Enthesitis may be one of the signs of severe disease in familial Mediterranean fever.

OBJECTIVE: Familial Mediterranean fever (FMF) is an auto-inflammatory disease that is also characterized with some of the common musculoskeletal features of spondyloarthritis (SpA). Enthesitis is the hallmark of SpA. Recently, it was postulated that exertional leg pain is a possible sign of lower extremity enthesitis associated with FMF severity. In this study, we have evaluated the association between the enthesitis, enthesitis score and disease severity in FMF patients. METHODS: We enrolled 238 FMF patients that fulfilled the modified Tel-Hashomer criteria. We assessed the presence of enthe

The Relationship Between Physical Examination and Ultrasonography of Large Entheses of the Achilles Tendon and Patellar Tendon Origin.

OBJECTIVE: To investigate the relationship between physical examination (PE) and sonographic features of enthesitis, based on anatomical sites. METHODS: The analysis was done using merged raw data of 3 studies on 2298 entheses. RESULTS: Patients with clinical Achilles enthesitis had more abnormalities on ultrasound (US): hypoechogenicity, p < 0.001; thickening, p = 0.001; Doppler signals, p = 0.002; and erosions, p = 0.02. The patellar tendon origin also correlated with PE but distal patellar tendon insertion and plantar aponeurosis were uncoupled from the US. CONCLUSION: The relationship b

Is entheses ultrasound reliable? A reading Latin American exercise.

The objective of this study is to evaluate inter-reader entheses ultrasound (US) reliability and the influence of the type of image or degree of sonographer experience on US reliability in patients with spondyloarthritis (SpA). Eighteen Latin American ultrasonographers with different experience took part in an US reading exercise evaluating 60 entheseal images (50 % static images and 50 % videos) from healthy controls and SpA patients. The following sonographic lesions were assessed: structure, thickness, bone proliferation/tendon calcification, erosions, bursitis, and Doppler signal. Another

X-Linked Hypophosphatemia Caused by the Prevailing North American PHEX Variant c.*231A>G; Exon 13-15 Duplication Is Often Misdiagnosed as Ankylosing Spondylitis and Manifests in Both Men and Women.

Inactivating mutations of the gene coding for phosphate-regulating endopeptidase homolog X-linked (PHEX) cause X-linked hypophosphatemia (XLH). A novel PHEX variant, c.*231A>G; exon 13-15 duplication, has emerged as a common cause of XLH in North America, emphasizing the importance of delineating its clinical presentation. Here, a comprehensive description of a five-generation American kindred of 22 treatment-naïve individuals harboring the c.*231A>G; exon 13-15 duplication is provided. After XLH was diagnosed in the proposita, pro-active family members used social media to facilitate a timely

Unusual cause of dysphagia and dysphonia.

Forestier's disease is an idiopathic noninflammatory condition associated with enthesopathy leading to hyperostosis of the vertebrae and peripheral skeletal system. The disease tends to affect elderly individuals and remains asymptomatic in most of the cases. Uncommonly, the patient may present with upper aerodigestive symptomatology, usually dysphagia. In elderly individuals, the disease may closely mimic upper aerodigestive tract malignancy, which should be actively excluded. In our patient, the hypopharyngeal soft tissue distortions created by the bony hypertrophy shifted the clinicoradiolo…

Chronic inflammatory polyarthritides in a select population of young men. A prospective study.

Twenty young servicemen suffering from chronic inflammatory polyarthritides (CIP) were studied. While rheumatoid arthritis (RA) was the final diagnosis in 15 patients, the arthritis remained 'unclassifiable' in the rest. An acute onset of asymmetric deforming arthritis, dominant in the lower limbs, was the usual pattern. Sacroiliitis and enthesopathy were uncommon. Extra-articular features were sparse and sub-cutaneous nodules absent. Rheumatoid factor was present in 86.7% of RA patients. Radiologic erosions were evident in 73.3% RA patients. Arthritis robustus variant of RA was diagnosed in 4…

Evaluation of the agreement between the ACR 1990 fibromyalgia tender points and an enthesitis score in patients with axial spondyloarthritis.

OBJECTIVES: Coexistence of FM represents a challenge in the evaluation of enthesitis in patients with axial spondyloarthritis (axSpA) due to a possible overlap between the tender points (TP) due to enthesitis and those of FM. The objective was to assess the agreement between the MASES enthesitis score and the tender points of the ACR 1990 criteria in patients with axSpA. METHODS: This was a cross-sectional ancillary analysis of the Predict-SpA study (NCT03039088). Patients had a diagnosis of axSpA according to their rheumatologist and an indication to start a TNFα blocker. All patients were s

Smoking and alcohol consumption are associated with peripheral musculoskeletal involvement in patients with spondyloarthritis (including psoriatic arthritis). Results from the ASAS-PerSpA study.

BACKGROUND: An inverse association between alcohol consumption and disease activity and functional impairment has been observed in patients with spondyloarthritis (SpA). However, neither this association nor the influence of smoking has been investigated in peripheral manifestations of SpA. OBJECTIVES: The objective of this study was to analyze the association between smoking and alcohol consumption and the presence of peripheral musculoskeletal manifestations (arthritis, enthesitis or dactylitis) and to determine the specific location of these manifestations. METHODS: Patients from the worl

Adult rheumatologic features, treatment and complications of X-linked hypophosphatemia.

X-linked hypophosphatemia (XLH) is a rare genetic phosphate disorder caused mainly by PHEX mutations. Unlike for children, knowledge of the disease's manifestations in adults is limited. Musculoskeletal symptoms are the main feature of the disease in young adults associated with a heavy burden on patients' life. They include fractures and pseudofractures, pain, joint stiffness, osteoarthritis, enthesopathies, and muscle weakness, eventually leading to impaired quality of life. Conventional treatment with phosphate supplements and vitamin D analogs is indicated in symptomatic patients. Appropri

Hydroxyapatite deposition disease (HADD) of the spring ligament: an unusual cause of medial foot pain.

Foot pain is one of the most common presenting complaints in orthopaedic clinical practice and can be attributed to a multitude of pathologies in the various osseous structures, ligaments, and tendons of the foot. The spring ligament complex (SLC) between the calcaneum and navicular supports the talus and plays a major role in the static stability of the medial longitudinal arch of the foot. Although calcific ligamentous enthesopathy around the ankle has been described in the literature, we report the first case of its kind affecting the SLC in a 51-year-old male with medial foot pain and no h…

Tissue changes detectable by sonography before radiological evidence of elbow chondrocalcinosis.

Chondrocalcinosis is a joint disease caused by deposits of calcium pyrophosphate dihydrate (CPPD) crystals with manifestations that may include intermittent attacks of acute arthritis or enthesitis. If no crystals are found on synovial analysis, the diagnosis in clinical practice is based on typical radiographic findings, although it is now recognized that some signs of the disease could also be seen by ultrasound. We studied six patients with elbow enthesopathy without radiographic evidence of CPPD that presented ultrasound findings suggesting joint calcification. On the basis of these echogr…

The spectrum of skeletal changes associated with long-term administration of 13-cis-retinoic acid.

The roentgenographic changes noted in 13 patients, who had been treated with long-term 13-cis-retinoic acid for inherited scaling disorders, are presented. These patients were aged 13-16 years and had received this therapy for 16-87 months (mean, 58 months). The most pronounced abnormality was osteophyte formation, particularly in the cervical spine. Other changes which were noted included ossification of the anterior longitudinal and atlanto-occipital ligaments, proliferative enthesopathies, diminished bone density, premature fusion of epiphyses, and modeling abnormalities. Six of the 13 pati

Enthesitis in Spondyloarthritis. From pathogenesis to clinical presentation and imaging: Similarities and differences between PsA and axSpA.

Radiographic axial spondyloarthritis (r-axSpA), non-radiographic axial spondyloarthritis (nr-axSpA), and psoriatic arthritis (PsA) represent distinct yet overlapping entities within the spondyloarthritis (SpA) spectrum. Enthesitis has emerged as a unifying hallmark of these diseases, linking mechanical stress, immune dysregulation, and structural remodeling. Understanding similarities and differences in entheseal pathology among these entities may elucidate shared and divergent mechanisms driving disease pathogenesis and progression. Mechanical stress at entheses activates mechanosensitive pat

Enthesitis in Spondyloarthritis Including Psoriatic Arthritis-To Inject or Not To Inject?: A Narrative Review.

PURPOSE: Enthesitis is a key manifestation of psoriatic arthritis (PsA) and spondyloarthritis (SpA) and is considered to be the tissue where the disease initiates. Enthesitis leads to pain and substantial limitations in patients with PsA. Treatment is key in achieving remission or minimal disease activity. Whether it is safe to apply injections to entheseal tissue is unknown. This narrative review aimed to summarize the literature on the efficacy and tolerability of entheseal corticosteroid (CS) injections. METHODS: The published literature was searched through PubMed as well as identifying r
