Socio-demographic characteristics and pre-hospital care of children with circulatory failure in a children's emergency room in southern Nigeria.

INTRODUCTION: circulatory failure is a major childhood emergency. Several disease-related and patient-related factors can predispose children to shock. Early detection of such factors will improve its prevention, management and outcome. This study aimed to evaluate the incidence, socio-demographic characteristics and pre-hospital care of children presenting with circulatory failure (shock) in children´s emergency room (CHER). METHODS: this study adopted cross-sectional design in CHER of the University of Benin Teaching Hospital, Nigeria, from October 2018 to March 2019. Data were collected us

[Guidelines for severe multiple and multiorgan traumatic injuries].

Traumatic injuries have been described as the largest epidemic of the 20th century. In view of the number of victims and the associated costs, they have been also called the most severe and longest war of the contemporary world. According to Lipiński, every year every tenth Pole is involved in an accident and every one hundred-thirteenth Polish citizen requires specialist medical care. Thus, the general incidence is approximately 750 accidents per 100,000 Poles per year. 300,000 of them need hospital treatment, 30,000 die from accidents. Despite considerable progress in medical sciences and pr

Impact of a phenytoin loading dose program in the emergency department.

PURPOSE: The use of a combined physician-and pharmacist-directed phenytoin loading dose program in an emergency department (ED) was evaluated. METHODS: This single-center, observational, preimplementation-postimplementation study evaluated adult patients who received a phenytoin loading dose in the ED. The primary outcome compared the proportion of optimal phenytoin loading doses in the preimplementation and postimplementation groups. The postimplementation group was further stratified into pharmacist- and prescriber-dosing groups. Other outcomes evaluated included the numbers of appropriate

Future applications for emergency ultrasound.

US has become an integral part of patient evaluation in emergency medicine. As discussed in this article, other potential uses for US are relatively early in their development. With the increased interest and study, however, there exists a real potential for US to be employed in an expanded clinical role in the evaluation of traumatically injured patients outside the standard FAST examination, for acute and chronic musculoskeletal injuries, and for triage of patients in disasters.

Validation of Medicaid claim-based opioid treatment program retention quality measures.

INTRODUCTION: Understanding the quality of care that opioid treatment programs (OTPs) deliver is a long-standing goal in the United States. In a prior study, this study team developed casemix adjusted OTP-level quality measures of patient retention that revealed a median 180-day OTP retention rate of 26% (25th percentile = 11%; 75th percentile = 37%) in Measurement Year (MY) 2023 (July 1, 2022 - June 30, 2023). The present study evaluated the measures' validity, particularly the association between each OTP's retention and their rate of substance use disorder (SUD) hospitalizations or emergenc

ED overcrowding spurs interest in quality and credentialing issues.

Quality in emergency medicine is not a new concept. But applying quality principles to emergency department operations is another thing altogether. While the JCAHO and the American College of Emergency Physicians are examining quality issues in emergency medicine, individual hospitals are looking for solutions to the quality question. And they are accepting the reality that hiring and retaining high-quality physicians remains a key element.

[ENT emergencies in a tertiary hospital. Study of frequency and etiology].

We have reviewed 19,061 emergencies attended in the ENT Department of our Hospital, over a period of 3 years. And analyzed the aetiologic distribution and the frequency of the cases, finding out the most frequent pathology were the external otitis followed by epistaxis. In the paper are also considered their relation with other parameters such as sex (no differences), age (25-35 years) and seriousness, this latter not being of gravity. Finally, we've studied the amount and degree of appropriteness of the visits attended and found out that near half of the cases could be attended in a less spec

A multidisciplinary Care Coordination Team improves emergency department discharge planning practice.

In response to difficulties meeting the demand for hospital services ("access block") at Royal Melbourne Hospital, a major metropolitan tertiary referral hospital, an audit of patient needs revealed a shortage of aged-care beds and a need for post-acute care. A multidisciplinary Care Coordination Team (CCT) was formed at the end of July 2000 to ensure that emergency department patients were provided with services that would facilitate their return to, or maintenance in, the community. The target population included the frail elderly, those living alone, the homeless, frequent emergency departm

Emergency ambulance service involvement with residential care homes in the support of older people with dementia: an observational study.

BACKGROUND: Older people resident in care homes have a limited life expectancy and approximately two-thirds have limited mental capacity. Despite initiatives to reduce unplanned hospital admissions for this population, little is known about the involvement of emergency services in supporting residents in these settings. METHODS: This paper reports on a longitudinal study that tracked the involvement of emergency ambulance personnel in the support of older people with dementia, resident in care homes with no on-site nursing providing personal care only. 133 residents with dementia across 6 car

A mapping review of interventions to address patients who frequently seek care in the emergency department.

BACKGROUND: The high utilization of acute care services, particularly emergency departments (ED), continues to be a significant concern for healthcare providers. Numerous approaches have been studied to meet the care needs of patients who frequently seek care in the ED; however, there is no comprehensive review of the current literature base. As such, a current understanding of the interventions initiated within the ED to address the needs of frequent users is required. This mapping review had three objectives: identify the characteristics associated with the need to frequently seek care in th

Minnesota Data Sharing May Be Model For Near-Real-Time Tracking Of Drug Overdose Hospital And ED Trends.

The drug overdose epidemic in the US necessitates detailed and timely data to inform public health responses. In this article we describe how an electronic health record (EHR) data-sharing collaboration across health systems in Minnesota that was developed in response to the COVID-19 pandemic was adapted to monitor trends in substance use-related hospital and emergency department (ED) visits. We found large increases in methamphetamine- and opioid-involved hospital and ED visits. Throughout the study period, Native American, Black, and multiple-race people experienced the highest rates of drug

Outcomes for Emergency Department Final Diagnosis of Non-specific Complaint compared to Dyspnoea and Pain.

AIM: To compare outcomes in Emergency Department (ED) final diagnoses of (non-specific complaint) NSC, dyspnoea and pain. METHODS: We studied all ED final diagnoses of NSC, dyspnoea, and pain over 6 years (2015-2020). Multivariable logistic regression was performed. RESULTS: There were 49,965 admissions. 30-day in-hospital mortality was significantly lower for pain, 3.0% (95%CI 2.4%, 3.6%), compared to NSC, 4.2% (95%CI 3.8%, 4.7%), and dyspnoea, 4.6% (95%CI 4.2%, 5.0%). NSC did not predict 30-day in-hospital mortality- univariate OR 1.05 (95%CI 0.93, 1.19), multivariable OR 1.07 (95%CI 0.93,

Evaluation of Fluid Overload by Bioelectrical Impedance Vectorial Analysis.

Early detection and management of fluid overload are critically important in acute illness, as the impact of therapeutic intervention can result in decreased or increased mortality rates. Accurate fluid status assessment entails appropriate therapy. Unfortunately, as the gold standard method of radioisotopic fluid measurement is costly, time-consuming, and lacks sensitivity in the acute care clinical setting, other less-accurate methods are typically used, such as clinical examination or 24 h output. Bioelectrical impedance vectorial analysis (BIVA) is an alternative impedance-based approach,

A Mixed Methods Evaluation of Interventions to Meet the Requirements of California Senate Bill 1152 in the Emergency Departments of a Public Hospital System.

UNLABELLED: Policy Points Clarifications to Senate Bill (SB) 1152 are necessary to address the differences between inpatient and emergency department (ED) discharge processes, determine how frequently an ED must deliver the SB 1152 bundle of services to a single patient, and establish expectations for compliance during off-hours when social services are unavailable. Because homelessness cannot be resolved in a single ED visit, the state should provide funding to support housing-focused case workers that will follow patients experiencing homelessness (PEH) through the transition from temporary
