Palliation of pancreatic cancer using electrolytic ablation.

BACKGROUND: Inoperable pancreatic cancer has a dismal prognosis. Palliation involves either stenting or surgical bypass. Stenting does not relieve gastric outlet obstruction, and surgical bypass is a major procedure. A minimally invasive procedure is needed that relieves both gastric outlet and biliary obstruction, with the potential for relieving pain. METHODS: In an experimental model, pancreatic electrolysis was investigated. The pancreatic duct was cannulated via a transduodenal approach with an electrode catheter. In 6 animals an electrolytic "lesion" was created using a direct current g

Acid-base and electrolyte disturbances in patients with hypercalcemia.

BACKGROUND: In the present study, we analyzed acid-base and electrolyte disturbances in hypercalcemic patients to determine the principal causes of hypercalcemia. METHODS: We studied a total of 76 hypercalcemic patients and 91 healthy individuals. Acid-base and electrolyte parameters were determined before any therapeutic intervention. RESULTS: Hyperparathyroidism and neoplasias were the most common causes of hypercalcemia. Hypercalcemic patients had increased serum urea and creatinine levels, a higher urea/creatinine ratio, and a higher rate of acid-base disorders, but lower serum albumin,

Jejunal water and electrolyte transport in human cryptosporidiosis.

Cryptosporidiosis may have severe clinical consequences in both immunocompromised and immunocompetent individuals. However, pathophysiologic … cretory state in the proximal small intestine. We conclude that diarrhea may be due to secretion of electrolytes and water efflux more distally or to other abnormalities of gastrointestinal function.

Paresthesia and tetanic finger cramps during hyperventilation-induced respiratory alkalosis are believed to derive from a pH-dependent decre … minute hyperventilation could not be related to changes of ionized serum calcium; however the other electrolytes, i.e. sodium, magnesium, potassium, chloride, phosphate and bicarbonate, showed, with the exception of sodium, significant changes.

Acid-base and electrolyte abnormalities in alcoholic patients.

This study was undertaken to analyze the acid-base and electrolyte abnormalities and the pathogenetic mechanisms involved in alcoholic patients admitted to our department for causes related to alcohol abuse. We studied 79 alcoholic patients aged 31-78 years. None had any other disease or was receiving drugs influencing acid-base balance and electrolyte parameters. On their admission and before any therapeutic intervention laboratory investigation of the acid-base status and electrolyte parameters in both sera and urine was carried out. Thirty-two patients (40.5%) had acid-base disturbances. Te

Cardiac arrhythmias and electrolyte changes during haemodialysis.

Cardiac arrhythmias are a frequent event in chronic haemodialysis patients, and their pathogenesis is still poorly understood. We evaluated plasma K+ (PK), intraerythrocytic K+ (EK) and acid-base changes during haemodialysis in six patients with frequent arrhythmias (A-pts), and in six (used as controls) nonarrhythmic dialysis patients (C-pts). PK decreased significantly (P less than 0.01) during haemodialysis in both groups: A-pts (pre HD: 4.81 +/- 0.52 mM; 1st hour: 3.66 +/- 0.44; end HD: 3.17 +/- 0.38) and C-pts (4.75 +/- 0.80; 3.71 +/- 0.32 and 3.18 +/- 0.18 respectively) without any signi

Bartter's syndrome: a chronic electrolyte losing syndrome.

Bartter's Syndrome is a chronic electrolyte losing syndrome with symptoms and abnormalities that form an organized framework in which to study clinical effects of chronic electrolyte disturbances. Bartter's Syndrome has received minimal attention in continuing education journals. The purpose of this article is to define Bartter's Syndrome, present the historical significance, pathophysiology and clinical signs and symptoms. Selected results of a case study, with specific clearance studies and nursing implications, designed to diagnose and treat this rare renal tubular dysfunction will also be

Water, electrolyte, and endocrine homeostasis in infants with bronchiolitis.

Twenty-two of 23 consecutive infants with bronchiolitis, 5.5 +/- 3.5 mo of age, showed a 1.9 +/- 1.4% increase in body weight, increased urinary osmolality of 737 +/- 193 mmol/L with low plasma osmolality of 275 +/- 4 mmol/L, and markedly elevated plasma antidiuretic hormone (ADH) levels of 114 +/- 225 pg/mL. Increased ADH, which usually suppresses plasma renin activity, was associated with increased plasma renin activity of 11-55 ng angiotensin 1/mL/h (normal for age less than 10 ng angiotensin 1/mL/h). Hyperaldosteronism was evident from the low fractional excretion of sodium of 0.27 +/- 0.2

Fluid and electrolyte concerns in intestinal surgical procedures.

All surgical patients are at risk for fluid and electrolyte imbalances. This risk increases when a patient undergoes intestinal surgery, because of the role of the gastrointestinal system in providing the body with water and electrolytes. Therefore, throughout the perioperative period the nurse acts to prevent such disturbances. The nurse who is aware of preoperative factors that contribute to fluid and electrolyte imbalances, including pre-existing patient factors, iatrogenic causes, and the body's response to stress, can help prevent disastrous intraoperative events. Furthermore, postoperati…

Selected mechanisms of diuretic-induced electrolyte changes.

Thiazide diuretic therapy has been associated with several biochemical electrolyte imbalances including potassium loss. The onset, duration and magnitude of potassium excretion can influence decisions concerning drug therapy, particularly in hypertensive patients at risk if they develop diuretic-induced hypokalemia. The potassium depletion seen during the course of antihypertensive therapy with thiazides is brought about by 2 primary mechanisms: the increased delivery of sodium to the distal tubules for sodium-potassium exchange, and the development of secondary hyperaldosteronism, which cause

Stimulation of electrolyte secretion in rabbit colon by adenosine.

Serosal addition of adenosine after inhibition of adenosine deaminase with deoxycoformycin increases short-circuit current (Isc) and tissue conductance of isolated epithelia of rabbit descending colon. In the presence of Cl this increase in Isc results from a reversal of electrically neutral Cl absorption to rheogenic Cl secretion. When Cl is absent the stimulating effect of adenosine on Isc is reduced to one-third and appears to be brought about by HCO3 secretion. Under all conditions active Na transport remains unaltered. Adenosine-induced electrolyte secretion is markedly decreased by seros

Electrolyte and other physiological abnormalities in patients with bulimia.

The frequencies of various forms of eating-related behaviour (such as vomiting and laxative abuse) are reported for a series of non-anorectic bulimia patients seen for evaluation in an eating disorders clinic. The results of serum electrolyte, glucose and other screening tests in these patients are presented. Electrolyte abnormalities were found in 82 of the 168 patients (48.8%) who were diagnosed as having either bulimia or atypical eating disorder. The most common abnormality was metabolic alkalosis (27.4%); hypochloremia (23.8%) and hypokalemia (13.7%) were also commonly seen. No significan

Protein and electrolyte alterations in human senile cataract.

Water content, wet and dry weights of the crystalline lens, and protein, free amino acid and electrolyte levels in serum, aqueous humour and lens were determined in patients with nuclear senile cataract and at the different stages of the maturation of cortical cataract. In immature senile cortical cataract wet weight and sodium contents of the lens were significantly higher while potassium levels were low when compared with the nuclear cataract. With the maturation of cortical cataract water content, wet weight and sodium concentrations of the crystalline lens significantly increased while pro
