Displaced aggression predicts switching deficits in people with temporal lobe epilepsy.

This study examined the relationship between task-switching abilities and displaced aggression in people with temporal lobe epilepsy (PWE). Participants (35 PWE and 35 healthy controls) performed emotion and gender classification switching tasks. People with temporal lobe epilepsy showed larger switch costs than controls. This result reflected task-switching deficits in PWE. People with temporal lobe epilepsy reported higher anger rumination, revenge planning, and behavioral displaced aggression compared with controls. Displaced aggression was a significant predictor of the task switch costs.

Periodic epileptiform discharges in mesial temporal lobe epilepsy with hippocampal sclerosis.

PURPOSE: Periodic epileptiform discharges (PEDs) are an uncommon, abnormal EEG pattern seen usually in patients with acute diseases and less frequently in chronic conditions, such as mesial temporal lobe epilepsy (mTLE). Evaluate the clinical histories, neuroimaging findings, and serial electrophysiological studies prior to the appearance of PEDs in patients with mTLE secondary to hippocampal sclerosis (HS). METHODS: We searched 19, 375 EEGs (2006-2012) for the presence of PEDs secondary to mTLE due to HS. RESULTS: 12 patients were included. The patients with PEDs had a high prevalence of ps

Ictal kissing: a release phenomenon in non-dominant temporal lobe epilepsy.

BACKGROUND: A variety of ictal automatisms with strong emotional elements have been described. Ictal kissing has not been well characterized and may provide useful clinical information and insight into the mechanisms of stereotyped ictal behaviour. METHODS: Three of 220 patients (1.4%) admitted for video EEG monitoring between 7/2006 and 6/2009 with ictal kissing were identified. Clinical, neurophysiological and imaging data were reviewed and correlated. RESULTS: All patients were right-handed women with a longstanding history and findings consistent with right temporal lobe epilepsy (TLE).

Nonmanipulative proximal upper extremity automatisms lateralize contralaterally in temporal lobe epilepsy.

PURPOSE: Upper extremity automatisms are considered to be an ipsilateral seizure lateralizing sign in temporal lobe epilepsy (TLE). Herein we describe different types of contralateral upper extremity automatisms (CUEAs). METHODS: One hundred ninety-three video-(electroencephalography) EEG recordings of 59 patients were reviewed. Other than two patients who refused surgery, all patients underwent standardized temporal lobectomy with favorable postoperative outcome. Fifty-seven seizures of 21 patients were selected with CUEAs. We evaluated their electroclinical characteristics and their relatio

Heart rate dynamics in refractory and well controlled temporal lobe epilepsy.

OBJECTIVES: Disorders of cardiovascular and other autonomic nervous system functions are often found in patients with temporal lobe epilepsy (TLE). Cardiovascular dysregulation in TLE has previously been quantified assessing traditional time and frequency domain measures of heart rate (HR) variability from short term ECG recordings. However, new complexity and fractal measures of HR variability based on non-linear dynamics and fractals ("chaos theory") may disclose certain patterns of HR dynamics that cannot be detected using only conventional measures. METHODS: In addition to the traditional

Memory lateralization in medial temporal lobe epilepsy assessed by functional MRI.

PURPOSE: To determine the utility of functional magnetic resonance imaging (fMRI) in preoperative lateralization of memory function in patients with medial temporal lobe epilepsy (MTLE). METHODS: Nine patients with MTLE underwent standard preoperative assessment including video-EEG and intracarotid amytal testing (IAT). fMRI was performed while subjects encoded four types of stimuli (patterns, faces, scenes, and words). Activation maps were created for each subject representing areas more active for novel than for repeated stimuli. Regions of interest were drawn around the MTL in individual s

Temporopolar changes in temporal lobe epilepsy: a quantitative MRI-based study.

OBJECTIVES: To quantify the morphologic changes of temporopolar structures to better understand the pathophysiology of anterior temporal white matter increased T2 signal observed in temporal lobe epilepsy (TLE). METHODS: MRI was performed in 30 patients with TLE and in 30 normal control subjects and independently assessed by visual analysis and quantitative measurements. Specifically, the temporal pole (TP) volume, as well as its gray and white matter components, was measured using three-dimensional T1 MR images and a semiautomatic protocol. The authors tested whether the presence of an incre

Predictive value of magnetic resonance imaging in temporal lobe epilepsy surgery.

The predictive value of magnetic resonance imaging (MRI) was assessed by a prospective study of 34 patients selected for surgical treatment of temporal lobe epilepsy. The MRIs were interpreted using standardized visual diagnostic criteria and the imaging findings were correlated with the surgical outcome. Lateralized MRI abnormalities were found in 25 (74%) of 34 patients. Significant associations were found between either the presence of a restricted foreign-tissue lesion or hippocampal atrophy and an excellent surgical outcome. An abnormal MRI had an 82% predictive value and a normal MRI had

Antecedents and Potential Risk Factors for Temporal Lobe Epilepsy: Data From a Canadian Epilepsy Clinic.

BACKGROUND: Despite extensive research on temporal lobe epilepsy (TLE), most work focuses on treatment outcomes and their determinants. Detailed evaluation of causal factors has not been adequately addressed in most published studies. This study aimed to explore potential etiologic risk factors for TLE through a chart review of prospectively collected data from a Canadian epilepsy clinic. METHODS: Consecutive patients diagnosed with TLE at a Canadian epilepsy clinic were included. Consecutive patients with genetic generalized epilepsy (GGE) served as a comparison group. Transmembrane protein modulates seizure in epilepsy: evidence from temporal lobe epilepsy patients and mouse models.

Transmembrane protein (TMEM230) is located in secretory/recycling vesicles, including synaptic vesicles in neurons. However, the functional relationship between TMEM230 and epilepsy is still a mystery. The aims of this study were to investigate the expression of TMEM230 in patients with temporal lobe epilepsy (TLE) and two different mice models of chronic epilepsy, and to determine the probable roles of TMEM230 in epilepsy. Our results showed that TMEM230 expression was increased in the temporal neocortex of epileptic patients and the hippocampus and cortex of epileptic mice compared with that

Neuroanatomical correlates of personality traits in temporal lobe epilepsy: Findings from the Epilepsy Connectome Project.

Behavioral and personality disorders in temporal lobe epilepsy (TLE) have been a topic of interest and controversy for decades, with less attention paid to alterations in normal personality structure and traits. In this investigation, core personality traits (the Big 5) and their neurobiological correlates in TLE were explored using the Neuroticism Extraversion Openness-Five Factor Inventory (NEO-FFI) and structural magnetic resonance imaging (MRI) through the Epilepsy Connectome Project (ECP). NEO-FFI scores from 67 individuals with TLE (34.6 ± 9.5 years; 67% women) were compared to 31 health

Mesial temporal involvement in epidermoid tumor-related temporal lobe epilepsy revealed by stereoelectroencephalography: illustrative case.

BACKGROUND: Intracranial epidermoid tumors are rare benign lesions that are typically nonepileptogenic; however, temporal lobe involvement may result in drug-resistant epilepsy. The relationship between the lesion and the epileptogenic network remains poorly understood, particularly due to limited electrophysiological evaluation of mesial temporal structures. OBSERVATIONS: A 52-year-old left-handed woman with drug-resistant temporal lobe epilepsy associated with an epidermoid tumor underwent staged surgical management. Initial resection of the posterior fossa component achieved histopathologi

Functional Anterior Temporal Lobectomy for Temporal Lobe Epilepsy: A Novel, Alternative, and Less Invasive Therapy.

BACKGROUND: Anterior temporal lobectomy is the most effective treatment for intractable temporal lobe epilepsy (TLE). However, patients are reluctant to choose this surgery for fear of risks after large frontotemporal craniotomy, and epileptologists likewise have a cautious attitude because of surgical trauma. Functional anterior temporal lobectomy (FATL) is a minimally invasive surgery procedure for addressing the above concerns. OBJECTIVE: To report preliminary data on this procedure and its safety and efficacy for treating TLE. METHODS: This consecutive case series study was conducted bet

Recovery of cortical atrophy in patients with temporal lobe epilepsy after successful anterior temporal lobectomy.

The aims of this study were to investigate whether the cortical atrophy caused by temporal lobe epilepsy (TLE) was reversible after successful anterior temporal lobectomy (ATL) and to further observe whether possible changes are related to age at surgery and cognitive changes. Twelve patients with unilateral mesial TLE who received ATL and remained seizure free in one year follow-up were included. They underwent two MRI scans few days before and oneyear after surgery. Thirty age- and sex-matched healthy participants were recruited as controls. Group comparisons were used to test the difference
