Cartilaginous bending spring for preventing tympanic membrane graft medialisation in anterior or subtotal tympanic membrane perforations-how I do it.

BACKGROUND: The reconstruction of anterior or subtotal tympanic membrane perforations is critical due to the risk of anterior graft medialisation and retraction or recurrent perforation. METHOD: After reconstruction of the tympanic membrane by means of grafting, a rectangular cartilage strut (length 6 mm, breadth 2 mm, thickness 0.1 mm) is prepared using a cartilage knife and scalpel. This strut graft is placed between the cartilage graft and the promontory in the anterior inferior part of the middle ear cavity. CONCLUSION: Our experience shows that using a U-shaped cartilage strut to sustai

A simple method for auricular reconstruction in mild cases of conchal type microtia.

The authors describe their method of helix/antihelix creation together with their algorithm of mild conchal type microtia treatment. In mild cases of conchal type microtia, they do not use a platform-based framework but fabricate the helix and the antihelix separately from costal cartilage and anchor them firmly into the corrected remnant concha. A framework created in this way is sufficiently supportive for the new ear, has good aesthetic features, and is fast and easy to fabricate. Moreover, because the procedure does not require a large amount of cartilage, it also decreases the occurrence

Effects of Synchrotron X-Ray Micro-beam Irradiation on Normal Mouse Ear Pinnae.

PURPOSE: To analyze the effects of micro-beam irradiation (MBI) on the normal tissues of the mouse ear. METHODS AND MATERIALS: Normal mouse ears are a unique model, which in addition to skin contain striated muscles, cartilage, blood and lymphatic vessels, and few hair follicles. This renders the mouse ear an excellent model for complex tissue studies. The ears of C57BL6 mice were exposed to MBI (50-μm-wide micro-beams, spaced 200 μm between centers) with peak entrance doses of 200, 400, or 800 Gy (at ultra-high dose rates). Tissue samples were examined histopathologically, with conventional

[Island cartilage myringoplasty. Anatomical and functional results in 122 cases].

INTRODUCTION AND OBJECTIVES: In certain situations ("high risk perforations") such as large perforations, revision cases, middle ear pathology, eustachian tube dysfunction and atelectatic ears, the failure rate of myringoplasty is high. Some authors have suggested that the materials most frequently used for myringoplasty (fascia and perichondrium) may have a role in this failure rate. Cartilage myringoplasty, however, achieves good results in these "high risk" cases. The purpose of this study is to analyze our results and describe the technique. METHODS: A retrospective study of all consecuti

The importance of costal cartilage framework stabilization in microtia reconstruction: anthropometric comparison based on 216 cases.

This study explored anthropometric changes in the reconstructed auricle and the contralateral normal ear in a series of 216 microtia patients using different stabilization methods. Our main personal modifications concerning the preparation of the framework were the following: 1. The individualized framework grafting was based on patients with different ages and different degrees of the strength and thickness of the rib cartilage. 2. The framework was stabilized as a "C" shape by using a piece of cartilage or suture to reinforce the two end points of the "C". In group A (the thickness of cartil

[Application of residual ear in auricular reconstruction of microtia].

OBJECTIVE: To summarize different treatments of the residual ear in auricular reconstruction, to investigate the reasonable applications of the residual ear. METHODS: From September 2005 to July 2006, 128 patients (79 males, 49 females; aging 5-21 years with an average of 11 years) with unilateral microtia underwent the staged repair. In the patients, there were 44 cases of left-unilaterally microtia and 84 cases of right-unilaterally microtia. The residual ears looked like peanut in 56 patients, like sausage in 35 patients, like boat in 27 patients, and like shells in 10 patients. Among all

Relapsing polychondritis.

Relapsing polychondritis is an uncommon disease consisting of inflammation of the eyes, inner ears, cardiovascular system, and cartilaginous portions of the joints, respiratory tract, and external ear. Cutaneous manifestations are the presenting feature in more than 50% of patients. These usually consist of erythema, swelling, and pain, reflecting involvement of the underlying cartilage. Direct involvement of the skin may occur as vasculitis, lesions resembling erythema nodosum, or nonspecific eruptions. The presence of circulating antibodies to type II collagen--more against native than denat

Use of Thermoplastic Splint for the Treatment of Auricular Hematoma.

Auricular hematoma is commonly seen in ear nose and throat clinical practice and mostly caused by blunt trauma as a result of traffic accident, wrestling, boxing etc. If hematoma does not discharge, blood supply of the cartilage fails and this results with the necrosis of the auricular cartilage. Incision and drainage of the hematoma is the cornerstone of the surgical treatment and simple compression methods are limited in terms of their ability to eliminate the empty space. Various techniques have been described for the elimination of the death space such as; dental rolls, cotton bolsters, bu

[Secondary surgery of the external ear].

Secondary surgeries of the external ear can be divided into two categories: secondary otoplasties and secondary ear reconstructions. The most frequent causes of secondary otoplasties are a recurrence, an over-corrected ear, an off-center ear, a prominent lobule, and finally chronic ear pain. Recurrence of the prominence can be treated by a new posterior stitch otoplasty, an Earfold clip, or a frame stitch. The over-corrected ear can be projected either by costal cartilage grafts or by hyaluronic acids. Patients who are unsatisfied of the aesthetic result of their prominent ear correction usual

Surgical Otoplasty: An Evidence-Based Approach to Prominent Ears Correction.

Otoplasty is one of the first procedures learned during residency. A myriad of surgical techniques and nuances exist. Many have merit, some are ineffective, some destructive, and some frankly fanciful. Adopting an effective and safe technique should be based on proven efficacy and effectiveness to avoid early disappointments. We present a review of traditional otoplasty techniques and more recent innovations. Their pros and cons are discussed in view of the relative risks/benefits balance. Recurrence rates are low for most techniques. Some techniques carry a higher risk of significant complica

[Cystic chondromalacia of the auricle. A case report. Review of the literature].

Cystic chondromalacia is a clinical and histopathological entity which can be clearly distinguish from all other cystic lesions of the auricle. We report a case which was clinically asymptomatic and involved the scaphoid fossa of the anterior surface of the pinna. The pathological process consisted of degenerative changes of the auricular cartilage which produced a cavity containing a serous fluid. No etiologic factor was found, in particular no trauma. The posterior wall of the cyst was excised under local anesthesia. This case allowed us to review the clinical, histopathological features and
