OBJECTIVE: To characterize stimulation of taste fibers in the facial nerve following cochlear implantation.
PATIENT: A 34-year old presented with reversible dysgeusia following activation of a cochlear implant.
INTERVENTION: Reprogramming targeted to specific offending electrodes reduced symptom intensity. Computed tomography demonstrated dehiscence of the bone separating the labyrinthine segment of the facial nerve and the basal turn of the cochlea in proximity to the electrode array.
RESULTS: Dysgeusia was attributed to stimulation of taste fibers in the facial nerve by electrodes 13 to 1

Electrical stimulation is a new way to treat digestive disorders such as constipation. Colonic propulsive activity can be triggered by battery operated devices. This study aimed to demonstrate the effect of direct electrical colonic stimulation on mean transit time in a chronic porcine model. The impact of stimulation and implanted material on the colonic wall was also assessed. Three pairs of electrodes were implanted into the caecal wall of 12 anaesthetized pigs. Reference colonic transit time was determined by radiopaque markers for each pig before implantation. It was repeated 4 weeks afte

Fifteen adult male Sprague-Dawley rats were implanted with a set of electrodes for standard sleep recordings. A stainless steel cannula was also implanted into the lateral ventricle of these rats. Fifteen additional rats were implanted with a cannula alone. Rats with electrodes were habituated for 3 days or more to the recording environment, then placed into 3 groups (n = 5). One group received saline (i.c.v.), while the other two groups received either the feline immunodeficiency virus envelope glycoprotein (FIV SU-Env) or a fragment of the Epstein-Barr virus envelope glycoprotein (EB gp105).

HYPOTHESIS: Electrical tinnitus suppression by cochlear implants requires stimulation of a subset of neural elements in the cochlea.
BACKGROUND: Tinnitus is the phantom perception of sound in the ears and is a known correlate of hearing loss. Cochlear implants restore hearing and are known to lessen or extinguish tinnitus. The amount of electrical charge required and the number and location of electrodes required to extinguish tinnitus with a cochlear implant are factors that remain poorly understood.
METHODS: In a subject with single-sided deafness, with tinnitus in the deaf ear, we enabled

Peripheral nerve stimulation and peripheral nerve field stimulation involve the delivery of electrical stimulation using implanted electrodes either over a target nerve or over the painful area with the goal of modulating neuropathic pain. The selection of appropriate candidates for this therapy hinges on skillful application of inclusion and exclusion criteria, psychological screening, and an invasive screening trial. Patients with significant improvement in pain severity and pain-related disability during the trial are considered candidates for implantation of a permanent system. As with oth

OBJECTIVE: Recent trends have moved from subdural grid electrocorticography (ECoG) recordings toward stereo-electroencephalography (SEEG) depth electrodes for intracranial localization of seizures, in part because of perceived morbidity from subdural grid and strip electrodes. For invasive epilepsy monitoring, the authors describe the outcomes of a hybrid approach, whereby patients receive a combination of subdural grids, strips, and frameless stereotactic depth electrode implantations through a craniotomy. Evolution of surgical techniques was employed to reduce complications. In this study, t

The induced electric field/current in the sacral nerve by stimulation devices for the treatment of bladder overactivity is investigated. Implanted and transcutaneous electrode configurations are considered. The electric field induced in the sacral nerve by the implanted electrode is largely affected by its surrounding tissues, which is attributable to the variation in the input impedance of the electrode. In contrast, the electric field induced by the transcutaneous electrode is affected by the tissue conductivity and anatomical composition of the body. In addition, the electric field induced

Restoration of hand function through functional electrical stimulation allows tetraplegic patients to use existing abilities to control paralyzed muscles. In patients with C5 or C6 spinal cord injuries, implanted upper extremity neuroprostheses use functional electrical stimulation technology to power hand and arm muscles. A variety of devices, often using contralateral shoulder motion, sends signals via a small external controller and transmitting coil to an implanted stimulator. The stimulator powers designated upper extremity muscles via implanted electrodes. The surgical procedure is minim

BACKGROUND: Stereo-electroencephalography (SEEG) is a key technique for the presurgical evaluation of drug-resistant focal epilepsy [2]. Robotic assistance facilitates precise multi-trajectory stereotactic implantation and allows workflow standardization [1, 3, 5, 9]. We describe a reproducible image-based robotic SEEG implantation protocol and report our institutional experience.
METHODS: An image-based robot-assisted SEEG technique using the Neuromate® system combined with intraoperative 3D imaging is presented. The workflow includes hypothesis-driven trajectory planning, alphabetical traje

Abnormal neural activity, particularly in the rostrodorsal anterior cingulate cortex (rdACC), appears to be responsible for intense alcohol craving. Neuromodulation of the rdACC using cortical implants may be an option for individuals with treatment-resistant alcohol dependence. This study assessed the effectiveness and feasibility of suppressing alcohol craving using cortical implants of the rdACC using a controlled one-group pre- and post-test study design. Eight intractable alcohol-dependent participants (four males and four females) were implanted with two Lamitrode 44 electrodes over the

OBJECTIVES/HYPOTHESIS: Cochlear implantation for patients with common cavity deformity (CCD) can be difficult, with a higher incidence of intra- and postoperative complications; outcomes with CCD patients are also highly variable. In this study, surgical time was compared between the traditional facial recess approach (TFRA) and the transmastoid slotted labyrinthotomy approach (TSLA). Audiological outcomes and the benefit of using customized electrode arrays for CCD patients are also discussed.
STUDY DESIGN: Retrospective review of 13 cochlear implant (CI) patients with CCD.

UNLABELLED: Tinnitus is a public health issue in France. Around 1 % of the population is affected and 30,000 people are handicapped in their daily life. The treatments available for disabling tinnitus have until now been disappointing. We report our experience on the treatment of these patients in neurosurgery.
PATIENT AND METHODS: Between 2006 and 2008, transcranial magnetic stimulation (rTMS) was performed following several supraliminal and subliminal protocols in 16 patients whose mean age was 47 years (range, 35-71). All patients underwent anatomical and functional MRI of the auditory cor

Myoelectric activity patterns of the upper gastrointestinal tract were recorded using chronically implanted electrodes in conscious rabbits. A cyclical pattern of intense spiking activity occurring on almost every slow wave for 10-15 min, corresponding to the regular spiking phase or phase III of the migrating myoelectric complex (MMC), was recorded. This activity was detected by electrodes implanted distal to the ligament of Treitz on the proximal jejunum at a frequency of 9.4-10.6/24 hr. The MMC pattern occurred in both fed and fasted animals, regardless of the presence of cecotrophy. Initia

In 1970, we began implanting electrodes for prolonged stimulation of injured peripheral nerves to reduce chronic pain. Thirty-eight peripheral nerves in thirty-five patients have been stimulated with electrodes for a period ranging from four to nine years. Nineteen electrode systems were implanted in the upper extremity (eleven on the median nerve, six on the ulnar nerve, one on the median and ulnar nerves, and one on the median and radial nerves), with successful relief of pain in 52..6 per cent of the patients. Sixteen stimulators have been implanted on the sciatic nerve with a success rate

Previous experience with steroid-eluting small electrode designs have described their increased pacing efficiency, yet some reports have questioned their electrical stability. We report our experience with a new pacing lead design incorporating small surface (i.e. 1.2 mm2), high impedance pacing electrodes. Medtronic Model 5034 ventricular pacing leads were implanted by a single physician in 167 patients. Of those, 96 patients had an additional Model 5534 atrial lead implanted. All patients were followed for at least 9 months. Microdislodgment, as defined by a sudden increase in pacing thresho

HYPOTHESIS: To elucidate possible mechanisms of facial nerve costimulation after cochlear implantation that are supposed to result from the close cochlea to facial nerve contact.
BACKGROUND: One of the postoperative complications of cochlear implantation is facial muscle twitching, which has preferentially been found in otosclerotic patients. It impairs hearing benefits because of deactivation of electrodes and can still not be adequately prevented.
METHODS: A total of 13 temporal bones were dissected to quantify where the labyrinthine portion of the facial nerve is closest to the scala tymp

Implantable neural microelectrodes exhibit the great ability to accurately capture the electrophysiological signals from individual neurons with exceptional submillisecond precision, holding tremendous potential for advancing brain science research, as well as offering promising avenues for neurological disease therapy. Although significant advancements have been made in the channel and density of implantable neural microelectrodes, challenges persist in extending the stable recording duration of these microelectrodes. The enduring stability of implanted electrode signals is primarily influenc
