Altered mapping of sound frequency to cochlear place in ears with endolymphatic hydrops provide insight into the pitch anomaly of diplacusis.

A fundamental property of mammalian hearing is the conversion of sound pressure into a frequency-specific place of maximum vibration along t … trated. Here we assess cochlear frequency-place maps in guinea-pig ears with experimentally-induced endolymphatic hydrops, a hallmark of Ménière's disease. Our findings are consistent with the hypothesis that diplacusis is due to an altered cochlear map. Map changes can lead to altered pitch, but the size of the pitch change is also affected by neural synchrony. Our da …

Comparison of furosemide-loading cervical vestibular-evoked myogenic potentials with magnetic resonance imaging for the evaluation of endolymphatic hydrops.

BACKGROUND: Furosemide-loading cervical vestibular-evoked myogenic potential (FVEMP), in which vestibular function is improved via diuretics-induced dehydration, can be used to estimate the presence of endolymphatic hydrops, one characteristic of Menière's disease. Inner ear magnetic resonance imaging (MRI) can also reveal endolymphatic hydrops.Aims/Objective: This study aimed to compare and confirm the usefulness of these two examination methods for the diagnosis of Menière's disease. METHODS: Twenty patients with definite unilateral Menière's disease were included. All subjects underwent bo…

Clinical characteristics of delayed endolymphatic hydrops in Japan: A nationwide survey by the Peripheral Vestibular Disorder Research Committee of Japan.

CONCLUSION: Similarly to almost all delayed endolymphatic hydrops (DEH) cases with both precedent sudden deafness and mumps deafness, two-thirds of DEH cases with precedent deafness of unknown cause with onset in early childhood developed DEH symptoms within 40 years after the precedent deafness. In spite of the diagnosis of precedent deafness, viral labyrinthitis may build up the late endolymphatic hydrops in most DEH cases up to four decades. OBJECTIVE: To clarify the characteristics of DEH in Japan. METHODS: Clinical information on 198 DEH cases was collected by nationwide, multicenter su…

Visualization of endolymphatic hydrops in Ménière's disease after single-dose intravenous gadolinium-based contrast medium: timing of optimal enhancement.

PURPOSE: Visualization of endolymphatic hydrops (EH) in patients with Ménière's disease (MD) is now possible by heavily T(2)-weighted 3-dimensional fluid-attenuated inversion recovery (hT(2)W-3D-FLAIR) obtained 4 hours after intravenous (IV) administration of single dose gadolinium-based contrast medium (GBCM). Although maximum enhancement has been reported 4 hours after contrast administration in healthy volunteers, the timing of optimal enhancement in patients with MD is not reported. We investigated if that optimal timing is earlier or later than 4 hours. MATERIALS AND METHODS: We evaluate…

Visualization of endolymphatic hydrops after intratympanic injection of Gd-DTPA: comparison of 2D and 3D real inversion recovery imaging.

PURPOSE: Endolymphatic hydrops of Ménière's disease has been visualized after intratympanic injection of gadopentetate dimeglumine (Gd-DTPA) using a 3-dimensional (3D) inversion-recovery sequence with real reconstruction (3D real IR). This technique enables differentiation of bone and endo- and perilymph space on a single image but requires 15 min of scan time. Therefore, we compared it with 2D real IR, which is faster. MATERIALS AND METHODS: We investigated 10 ears in 9 patients with suspected Ménière's disease. Twenty-four hours after intratympanic administration of 8-fold diluted Gd-DTPA, …

MRI of endolymphatic hydrops in patients with Meniere's disease: a case-controlled study with a simplified classification based on saccular morphology.

OBJECTIVES: Endolymphatic hydrops (EH) can be studied in patients by MRI. With the semi-quantitative grading system, previous imaging studies showed discrepancies in the occurrence and grading of EH in patients with Meniere's disease (MD). Here, we compared the inversion of the saccule to utricle area ratio (SURI) with the semi-quantitative method of grading conventionally used to diagnose MD. METHODS: Imaging was carried out on a 3-T MRI scanner. We performed 3D-FLAIR sequences 4 h after a single intravenous dose of contrast agent. Two radiologists independently studied the morphology of the…

Intratympanic steroids injection is effective for the treatment of drop attacks with Ménière's disease and delayed endolymphatic hydrops: A retrospective study.

Drop attack (DA) associated with Ménière's disease (MD) and delayed endolymphatic hydrops (DEH) is not common and may cause life-threatening clinical problems. The intratympanic dexamethasone (ITD) is one of primary treatments for MD or DEH. Our study investigated the effect of ITD on the DA associated with endolymphatic hydrops (EH).We retrospectively reviewed 10 patients with MD- and DEH-associated DA between January 2009 and December 2013 in Outpatient Department of Otolaryngology, Union Hospital, Wuhan, China. Among them, 7 patients (5 cases with MD, 2 cases of DEH) received ITD (4 times, …

Measuring traveling wave velocity in the basilar membrane as a potential indicator of endolymphatic hydrops in definite Ménière's disease: a narrative review.

BACKGROUND: The pathological hallmark of Ménière's disease is endolymphatic hydrops, which can lead to an increase in basilar membrane stiffness and, consequently, an acceleration of the traveling wave of sound. The cochlear hydrops analysis masking procedure (CHAMP), which is an auditory brainstem response test masked at various frequencies with high-pass noise masking, uses the principle of the traveling wave velocity theory to determine the presence of endolymphatic hydrops. PURPOSE: This study aimed to review the previous results of the CHAMP, expound the principles and key indicators, an…
