Evaluating Vaccination Programs That Prevent Diseases With Potentially Catastrophic Health Outcomes: How Can We Capture the Value of Risk Reduction?

In the last 5 years, guidelines have been developed for performing cost-effectiveness analyses (CEAs) for the economic evaluation of vaccination programs against infectious diseases. However, these cost-effectiveness guidelines do not provide specific guidance for including the value of reducing the risk of rare but potentially catastrophic health outcomes, such as mortality or long-term sequelae. Alternative economic evaluation methods, including extended CEA, the impact inventory, cost-benefit analyses, willingness to pay or the value of a statistical life, to capture the value of this risk

Teaming up to crack innovation and enterprise integration.

In the continuing quest for business growth, many CEOs are turning to their CIOs and IT organizations because technology is essential to two compelling sources of growth: innovation and integration. Innovation, of course, is doing new things that customers ultimately appreciate and value--not only developing new generations of products, services, channels, and customer experience but also conceiving new business processes and models. Integration is making the multiple units, functions, and sites of large organizations work together to increase capacity, improve performance, lower cost structur

Profiling the plight of adolescent girls and young women: risks and pathways for intervention in rural Uganda.

BACKGROUND: Adolescent girls and young women (AGYW) in rural Uganda face overlapping socio-economic, educational, and behavioral vulnerabilities that heighten their risks of HIV, unintended pregnancies, gender-based violence (GBV), and other adverse health outcomes. Establishing baseline risk profiles is essential for designing targeted interventions. METHODS: This study analyzed secondary data collected through the Uganda DREAMS and OVC Tracking System (UDOTS) during program enrollment between July 2017 and December 2023 in four districts of Central Uganda (Mubende, Mityana, Luwero, and Kass

Infertility, infection, and iatrogenesis in Egypt: the anthropological epidemiology of blocked tubes.

In this article, we integrate medical anthropological and analytical epidemiological methods, forms of data analysis, and interpretive insights to examine the culture-specific behavioral factors that place poor, urban Egyptian women at risk of tubal-factor infertility (TFI). Such risk factors include biomedically and ethnomedically produced iatrogenesis, including the consequences of the practice of female circumcision, and male sexual behavior leading to sterilizing sexually transmitted diseases (STDs) in their female sexual partners. We examine the socio-cultural and political-economic conte

Risk factors for perinatal intimate partner violence: a global systematic review and meta-analyses.

BACKGROUND: Perinatal intimate partner violence (IPV) is highly prevalent in different regions worldwide. However, there is still insufficient systematic evidence on the risk factors for perinatal IPV across various regions and socioeconomic levels, which presents challenges in developing targeted prevention programs. The aim of this systematic review and meta-analyses was to identify the risk factors for perinatal IPV across various regions and socioeconomic levels worldwide. METHODS: A comprehensive systematic literature search was conducted across multiple electronic databases, including Pu

Modelling the effect of market forces on the impact of introducing human immunodeficiency virus pre-exposure prophylaxis among female sex workers.

Pre-exposure prophylaxis (PrEP) to prevent human immunodeficiency virus (HIV) enables female sex workers (FSWs) to protect themselves from HIV without relying on clients using condoms. Yet, because PrEP reduces HIV risk, financial incentives to not use condoms may lead to risk compensation: reductions in condom use and/or increases in commercial sex, and may reduce the price of unprotected sex. In this analysis, we integrate market forces into a dynamic HIV transmission model to assess how risk compensation could change the impact of PrEP among FSWs and clients. We parameterise how sexual beha

Gambling in the Mist of Economic Crisis: Results From Three National Prevalence Studies From Iceland.

In October 2008 all three major banks in Iceland went bankrupt with serious consequences for Icelandic society. The national currency lost more than half of its value and there was a sharp increase in household debts and prices for domestic goods. Very little is known about the potential effects of economic recessions on gambling participation and problem gambling. This study reports on the results of three national prevalence studies conducted before and after the economic collapse in Iceland. The same methodology and measures were used in all three studies to ensure their comparability and t

A Mixed-Method Evaluation of the Association Between Attitudes Toward Gender-Based Violence and Sexual Behaviors Among Adolescents and Young Adults in Rustenburg, South Africa.

In South Africa, gender-based violence (GBV) is a public health crisis contributing to the transmission of HIV and requiring urgent intervention. Using mixed methods, this study evaluated the relationship between adolescents' attitudes toward GBV and their sexual behaviors. Adolescent girls and young women (AGYW) and adolescent boys and young men (ABYM) completed a questionnaire assessing socio-demographic characteristics, sexual risk behavior, and attitudes regarding GBV. Two focus group discussions (FGDs) were conducted to explore perceptions of GBV. Among 178 participants, 80 (44.9%) had pe

Quality competition in local hospital markets: some econometric evidence from the period 1982-1988.

This study examines whether American hospitals continued to engage in non-price or quality competition over the recent past as health care markets underwent fundamental structural changes and the economic incentives facing hospital managers were correspondingly altered. It also investigates the degree to which such rivalrous behavior contributes to losses in economic welfare. An econometric model of quality competition is specified that tests, among other things, for the effect of spending by the hospital to enhance the quality of output on annual changes in its share of the local (inpatient)

Factors predicting birth weight in a low-risk sample: the role of modifiable pregnancy health behaviors.

OBJECTIVES: The purpose of the present study was to examine background and modifiable pregnancy health behavior factors predicting infant birthweight in an economically and educationally disadvantaged sample with low medical risk. METHODS: PARTICIPANTS were recruited from a family practice center in rural Appalachia. PARTICIPANTS: Over 220, predominantly Caucasian and lower SES women with low risk pregnancies were included in the sample. Data were collected through systematic chart review. Half of the women smoked during pregnancy and over 10% gave birth to low birth weight (LBW; < 2500 g) b

Relationship of intestinal parasites to the environment and to behavioral factors in children in the Bolikhamxay Province of Lao PDR.

From March to July 1998 the infection rates of 732 children aged below 15 years were assessed. The investigation was conducted in selected villages of the Bolikhamxai Province in Lao PDR. Socio-economic conditions and behavioral pattern were studied. The three soil-transmitted helminths, Ascaris lumbricoides, Trichuris trichiura and hookworm were found with prevalence rates of 67.14, 17.49 and 12.83%, respectively. Infection rates with other intestinal parasites were negligible. Of the children investigated, 56.7% harbored one and 20.45% more than one parasite. Except for hookworms, no statist

Factors Associated with Cervical Cancer Screening for Women in New York State: Analyzing the 2022 NYS BRFSS.

Introduction: Cervical cancer remains a significant public health challenge despite being largely preventable through early screening and treatment and is one of the largest causes of cancer-related deaths among women aged 20-39. New York State, with its diverse population, may have unique differences and disparities that can impact cervical cancer screening rates. Methods: Using the 2022 NYS Behavioral Risk Factor Surveillance System, we analyzed data from 3,968 participants. Bivariate analysis and logistic regression assessed the associations between demographic, general health, economic, an

Safety, effectiveness, and economic evaluation of an herbal medicine, Ukgansangajinpibanha granule, in children with autism spectrum disorder: a study protocol for a prospective, multicenter, randomized, double-blinded, placebo-controlled, parallel-group clinical trial.

BACKGROUND: Autism spectrum disorder (ASD) is characterized by continuous impairment in communication and social interaction and by limited and repetitive behaviors, interests, or activities. Behavioral, educational, and pharmaceutical interventions have been shown to reduce behavioral disabilities, improve verbal/non-verbal communication, and help patients acquire self-reliance skills. However, there has been a lack of systematic verification and consensus regarding the treatment of the core symptoms of ASD because of its unclear etiology. Far more than a shot in the arm: Vaccines and consumer spending.

The spread of the COVID-19 pandemic, along with government interventions, significantly reshaped economic activity, leading to abrupt shifts in household consumption behavior. This paper offers an empirical analysis of how the rollout of COVID-19 vaccines influenced consumer spending, using high-frequency debit and credit card transaction data from three European countries. The findings indicate that vaccinations, in conjunction with other policy measures, helped mitigate the pandemic's severe economic impact and supported a recovery in consumer spending. First, the deployment of vaccines had

Initial development of a brief assessment of cocaine demand.

Cocaine demand is a behavioral economic measure assessing drug reward value and motivation to use drug. The purpose of the current study was to develop a brief assessment of cocaine demand (BACD). Results from the BACD were compared with self-report measures of cocaine use. Participants consisted of treatment-seeking individuals with cocaine use disorder (N = 22). Results revealed that indices of brief demand were significantly associated with various self-report measures of cocaine use. Overall, these results support the utility of a BACD for assessing cocaine demand.

Mothers' childhood hardship forecasts adverse pregnancy outcomes: Role of inflammatory, lifestyle, and psychosocial pathways.

Research suggests the health consequences of economic hardship can be transmitted across generations. Some of these disparities are thought to be passed to offspring during gestation. But this hypothesis has not been tested in contemporary American samples, and the mechanisms of transmission have not been characterized. Accordingly, this study had two goals: first, to determine if women exposed to economic hardship during childhood showed higher rates of adverse birth outcomes; and second, to evaluate the contribution of inflammation, psychosocial, lifestyle, and obstetric characteristics to t

Economic violence to women and girls: is it receiving the necessary attention?

Most studies on gender-based violence (GBV) have focused on its physical, sexual, and psychological manifestations. This paper seeks to draw attention to the types of economic violence experienced by women, and describes its consequences on health and development. Economic violence experienced included limited access to funds and credit; controlling access to health care, employment, education, including agricultural resources; excluding from financial decision making; and discriminatory traditional laws on inheritance, property rights, and use of communal land.
