The landscape of adult obesity in Malaysia: a mapping of evidence and contemporary narrative review.

Malaysia has one of the highest adult overweight/obesity rates (54.4%) regionally, with prevalence projected to keep rising. We mapped and synthesized updated Malaysian adult overweight/obesity research to guide practice and policy. We searched Web of Science, PubMed, and Google Scholar (English, 2016-2024) for studies on adult overweight/obesity in Malaysia. We organized eligible studies into the scopes defined by the national Nutrition Research Priority and conducted thematic narrative synthesis. The evidence base is heavily skewed toward descriptive epidemiology (≈70%), while longitudinal c

Perinatal mental health equity: patient perspectives on virtual collaborative care providing what women need now in the post-COVID era.

INTRODUCTION: The COVID-19 pandemic increased perinatal mental health needs and disrupted routine obstetric care, disproportionately affecting Black and economically disadvantaged communities. Michigan's Perinatal Collaborative Care Model (Peri CoCM), a publicly funded virtual program, launched during the pandemic and continued after the public health emergency. This qualitative study examined participant experiences with Peri CoCM, including perceived usefulness, acceptability, and barriers/facilitators to engagement, particularly among marginalized perinatal people to inform post-COVID servi

Smoking cessation in adolescents and effective strategies: a position paper.

Adolescent nicotine dependence represents a public health challenge, driven by the rapid evolution of tobacco products and by the unique neurobiological vulnerability of the developing brain to nicotine exposure. Clinically significant dependence may emerge after brief and intermittent use, often before adolescents self-identify as users, underscoring the need for proactive and systematic screening across all nicotine and tobacco product modalities. This position paper reviews the evidence on effective cessation strategies for adolescents and provides practical recommendations for clinicians a

Discrepancies in anaphylaxis protocols across emergency medical services in the United States: Opportunities for improvement.

BACKGROUND: Across the United States, there are significant inconsistencies in the protocols used by emergency medical services (EMS) in the prehospital treatment of anaphylaxis. These discrepancies include variations in the definition of anaphylaxis and treatment recommendations. OBJECTIVE: To identify gaps in the recognition of anaphylaxis and to provide areas for improvement in prehospital management through an analysis of state-wide anaphylaxis protocols. METHODS: States with mandatory or model state-wide protocols were included (total of 30). Each allergic reaction and/or anaphylaxis pr

Management of Out-of-Hospital Cardiac Arrest during COVID-19: A Tale of Two Cities.

Variations in the impact of the COVID-19 pandemic on out-of-hospital cardiac arrest (OHCA) have been reported. We aimed to, using population-based registries, compare community response, Emergency Medical Services (EMS) interventions and outcomes of adult, EMS-treated, non-traumatic OHCA in Singapore and metropolitan Atlanta, before and during the pandemic. Associations of OHCA characteristics, pre-hospital interventions and pandemic with survival to hospital discharge were analyzed using logistic regression. There were 2084 cases during the pandemic (17 weeks from the first confirmed COVID-19

Errors in adult trauma resuscitation: a systematic review.

INTRODUCTION: Trauma resuscitation at dedicated trauma centers typically consist of ad-hoc teams performing critical tasks in a time-limited manner. This creates a high stakes environment apt or avoidable errors. Reporting of errors in trauma resuscitation is generally center-dependent and lacks common terminology. METHODS: We conducted a systematic review by searching Ovid Medline, Scopus and Embase from inception to February 24, 2021 for errors in adult trauma resuscitation. English studies published after 2001 were included. Studies were assessed by two independent reviewers for meeting in

Predictors of early and late survival after out-of-hospital cardiac arrest in which asystole was the first recorded arrhythmia on scene.

BACKGROUND: A large proportion of patients who suffer out-of-hospital cardiac arrest have asystole as the initial recorded arrhythmia. Since they have a poor prognosis, less attention has been paid to this group of patients. AIM: To describe a consecutive population of patients with out-of-hospital cardiac arrest with asystole as the first recorded arrhythmia and to try to define indicators for an increased chance of survival in this population. SETTING: The community of Gothenburg. PATIENTS: All patients who suffered out-of-hospital cardiac arrest during 1981 to 1992 and were reached by ou

Japan as the front-runner of super-aged societies: Perspectives from medicine and medical care in Japan.

BACKGROUND: The demographic structure of a country changes dramatically with increasing trends toward general population aging and declining birth rates. In Japan, the percentage of the elderly population (aged ≥65 years) reached 25% in 2013; it is expected to exceed 30% in 2025 and reach 39.9% in 2060. The national total population has been decreasing steadily since its peak reached in 2008, and it is expected to fall to the order of 80 million in 2060. Of the total population, those aged ≥75 years accounted for 12.3% as of 2013, and this is expected to reach 26.9% in 2060. As the demographic

Improving Management of Chest Pain with a High Sensitivity Troponin-Based Protocol.

Chest pain is one of the most common presenting complaints to emergency departments in the United States, and management centers on identifying myocardial infarction or other forms of rare but problematic cardiac diagnoses. The high-sensitivity troponin assay can detect abnormal troponin elevations at 10- to 100-fold lower levels compared with traditional troponin assays and thus can allow faster time to disposition and diagnosis, yet adoption has not been universal. Implementing a high-sensitivity troponin protocol with a risk prediction algorithm can decrease the numbers of patients admitted

Access challenges to opioid use disorder treatment among individuals experiencing homelessness: Voices from the streets.

BACKGROUND: Achieving equitable access to medications for opioid use disorder (MOUD) such as buprenorphine is a pressing issue. Evidence suggests disparities in MOUD access based on race and socioeconomic status, further exacerbated by the COVID-19 pandemic. However, the drivers behind this access gap remain poorly understood. This study explores barriers to treatment access among individuals with opioid use disorder (OUD) experiencing homelessness. METHODS: We interviewed 28 individuals in and around the Boston Public Health Commission (BPHC) Engagement Center, an area known for its high den

Severe Brain Injury in Massachusetts: Assessing the Continuum of Care.

Acquired brain injury (ABI) is a major public health problem in Massachusetts (Hackman et al, 2014) and includes traumatic brain injury (TBI), stroke, ABI-related infectious diseases, metabolic disorders affecting the central nervous system (brain and spinal cord), and brain tumor. Advances in emergency medical care and neurosurgery mean that more people are surviving severe traumatic brain injury (Trexler et al, 2014). Yet many patients with severe TBI in particular, are not receiving inpatient services after initial treatment (Hackman et al, 2014; CDC, 2014) or later that are known to be eff

Identifying and profiling prearrival characteristics of avoidable emergency department visits transported by paramedics: a cohort study using linked prehospital and hospital data.

BACKGROUND: Increasing demand and crowding in emergency departments (EDs) remain persistent challenges for healthcare systems worldwide. A portion of these visits is avoidable, indicating they could have been effectively managed in non-ED care settings. There has been increased attention on paramedics redirecting avoidable visits to community-based care before ED transport. However, limited evidence exists to identify which patients might be suitable for non-ED care models, particularly based on prehospital clinical presentations. This study aimed to examine the patient characteristics associa
