Necrotizing enterocolitis (NEC) is the leading cause of death from gastrointestinal disease in neonates, and is characterized by the development of diffuse intestinal necrosis in the stressed, pre-term infant. Systemic stress causes a breakdown in the intestinal mucosal barrier, which leads to translocation of bacteria and endotoxin and the initiation of a signaling response within the enterocyte. This review summarizes recent evidence defining a clear role that defective enterocyte signaling plays in the pathogenesis of NEC through the following mechanisms: 1) The localized production of nitr

Thrombocytopaenia occurs during necrotizing enterocolitis (NEC), and nadir platelet count is associated with extent of disease. In paediatric meningococcal disease, the product of neutrophil and platelet count at admission is prognostically useful. We therefore aimed to determine whether the first full blood count (FBC) after diagnosis of NEC is useful as a score for poor outcome and severity of disease. Between 1987 to 2001, neutrophils (N), platelets (P) and their product (PN) was available in 187 neonates treated for NEC at our institution. Neonates with NEC were grouped according to the ex

An outbreak of enteric disease occurred in a herd of Nigerian Dwarf goats in Indiana, USA, in March 2024. Kids 2-5-wk-old had severe diarrhea that did not respond to treatment. Two kids that underwent postmortem examination had gross and histologic evidence of fibrinonecrotizing typhlocolitis. Ancillary testing for known causes of enterocolitis in goats did not yield any potential causes. High-throughput sequencing (HTS) was performed to identify possible novel viral pathogens. Total nucleic acids were extracted from fresh intestine for HTS analysis, and a subset of astrovirus reads was obtain

BACKGROUND: The diagnosis and surgical prediction of necrotizing enterocolitis (NEC) remain challenging. Our goal is to develop an interpretable multimodal artificial intelligence model to assist these key clinical decisions.
METHODS: This retrospective study included 484 neonates (242 with NEC, 242 without NEC). We developed a dual Swin Transformer integrating abdominal X-rays (2D branch) and laboratory parameters (1D branch) via late fusion. The model was refined using an external data domain adaptation strategy (n = 50) and evaluated on independent internal and external test sets. The inte

Necrotizing enterocolitis (NEC) is a life-threatening intestinal disease of preterm infants, with a complex pathogenesis involving inflammation, oxidative stress, and cell death. In recent years, ferroptosis, a novel iron-dependent form of programmed cell death, has attracted increasing attention for its role in NEC. This review systematically summarizes the mechanisms of ferroptosis and its relationship with NEC, elucidates its roles in intestinal barrier injury, inflammatory response, and oxidative stress in NEC, and explores potential intervention strategies, aiming to provide new direction

Necrotizing enterocolitis (NEC) remains one of the most devastating gastrointestinal emergencies in neonates and also presents major diagnostic challenges. Despite extensive research, NEC still lacks a practical definition and relies on a set of nonspecific clinical, laboratory, and radiological findings rather than a single pathognomonic presentation or test. The modified Bell staging system remains the most widely used framework in clinical practice and research, but it was originally developed to guide treatment decisions rather than aid diagnosis and has important limitations when applied

The use of Santulli enterostomy (SE) for necrotizing enterocolitis (NEC) has been limited to a small number of studies involving a small number of patients and no control group. Our study aimed to compare the clinical safety and efficacy of Santulli enterostomy with those of single- or double-lumen enterostomy for neonatal NEC through a retrospective cohort study. One hundred ten patients who met the criteria were divided into an SE group (64 patients) and a conventional enterostomy (CE) group (46 patients). The CE group underwent single- or double-lumen enterostomy. There were no significant

INTRODUCTION: Previous literature suggests infants of multiple gestation pregnancies have a higher incidence of necrotizing enterocolitis (NEC) than singleton gestations; however, no study has examined the likelihood of co-incident NEC among multiples. We hypothesized an increased incidence of NEC among multiples whose siblings were diagnosed with NEC.
METHODS: The admissions of infants to neonatal intensive care units were abstracted from the Pediatric Health Information System between 2017 and 2023. Multiple gestational sets were identified by diagnostic code indicating multiple gestation,

PURPOSE: Necrotizing enterocolitis (NEC) is a severely life-threatening gastrointestinal emergency in newborns, especially when surgical intervention is needed. The ideal time for resuming oral feeding after surgery is still unclear, leading to variations in practice and prolonged parenteral nutrition. This study aims to develop and validate a scoring system to determine the best time to reintroduce oral feeding in neonates following surgical NEC.
METHODS: A multicenter retrospective study was conducted from 2016 to 2024, including 500 neonates with surgical NEC (Bell's Stage III-IV). Patient

OBJECTIVE: The analysis of abdominal radiography is essential for the diagnosis and management of necrotizing enterocolitis (NEC) in newborns (NB). Studies, however, show a lack of agreement among physicians in the interpretation of images. This study aims to evaluate the agreement in the radiological interpretation of the NEC between examiners from different specialties (interexaminer analysis) and between the same examiner at different times (intraexaminer analysis).
METHODS: Cross-sectional study for concordance analysis using plain radiographs of the abdomen of NB with suspected or confir

An 18-y-old American Saddlebred mare was admitted with fever and acute onset of neurologic signs including grade 3 of 5 ataxia, difficulty in prehension, and dull mentation. Because of financial restraints, desired testing could not be performed; the horse's condition declined despite supportive treatment, and euthanasia was elected. Postmortem examination revealed petechiae and ecchymoses in the meninges and neuroparenchyma of the encephalon. Blast-like neoplastic round cells were identified within the vasculature and areas of hemorrhage in the neuroparenchyma, the intestinal submucosa, and o

UNLABELLED: While neonatal necrotising enterocolitis (NEC) is associated with high mortality rates in newborns, survivors can face long-term sequelae. However, the relationship between NEC and neurodevelopmental impairment (NDI) in preterm infants remains unclear. To explore the relationship between neonatal NEC and neurodevelopmental outcomes in preterm infants, we searched PubMed, EMBASE, and the Cochrane Library from their inception to February 2024 for relevant studies. Studies included were cohort or case-control studies reporting neurodevelopmental outcomes of NEC in preterm infants. Two

BACKGROUND: Necrotizing enterocolitis (NEC) is one of the main causes of acute abdomen in neonates. Surgical treatment entails important morbidity and mortality and conservative management, when possible, offers better outcomes. Post-NEC intestinal strictures are one of the main complications.
METHODS: Retrospective analysis from June 2011 to November 2022 of post-NEC strictures (PNS) after conservative management of neonates diagnosed with NEC (modified Bell stage IIA or higher) at a tertiary neonatal surgery center.
RESULTS: Out of 219 NEC, 126 received initial conservative management (57.

Necrotizing enterocolitis (NEC) occurs primarily in immature preterm infants with low birth weight. It has a prevalence of 7 - 11 % with a poor prognosis and a mortality rate of 20 - 40 %. Plain abdominal radiography is still considered the diagnostic method of first choice even though ultrasound can visualize significantly more findings suspicious for NEC. Such as thickened bowel walls, dilated fluid-filled bowel loops, and gas bubbles in the intestinal wall, portal vein. Moreover, it is possible to assess intestinal peristalsis, to differentiate between intraluminal and extraluminal fluid an

This state-of-the-art review article aims to highlight the most recent evidence about the therapeutic options of surgical necrotizing enterocolitis, focusing on the molecular basis of the gut-brain axis in relevance to the neurodevelopmental outcomes of primary peritoneal drainage and primary laparotomy. Current evidence favors primary laparotomy over primary peritoneal drainage as regards neurodevelopment in the surgical treatment of necrotizing enterocolitis. The added exposure to inhalational anesthesia in infants undergoing primary laparotomy is an additional confounding variable but requi

Small for gestational age (SGA) infants have an increased risk of necrotizing enterocolitis (NEC), but SGA has been found to not be a risk factor for the deterioration of NEC in previous literature. Few studies have focused on correlative factors of the progression of NEC in SGA newborns. The present retrospective observational study was performed in 64 SGA infants with Bell's stage II NEC. The dependent variable was Bell's stage II NEC that progressed to stage III after diagnosis. A stepwise forward multivariate logistic regression model was used to select potential correlative factors for th
