INTRODUCTION: After centromedullary nailing (CMN) of 4-parts (4P) cephalo-tuberositary fractures of the proximal humerus (PH), shoulder immobilization for a few weeks is usual, although no scientific justification does support this attitude, nor the duration of immobilization. The objective of this study was to assess the impact of early mobilization after CMN of PH 4P fractures on clinical, radiological outcomes and complication rates. The hypothesis was that early mobilization would not lead to a deterioration in results or an increase in the complication rate.

The effect of early mobilization and unrestricted weight bearing on final ankle motion in 51 operatively stabilized ankle fractures was prospectively investigated. We reviewed 19 patients treated for upper arm fractures using Seidel's locking nail system, which allows for early function after operation. The overall results were good with no cases of pseudarthrosis, infection or radial nerve palsy. All the patients regained full shoulder movements with no evidence of rotator cuff lesions.

A new design, the 'Hip Hinge Thigh Brace' is presented for the safe early functional mobilization of patients with proximal half femoral shaft fractures. The mechanics, prescription, application technique and early clinical experience are reported. Two large series of civilian-incurred (212) and combat-incurred (228) tibial shaft fractures are compared.
