomly allocated for treatment with placebo or cimetidine (1.0 g daily) over a period of four weeks. All patients had free access to antacids to relieve epigastric pain. In the cimetidine group a significantly higher proportion of the ulcers had healed (82.6% of the patients) compared with the placebo group (48.0%). There was poor correlation between the healing of the ulcer and dyspeptic symptoms in the placebo group. A 79-year-old female patient was admitted to the hospital with a one-month history of dull pain in the right upper abdomen, accompanied by fever, nausea, vomiting, melena and irritative urinary symptoms. Abdominal contrast-enhanced CT revealed a 5.8 × 5.0 cm cystic lesion in the right kidney with an air-fluid level. Upper gastrointestinal endoscopy detected a duodenal bulb ulcer accompanied by fistula formation.
