Efficacy of endolymphatic duct blockage in treating Ménière's disease.

OBJECTIVES: Ménière's disease (MD) is an idiopathic inner ear disorder characterized by recurrent episodes of episodic rotational vertigo, fluctuating hearing loss, tinnitus, and a feeling of ear stuffiness. Endolymphatic sac (ES)-related surgery is used primarily in patients with MD who have failed to respond to pharmacologic therapy. Endolymphatic duct blockage (EDB) is a new procedure for the treatment of MD, and related clinical studies are still scarce. This study aims to investigate the dynamic changes in endolymphatic hydrops (EH) and the long-term surgical outcomes in MD patients under…

Experimental study on Meniere's disease.

Experimental hydrops caused by underabsorption of endolymphatic fluid is a model of remissional stage of Meniere's disease. In this study, another type of model, ie, hydrops caused by overproduction of endolymphatic fluid, was accomplished by applying various pressures into scala media through a micropipette via stria vascularis. This type of hydrops could be a model of attacks of Meniere's disease. By using two types of the model, effects of glycerol administration and of opening the endolymphatic sac were discussed.

The use of intraoperative electrocochleography in Meniére's surgery.

Intra-operative electrocochleography was undertaken during surgery for Meniére's disease. It was found that the electrocochleogram (ECoG) provided a stable measure which accurately reflected changes within the inner ear during surgery. During salt osmosis of the round window, a rapid change in the summating potential versus action potential ratio (SP/AP) occurred which showed when the endolymphatic hydrops (ELH) had been altered. During a modified cochleostomy procedure, the ECoG showed when the cochlear duct had been ruptured. Forty ears were monitored during endolymphatic sac surgery. Twenty…

Endolymphatic pressure in experimental hydrops.

Perilymphatic and endolymphatic pressures were consecutively measured by means of a servomechanical nulling pressure system through the round window and basilar membrane one to 2.5 months after obliteration of the left endolymphatic sac and duct in seven guinea pigs. The position of the pipette was monitored by the simultaneous recording of the direct current potential through the micropipette used for the pressure measurement. (1) The perilymphatic and endolymphatic pressures of the control ears were equal. (2) The histological evaluation showed that the ears operated on had a significantly g…

Three new surgeries for treatment of intractable Meniere's disease.

OBJECTIVE: This study aimed to describe the rationales for and preliminary results of three new types of surgery for the treatment of intractable Meniere's disease, all involving insertion of a capillary tube into the endolymphatic duct. This study also aimed to compare the contrasting surgical strategies of endolymphatic sac enhancement versus sac supplantation. STUDY DESIGN AND SETTING: The study design was a retrospective review of 129 surgeries conducted by the author at Chang Gung Memorial Hospital since 1993: 51 cases of Huang/Gibson inner ear shunt implantation, 52 cases of intraductal…

The Portmann procedure after sixty years.

This article recalls the technique described by Georges Portmann in 1926 for the opening of the endolymphatic sac in cases of Meniere's disease. Criteria of indication used at the time are critically analyzed. Modifications of the original operation proposed by various authors, particularly those of T. Naito, W. House, A.W. Morrison, and I.K. Arenberg, are then described. Finally, the current technique and indication criteria implemented in Bordeaux are analyzed, with stress on electrocochleographic determination of the summation potential and its disappearance under the effect of glycerol. Ac…

Menière's disease: results of treatment with the endolymphatic subarachnoid shunt compared with the endolymphatic mastoid shunt.

The senior author has performed 346 endolymphatic sac operations since 1971. Questionnaires were sent to these patients to evaluate the results of this surgery. One hundred ninety-six questionnaires were returned and formed the basis for this report. The results of surgery are reported by the new American Academy of Otolaryngology--Head and Neck Surgery criteria as well as by the older method of reporting. There is no significant difference in results between the endolymphatic subarachnoid shunt and the endolymphatic mastoid shunt. Because of the lower morbidity of the latter procedure the aut…

Animal models of endolymphatic hydrops.

Endolymphatic hydrops in the animal can be produced by various methods. The most promising is obliteration of the endolymphatic duct in the guinea pig. The extent of hydrops is similar to that of Ménière's disease specimens, but the animals are asymptomatic. A better model, utilizing a less invasive method and having the ability to elicit episodic vestibular symptoms, is needed. There is a reasonable doubt that one of the causes for Ménière's disease lies in the abnormal endolymphatic duct and sac. A wide ablation of such pathologic tissue may provide additional pathogenetic information.

An experimental model for the study of endolymphatic hydrops in sharks: implications for the clinician.

In recent years, attention has focused on the role of the endolymphatic sac (ELS) and the endolymphatic duct (ELD) in the pathogenesis of endolymphatic hydrops (ELH). Changes have been noted surgically and radiographically by others in the ELS and ELD in patients with ELH. This report summarizes the development of a shark model with which to study the pathophysiology of ELH. The background material and overall results of anatomic, histopathologic, and ultrastructural studies using the model are presented. Possible implications for the clinical handling of ELH as a result of this work is emphas…

Despite historical controversy surrounding the use of endolymphatic sac (ELS) surgery, the use of ELS decompression has been widely adopted by the international neurotologic community due to high vertigo control rate, coupled with low risk of audiovestibular loss. Wider decompression of the sac and surgical manipulation …

Cochlear ultrastructure in two-phase endolymphatic hydrops in the guinea-pig.

Endolymphatic hydrops continues to be considered as a pathological factor in the etiology of Meniere's disease. We have developed the two-phase endolymphatic hydrops model, which seems to represent a functional model combining multiple etiologies, and which may resemble the fluctuating characteristics of Meniere's disease. A transmission electron microscopic study was performed on the endolymphatic sacs of four groups of guinea-pig cochleas: (1) controls, (2) non-operated, aldosterone-treated cochleas, (3) operated (dissection of the endolymphatic sac) cochleas, (4) operated and aldosterone-tr…

Live imaging and functional changes of the inner ear in an animal model of Meniere's disease.

The symptoms of Meniere's disease (MD) are generally considered to be related to endolymphatic hydrops (EH). There are many recent reports supporting the possibility that vasopressin (VP) is closely linked to the formation of EH in Meniere's disease. Based on this, we developed a clinically relevant animal model of Meniere's disease in which a VP type 2 receptor agonist was administered after electrocauterization of the endolymphatic sac. We report live imaging of the internal structure, and functional changes of the inner ear after electrocauterization of the endolymphatic sac and administrat…

Streptomycin perfusion of the labyrinth.

The combination of fluctuant hearing loss, fullness, tinnitus and dizzy spells we call Meniere's disease is thought to be caused by endolymp … endolymph, while doing harm to the fluid absorption, immune response and phagocytosis roles of the endolymphatic sac. The various vestibular neurectomy operations, while usually stopping the dizzy attacks, are both difficult and potentially dangerous, but more important, do nothing for the hearing loss. The one direct attack on the problem, both easy to perform an …

The influence of noise exposure on endolymphatic hydrops. An experimental study.

The influence of noise exposure on endolymphatic hydrops was examined morphologically. Endolymphatic hydrops was produced experimentally in guinea pigs by means of endolymphatic sac obliteration. Approximately 2 months later, the animals were subjected to noise exposure for 2 h and, as a result, the following findings were obtained: Vascular and circulatory disturbances in the 3rd and 4th turns of cochlea, i.e., vasoconstriction-like changes in the upper coiled arterioles and capillaries in the stria vascularis, as well as blood cell sludge. In addition, the outer hair cells in Corti's organ a…

The measurement and manipulation of intralabyrinthine pressure in experimental endolymphatic hydrops.

Three to four months after unilateral surgical ablation of the endolymphatic duct and sac, endolymphatic and perilymphatic pressures were measured in both the normal and hydropic ears of 11 guinea pigs. In normal ears, endolymphatic pressure always approximated perilymphatic pressure. Endolymphatic pressure exceeded perilymphatic pressure in all ears with hydrops, except one in which these pressures were equal. The effect of postural inversion on inner ear pressures were studied in both normal and hydropic inner ears. Normal ears showed endolymphatic and perilymphatic pressure to rise equally

[The value of saccotomy in Menière disease. A long-term analysis of 42 cases].

MATERIAL AND METHODS: Saccotomy is well known to be a specific alternative treatment for patients suffering from conservatively intractable Meniere's disease. In a long-term study, we investigated the results in hearing and vertigo after this operation and evaluated the subjective contentment of our patients. For the first time, we focused on the influence of an intraoperatively observed fibrotic narrowing (11 cases) of the endolymphatic sac ("sac fibrosis") on these results. RESULTS: We found complete restitution or significant improvement of the typical vertigo attacks after saccotomy in mo…

Is cochlear synapse loss an origin of low-frequency hearing loss associated with endolymphatic hydrops?

There is a strong association between endolymphatic hydrops and low-frequency hearing loss, but the origin of the hearing loss remains unkno … ring. We overcame this obstacle with the combined use of a reliable surgical approach to ablate the endolymphatic sac in guinea pigs and create endolymphatic hydrops, the Auditory Nerve Overlapped Waveform to measure low-frequency hearing loss (≤ 1 kHz), and immunohistofluorescence-based confocal microscopy to count cochlear synapses. Results showed low- and mid-(1 …

Blockage of longitudinal flow in endolymphatic hydrops.

The external shunt operation on the endolymphatic sac is based on the concept that it will drain excess endolymph and thus arrest the progression of endolymphatic hydrops. We performed histological studies on 46 temporal bones from 13 cases of bilateral and 20 cases of unilateral Meniere's disease to evaluate the status of the pathways of longitudinal flow of endolymph to the sac. The endolymphatic ducts were blocked in 8 specimens (17%), the endolymphatic sinuses in 9 (19.5%), the utricular ducts in 12 (26%), the saccular ducts in 7 (15%), and the ductus reuniens in 27 (59%). These blockages …
