Hypertensive Emergencies in the Emergency Department.

Hypertension affects approximately one-third of Americans. An additional 30% are unaware that they harbor the disease. Significantly increased blood pressure constitutes a hypertensive emergency that could lead to end-organ damage. When organs such as the brain, heart, or kidney are affected, an intervention that will lower the blood pressure in several hours is indicated. Several pharmacologic options are available for treatment, with intravenous antihypertensive therapy being the cornerstone, but there is no standard of care. Careful consideration of each patient's specific complaint, histor

Imaging of head and neck emergencies.

The anatomy of the head and neck contains very few structures that could be considered expendable and, consequently, is exceptionally intolerant to infection, inflammation, and injury. Acute pathologic processes in this body region, therefore, tend to result in significant suffering, functional impairment, or life endangerment if the diagnosis is missed or treatment is delayed. Many emergent processes within the cervical region also need to be considered for their possible impact on structures within the head and chest, into which there are many routes for potential communication.

Damage control surgery for abdominal emergencies.

BACKGROUND: Damage control surgery is a management sequence initiated to reduce the risk of death in severely injured patients presenting with physiological derangement. Damage control principles have emerged as an approach in non-trauma abdominal emergencies in order to reduce mortality compared with primary definitive surgery. METHODS: A PubMed/MEDLINE literature review was conducted of data available over the past decade (up to August 2013) to gain information on current understanding of damage control surgery for abdominal surgical emergencies. Future directions for research are discussed…

Acute demyelinating disorders: emergencies and management.

Central nervous system (CNS) inflammatory demyelinating diseases are a group of disorders that include multiple sclerosis, acute disseminated encephalomyelitis, and neuromyelitis optica. These conditions may result in emergencies because of severe inflammatory destruction of CNS tissues or complications thereof. Most of these conditions are responsive to appropriate therapy and early diagnosis and treatment leads to better outcomes. We discuss the spectrum of emergencies associated with these disorders, as well as clinical features, investigations, and management.

[Girls' abdominal pains and gynaecological emergencies].

Among many causes of abdominal pain in children, gynaecological ones have to be systematically considered in even no pubescent girls. Diagnosis leans on physical and US examinations and has to screen first urgent causes before frequent ones. Rare but urgent are ovarian torsions either on a normal ovary or an underlying ovarian tumor, an earlier surgery may avoid oophorectomy. Hematocolpos linked to imperforate hymen is the most common obstructive congenital abnormality of the female genital tract and the diagnosis is easy with inspection of external genitalia. Ovarian tumors are infrequent and

Endocrine and metabolic emergencies: thyroid storm.

Thyrotoxicosis is a common endocrine condition that may be secondary to a number of underlying processes. Thyroid storm (also known as thyroid or thyrotoxic crisis) represents the severe end of the spectrum of thyrotoxicosis and is characterized by compromised organ function. Whilst rare in the modern era, the mortality rate remains high, and prompt consideration of this endocrine emergency, with specific treatments, can improve outcomes.

Respiratory emergencies in HIV-infected persons.

Respiratory complaints are a common reason for patients with HIV infection to present to an emergency department (ED). "HIV and shortness of breath" is one of the most frequent chief complaints on ED triage sheets, and the differential diagnosis is broad. Pulmonary etiologies include infectious and noninfectious causes that are related and unrelated to underlying HIV infection and range from the minor to the life threatening. This article focuses on respiratory emergencies among HIV-infected patients and discusses their typical presentation and diagnostic evaluation as well as therapeutic inte…

Management of cardiac emergencies in children.

Children with heart disease may present to the Emergency Department in many stages of life, with a range of cardiovascular manifestions, from minimally irritating palpitations to the life-threatening derangements of shock or lethal dysrhythmia. Cardiac emergencies are rare in children in comparison to adults. The pathophysiology differs: ischemic heart disease is virtually unknown, whereas most cases occur secondary to congenital heart disease. Their successful management requires an accurate diagnosis and timely interventions to achieve optimal outcomes in this heterogeneous and complex patie…

Neurological emergencies as causes of accidents.

OBJECTIVES: A large number of people throughout Europe are involved in accidents every year. Neurological emergencies as the underlying cause of accidents are poorly investigated. This retrospective chart review study was performed to evaluate the incidence of such events. METHODS: We retrospectively screened the records of the database of our physician-staffed emergency medical service for a 2-year period of time for patients involved in accidents suspicious of having had a neurological incident leading to the accident. The admission diagnoses of these records were compared with the final di…

Children who have heart disease may present to the emergency department (ED) in many stages of life with a range of cardiovascular manifestions, from minimally irritating palpitations to the life-threatening derangements of shock or lethal dysrhythmia. They can present with congenital heart disease, after a temporizing procedure has been performed or after their definitive repair. Children can also present with fever, weakness, dyspnea, syncope, or chest pain; alternatively, children may present to the ED with active dysrhythmia, pulmonary edema, or cardiogenic shock . [Emergencies in the knee joint surgery].

Anatomy of the knee joint is one of the most complicated when compared to other joints in the human body. Due to high demands and strong forces acting on the knee joint, it is also one of the most injured joints. Every structure in the joint can be damaged and some of the damages demand earlier treatment. Except intraarticular fractures, stress fractures of the proximal tibia, as a result of lower extremity malalignment, need earlier treatment to prevent the dislocation of the fragments. Damage to the articular cartilage needs earlier treatment due to its tendency to involve wider area of the

[Oncologic emergencies associated with pancreatobiliary cancer].

Clinically common oncologic emergencies associated with pancreatobiliary cancer are gastrointestinal bleeding caused by duodenal invasion of pancreatic carcinoma, severe duodenal obstruction due to pancreatic carcinoma, and acute cholangitis accompanied by obstructive jaundice in patients with biliary tract carcinoma. When a patient with gallbladder cancer presents with acute cholecysitis, emergency surgery is sometimes performed on the basis of the latter diagnosis. Emergency procedures can also be required in the perioperative management of pancreatobiliary cancer, for example, in biliary pe…

[Metabolic emergencies in the oncology patient].

Amongst the principal metabolic situations that can require emergency attention in the oncology patient we find: hypercalcaemia, hyponatraemia, tumoural lysis syndrome, lactic acidosis, hyperuricaemia, renal failure, hyperammonaemia, hypermpotasaemia, etc. Hypercalcaemia is the most frequent metabolic complication in oncology, appearing in 10-30% of these patients. It has two main mechanisms, tumoural lysis and humoural hypercalcaemia mediated by PTHrP (a protein related to parathormone). The principal factor for its diagnosis is suspicion, since some symptoms are non-specific and can be attri

Spectrum of hypertensive emergencies in pregnancy.

Hypertension in pregnancy represents a spectrum of clinical entities, including pregnancy-induced hypertension (PIH), preeclampsia, eclampsia, and hemolysis, elevated liver enzyme levels, low platelet count syndrome. Although hypertension is a common denominator in this group of disorders, the pathogenesis, clinical features, and clinical course of these disorders is variable and somewhat distinct. Therapy must be tailored to the clinical entity and the patient. The incidence and prevalence of preeclampsia and eclampsia is decreasing worldwide. This decrease partly may be caused by the improve

Pathophysiology of endothelin and medical emergencies.

Pathophysiology of endothelin-1, a vasoconstrictor and a mitogenic peptide, has been extensively investigated in recent years. The authors have examined the main clinical and experimental evidence regarding the involvement of this peptide in some medical emergencies, namely myocardial infarction, stroke and hepato-renal syndrome. Literature data suggest an emerging pathophysiological role for endothelin in such clinical conditions.

[Hypertension and hypertensive emergencies in pregnancy].

BACKGROUND: Hypertensive emergencies are acute, life threatening, and usually--but not necessarily--associated with severe increases in blood pressure. In pregnancy, this is the fact in eclampsia. Eclampsia refers to the occurrence of one or more generalized convulsions in the setting of preeclampsia with proteinuria, edema, and hypertension. PATHOGENESIS: Our current understanding of the pathogenesis of preeclampsia will be reviewed here. Some major risk factors for the development are preexisting hypertension and renal disease. PREVENTION AND THERAPY: Preventive measures of preeclampsia an…
