[Vertigo and middle ear pathology].

The association of vertigo and diseases of the middle ear is not uncommon. It occurs especially during the course of chronic or acute otitis but can also occur as a sequela. Vestibular involvement is due to several pathophysiological mechanisms that influence the choice of treatment. The same is true for expansive processes of the tympanic cavity. When associated to otospongiosis, vertigo occurring during Ménière's disease can transiently contraindicate an operation on the tapes. On the contrary, vertigo resulting from traumatic lesion of the middle ear can suggest surgical intervention. Surge…

Autoimmune inner ear disease: therapy.

Sixty-six patients with this disease have been treated to date with cyclophosphamide and prednisolone resulting in significant improvement in hearing. In some patients the vestibular compartment may be involved as well, but the clinical pattern is significantly different enough from Meniere's disease to distinguish it by the careful historian. The cornerstone of treatment is cyclophosphamide, abetted by prednisolone. A test of treatment is to date the best method of determining who to treat for cure.

Ear replantation: a case report.

This paper presents the fifth reported case of replantation of a traumatically amputated ear and its management. The ear was amputated in an automobile accident and was revascularized microsurgically. Replantation, when possible, offers the ultimate result; however, shortcomings include the length of operation, prolonged hospitalization, and postoperative management of venous congestion.

Atelectatic ear and pathologic nasopharynx.

Pathologic relation between atelectatic ears and their nasopharynx were investigated in 50 patients with various degrees of atelectatic ears by transnasal fibrescope and C.T. And their morbid adenoids were compared with the adenoids of 20 patients with mouth breathing but free from ear problems. The process of drum bulging after catheterization, retraction after inflation, and inward movement during swallowing, proved that intratympanic pressure was influenced by nasopharyngeal pressure produced by swallowing, and that Eustachian tube itself was functioning to the pressure. Intratympanic negat…

[Postoperative treatment after ear surgery].

Postoperative treatment after ear surgery is described. Disturbances of the normal healing and their treatment in the office are especially mentioned.

Management of ear-canal collapse.

Thresholds were measured at 250, 500, 1,000, 2,000, 3,000, 4,000, 6,000, and 8,000 Hz in 20 normal adult ears and in ten ears from adults with varying amounts of ear-canal collapse. To alleviate the attenuation caused by ear-canal collapse, ear-canal inserts, postauricular pads, or circumaural cushions (Telephonics 510-020) were used with earphones (TDH-49). The circumaural cushions provided the lowest thresholds for the listeners with ear-canal collapse. Although thresholds with the ear-canal inserts were comparable to those with the circumaural cushions for many of the listeners, the ear-can…

Long-term middle ear ventilation.

A 15 year experience using wide flange tube for long term ventilation is reported with emphasis on complications and is compared in some respects to a small series of Paparella tube implants. At one time or another 70% of the wide flange tube implants were complicated, as were 100% of the Paparella tubes. The incidence of infection over a long period is high, at times only manageable with removal of the tube. However, longevity when tubes are removed for uncontrolled complication is still almost 3 years and as long as 12 years. The average longevity for electively removed tubes exceeds 4 years…

Managing ear infection in children.

Serous or suppurative middle ear effusion in children 2 years of age or older is common and is relatively easy to diagnose and treat. However, assessing and managing apparent ear infection in younger children--particularly infants--are more difficult tasks, and in many of these young patients the middle ear is found to be normal.

Otologic aspects of ear burns.

Otologic experiences in a large burn center are discussed and the following areas are emphasized. First, the otologic effects of hyperbaric oxygen therapy and the prevention and treatment of barotrauma that is occasionally secondary to this therapy. Second, the diagnosis and treatment as well as the prevention of perichondritis in severely burned ears will be discussed. A total of thirty-five cases of perichondritis will be reviewed. An encouraging aspect of the method of treatment has evolved over the past five years: there has been a minimum of deformity in the healed ear, with no subsequent…

Middle ear effusion and sinusitis.

The present investigation comprises a retrospective and a prospective study on children with middle ear effusion (MEE). It is shown that, in the majority of patients, middle ear effusion was found in combination with an infective disease of other parts of the upper respiratory system, especially in the maxillary sinuses. Lavage of the infected sinuses resulted in resolution of the middle ear effusion in about 85 per cent of the cases, while adenoidectomy, in patients in whom adenoid enlargement was found together with middle ear effusion, had only a minor effect.

[Middle ear tuberculosis (author's transl)].

With the decrease in tuberculosis in general that of the middle ear has become a rarity, such that it is almost unknown to the younger generation of doctors. Nevertheless one must consider this diagnosis in all relapsing and treatment-resistant inflammations of the middle ear, especially when tympanoplasty procedures have been unsuccessful. Bacteriological findings are less reliable than histological proof and therefore examination of granulation tissue is always indicated. A precise history into earlier tuberculosis is important, since the disease is mostly disseminated haematologically. Two …

Regeneration of rabbit ear tissue.

Holes punched in the ears of rabbits are repaired by regeneration of new tissues from blastemas found on the periphery of the wounds. The proportion whch successfully regenerated was greater in males than in females (75% vs 20%), in pregnancy than during etrus or lactation (75% vs 25%), and in ovariectomized does given testosterone than in those given oil carrier alone (88% vs 50%), but the speed of closure did not differ in these groups. The steroid hormone influence postulated by other workers is confirmed. Closure was faster in younger animals and for second holes punched on the same site w…

Ear damage due to diving.

Diving injury may affect all parts of the ear during all types of diving. Divers are regularly exposed to noise and 120 dB(A) is measured during ventilation of a hyperbaric chamber. Ear canal squeeze, possibly with drum perforation, may give a transient conductive hearing loss. Middle ear squeeze, possibly with drum perforation, also gives a transient conductive loss. Inner ear barotrauma, possibly with perilymph fistula, most often results in transient vertigo and lasting sensorineural high tone loss, often resembling a noise-induced loss. Decompression sickness and gas embolism can also dama…
