The diagnosis, management and outcome in 12 adults with acute epiglottitis was reviewed. Painful dysphagia was a universal symptom and respiratory distress affected eight patients, six of whom required urgent airway intervention. All patients received parenteral antibiotics, ten received steroids and four received adrenaline. Respiratory distress resolved in two patients given adrenaline and airway intervention was avoided. Indirect laryngoscopy is the investigation of choice and this is preferable to neck radiology. Two patients died and it is stressed that this condition must be distinguishe…

With the introduction of an anti-haemophilus type B vaccine, a standard incidence rate for acute epiglottitis is needed to judge its efficacy. Epiglottitis is thought to be more common in children than in adults, but this relation has never been clearly established. This article attempts to evaluate the incidence of this disease. A review of the literature regarding the incidence is correlated to new data from the 1981 Census of Canada and the Med-Echo System of the province of Quebec. It suggests that the incidence rate is approximately 60/million for children (1/17,000), whereas it is 10/mil…

The incidence and mortality of acute epiglottitis were retrospectively analysed covering a 12-year period, 1975-1987. The diagnosis was made by the laryngoscopic findings in 95% of patients. Based on 902 hospitalized patients, a mean annual incidence of 4.9 cases per 100,000 children and adults was found. Although adult patients predominated (60%), the incidence was far higher in children, 13.8 per 100,000 compared with 3.9 per 100,000 in adults. Two peaks in incidence were identified, one for the youngest children, and the second for young adults (15-29 years). Over the years the incidence va…

Adult patients with acute epiglottitis hospitalized between 1975 and 1988 were retrospectively analysed. 79 of 138 patients had a valid diagnosis and had delivered at least one blood culture and were considered eligible for further evaluation. Cultures from the upper airways, including epiglottis, were available in 43 of the patients as a consequence of prospective measures. 27% of the patients had bacteremia, which may however be an overestimation. Haemophilus influenzae was the predominating finding, but Streptococcus pneumoniae was isolated from 3 severely ill patients, indicating the exist…

Acute epiglottitis is the most treacherous inflammatory process that can affect the respiratory tract. It can be a cause of sudden death in children. The clinical features of this entity are reviewed and our experience with its management at The Montreal Children's Hospital during the past 25 years is presented. The establishment of a "secure" airway by tracheostomy and in recent years, by nasotracheal intubation has decreased to practically nil the mortality of children who arrive alive at this hospital with acute epiglottitis.

Laryngeal tuberculosis (TB), accounting for less than 1% of extrapulmonary TB cases, poses significant diagnostic challenges due to its mimicry of malignant lesions and high transmissibility. This case describes a 55-year-old male from a TB-endemic region in Colombia presenting with an 8-month history of progressive weight loss, odynophagia, dysphagia, and dysphonia, in whom nasolaryngoscopy revealed a tumor-like lesion occupying the epiglottis. Microbiological testing confirmed drug-sensitive Mycobacterium tuberculosis, and following a four-drug antituberculosis regimen, complete resolution o

BACKGROUND: Acute epiglottitis is a rapidly progressive infection of the supraglottic structures that can potentially be life-threatening. Although its incidence has decreased in pediatric populations owing to widespread vaccination, it remains a diagnostic and therapeutic challenge in adults, where clinical presentations may be subtle, and airway compromise can occur with little warning. The case of a 64-year-old white European female patient is presented.

INTRODUCTION AND IMPORTANCE: Capillary hemangioma of larynx can occur in both pediatric and adult population. However, epiglottic capillary hemangioma in adults is a very rare presentation. Its pathophysiological basis involves self limiting and self expanding vascularization and it is a characteristically well delineated benign mass.
CASE PRESENTATION: Here we present a case of a 40-year-old female who presented with complaint of per oral bleed in spitting which was about a teaspoon in quantity since 1 month. There was no associated hoarseness, dysphagia, respiratory distress, hemoptysis or

OBJECTIVES: Understanding of the evolution of the larynx clarifies the physiology and functional histoanatomy of the present-day human organ. Comparative histoanatomy of the epiglottis and pre-epiglottic space of the chimpanzee larynx (a hominid, phyletic closest relative of humans) was investigated.
METHODS: Two normal adult chimpanzee larynges obtained from natural deaths were investigated. The whole organ serial section technique was employed.
RESULTS: The histoanatomical structures of the chimpanzees' epiglottis and pre-epiglottic space were considerably similar to those of human adults.

Diagnosis of the Human Immunodeficiency Virus (HIV) remains challenging due to non-specific clinical presentations and mostly flu-like sympt … atment and the laryngeal symptoms improved within weeks. In patients with idiopathic and persistent epiglottitis or an epiglottic ulcer after medical treatment, HIV infection needs to be considered as a potential etiology in order to institute proper treatment.

Epiglottitis is inflammation of the epiglottis with or without the involvement of supraglottic structures including the hypopharynx. Timely diagnosis is crucial as the treatment of epiglottitis is tailored to the degree of airway obstruction. Most patients improve with conservative measures, while some require an emergent airway intervention. We report a case of a 39-year-old Caucasian male with a history of uncontrolled diabetes mellitus and smoking who presented to the emergency department (ED) with a sore throat, dry cough, odynophagia, and difficulty swallowing. He was afebrile, tachycardi…

A 78-year-old female patient presented to our emergency department with a 5-day history of throat pain, hoarseness and a progressive and dolorous submandibular swelling. Due to non-conclusive clinical examination and the stable but visibly affected patient, we performed a neck CT scan with intravenous contrast, which showed the thumbprint sign typical for acute epiglottitis. Within minutes, the patient's condition deteriorated and the patient was close to respiratory exhaustion. As intubation was severely complicated by massive swelling of the supraglottic soft tissue, the patient went into hy…

Epiglottitis is a rare and life-threatening cause of acute sore throat. Usually, epiglottitis is caused by a bacterium, such as Haemophilus influenzae type b. Symptoms of epiglottitis are acute and rapidly progressive sore throat, a hoarse voice, fever, and drooling. We present two adult patients with acute sore throat who were diagnosed with epiglottitis. In adults with symptoms compatible with epiglottitis, it is justified to look down the throat with a light. Only if the view is impeded should a spatula be used but carefully. If there is a discrepancy between the severity of symptoms and fe…

BACKGROUND: Thermal epiglottitis is a rare but potentially life-threatening disease. Diagnosis requires a thorough history and high clinical level of suspicion, particularly in children. Thermal epiglottitis from steam inhalation can have a slow onset without oropharyngeal signs of thermal injury, findings that can hide the clinical diagnosis.
OBJECTIVE: Our aim was to review the pathophysiology and clinical presentation of thermal epiglottitis and the challenges involved in diagnosis and management of this form of atypical epiglottitis.
CASE REPORT: We describe the case of a 22-month-old ma…

Acute epiglottitis is a true airway emergency in the emergency department (ED). The patient may appear very toxic and rapidly progress to respiratory distress and life-threatening condition. The inflammatory process includes not only epiglottis but also the rest of the supraglottic area including the vallecula, aryepiglottic folds, and arytenoids. Soft tissue swelling over this windpipe area can be very dramatic. The criterion standard of diagnosis is direct inspection of cherry red and swollen epiglottis by laryngoscopy in the operation room with immediate access to anesthetists or ear, nose,…
