Elasticity of the Forearm Flexor-Pronator Muscles as a Risk Factor for Medial Elbow Injuries in Young Baseball Players: A Prospective Cohort Study of 314 Players.

BACKGROUND: Young baseball players with medial elbow injuries are known to have high forearm flexor-pronator muscle elasticity; however, the causal relationship between forearm muscle elasticity and the occurrence of medial elbow injuries remains unclear. PURPOSE/HYPOTHESIS: The purpose of this study was to determine whether the forearm flexor-pronator muscle elasticity is a risk factor for medial elbow injury in young baseball players. It was hypothesized that high flexor carpi ulnaris (FCU) elasticity would be a risk factor for medial elbow injuries. STUDY DESIGN: Cohort study; Level of ev…

Injury chronicity does not affect outcomes following ligament repair with suture tape augmentation for post-traumatic elbow instability.

HYPOTHESIS: Repair and augmentation techniques have been successfully applied in the treatment of acute ligament injuries causing elbow instability. This study evaluates whether the chronicity of elbow instability affects postoperative outcomes following ligament repair with augmentation using suture tape internal bracing. METHODS: A retrospective study was conducted on 40 patients (mean follow-up 42.5 months; range, 9-99 months) who underwent ligament repair with suture tape augmentation for traumatic elbow instability between 2013 and 2019. Functional outcomes (Disabilities of the Arm, Shou

Combined flexor carpi ulnaris and flexor carpi radialis transfer for restoring elbow function after brachial plexus injury.

The result of combined agonist and antagonist muscle innervation in traumatic brachial plexus injury through the intraplexal fascicle nerve transfers with the same donor function has not yet been reported. We describe a patient with a C5-C7 traumatic brachial plexus injury who had a combined transfer of the flexor carpi radialis (FCR) fascicle to the musculocutaneous nerve and the flexor carpi ulnaris (FCU) fascicle to the radial nerve of the triceps. The patient returned for his follow-up visit 2 years after his surgery. The muscle strengths of his triceps and biceps were Medical Research Cou

Posteromedial Elbow Dislocations without Relevant Osseous Lesions: Clinical Characteristics, Soft-Tissue Injury Patterns, Treatments, and Outcomes.

BACKGROUND: Although simple posterolateral or posterior elbow dislocations are relatively common and usually stable after closed reduction, simple posteromedial dislocations are extremely rare and poorly characterized. We investigated the clinical characteristics, soft-tissue injury patterns, treatments, and outcomes of a series of posteromedial elbow dislocations without relevant osseous lesions. METHODS: We retrospectively reviewed 20 cases of simple posteromedial elbow dislocations without relevant osseous lesions that were treated at 7 fellowship training hospitals during a 10-year period

Single-staged treatment using a standardized protocol results in functional motion in the majority of patients with a terrible triad elbow injury.

BACKGROUND: Terrible triad injuries of the elbow, defined as elbow dislocation with associated fractures to the radial head and coronoid, are associated with stiffness, pain, and loss of motion. Studies to date have consisted of small sample sizes and used heterogeneous surgical techniques, which render comparisons difficult and unreliable. QUESTIONS/PURPOSES: In a group of patients treated under a standard surgical protocol, we sought to determine the early dislocation rate, the range of motion in those not undergoing secondary procedures, the frequency and types of secondary surgical interv

Treatment of heterotopic ossification of the elbow following burn injury: recommendations for surgical excision and perioperative prophylaxis using radiation therapy.

BACKGROUND: Heterotopic ossification (HO) is reported to occur in 0.1-3.3% of elbows after a severe burn, and can significantly limit elbow motion and upper extremity function. METHODS: The study included 9 patients (11 elbows) treated by the senior author (TW). The surgical technique consisted of making multiple small surgical approaches to remove heterotopic ossification (without raising cutaneous flaps) and concomitantly releasing the elbow capsule and skin contracture. Perioperative radiation therapy was performed to decrease heterotopic ossification recurrence. Outcome measures included

Operative release of complete ankylosis of the elbow due to heterotopic bone in patients without severe injury of the central nervous system.

BACKGROUND: Although uncommon, complete ankylosis of the elbow secondary to heterotopic ossification results in severe disability. The results of surgical management remain unclear. METHODS: A single surgeon used a consistent operative technique to treat complete osseous ankylosis of the elbow in eleven limbs in seven patients after severe burns and in nine elbows in eight patients after trauma. The elbows in the burn cohort were more often ankylosed in extension (average, 47 degrees of flexion) compared with those in the trauma cohort (66 degrees of flexion), and they had more skin problems

A novel screening tool is effective in the early detection of peripheral nerve injury with elbow and forearm trauma subjects: A case series.

BACKGROUND: Traumatic elbow and forearm injuries involving a peripheral nerve injury are complex and costly due to combined nerve and soft tissue involvement, often resulting in sensory, motor, and pain impairments that limit daily activities, work, and social participation. Early, comprehensive assessment of peripheral nerve injury associated with elbow or forearm trauma is therefore essential to optimize patient outcomes. PURPOSE: This case series aims to describe the development and practicality of the Upper Extremity Peripheral Nerve Screen (UEPNS) for individuals post elbow/forearm traum

Ipsilateral Distal Both Bone Forearm and Lateral Humeral Condyle Fractures With Posterolateral Elbow Dislocation: A Rare Injury in a Child.

Lateral humeral condyle fractures are common in children, but concomitant elbow dislocation is rare. This case report involves a 10-year-old girl with an ipsilateral distal both-bone forearm fracture and a lateral humeral condyle fracture accompanied by a posterolateral elbow dislocation. Closed reduction of the elbow was performed in the emergency department without delay, and the patient was operated on as soon as possible. Closed reduction percutaneous pinning (CRPP) was performed for the distal radius fracture. The Weiss type III lateral humeral condyle fracture was reduced through a later

Relationship of the Median and Radial Nerves at the Elbow: Application to Avoiding Injury During Venipuncture or Other Invasive Procedures of the Cubital Fossa.

INTRODUCTION: The median and radial nerves are two important neural structures found in the cubital fossa. The trajectory and landmarks used to identify their location are important when procedures are done in this area. METHODS AND MATERIALS: Ten fresh-frozen cadavers were dissected (20 upper limbs) and measurements were taken from the medial epicondyle to the median and radial nerves as well as to the lateral epicondyle of each limb. RESULTS: The distance between the medial epicondyle and the median nerve was found to be three centimeters with a range of 2.1 to four centimeters and the dis

Elbow dislocation with ipsilateral fracture of the distal radius associated with a brachial artery injury: A new pathological condition of traumatic origin.

Elbow dislocation associated with ipsilateral fracture of the distal radius and a brachial artery injury is an uncommon traumatic entity. The two references of this injury combination appeared in 2015, although both authors did not realise that they were the first two cases published in the medical literature. Although mentioned in the text of the articles, no mention was made of the fracture of the distal radius in the titles. The purpose of this paper is to present three cases with this new traumatic pathological entity, explaining its pathogenetic mechanism, the treatment used, and the resu

Functional restoration of elbow extension after spinal-cord injury using a neural network-based synergistic FES controller.

Individuals with a C5/C6 spinal-cord injury (SCI) have paralyzed elbow extensors, yet retain weak to strong voluntary control of elbow flexion and some shoulder movements. They lack elbow extension, which is critical during activities of daily living. This research focuses on the functional evaluation of a developed synergistic controller employing remaining voluntary elbow flexor and shoulder electromyography (EMG) to control elbow extension with functional electrical stimulation (FES). Remaining voluntarily controlled upper extremity muscles were used to train an artificial neural network (A
