Epiphysiodesis and subtrochanteric osteotomy have long been recognized as valuable procedures in the treatment of the patient with a moderately or markedly displaced femoral capital epiphysis. Typically these have been performed individually or sequentially. The operation herein described combines the two in one single-stage procedure. Using a stainless steel Lorenzo Screw, the epiphyseal plate is transfixed and further capital displacement prevented. The subtrochanteric compensatory osteotomy is performed using simple visual clinical guidelines and is held rigidly immobilized with a variable

UNLABELLED: Nine transfusion-dependent beta-thalassaemia major patients (seven males and two females), aged 4-15 years, with growth retardation and severe rickets-like radiological lesions due to continuous subcutaneous chelation therapy with desferrioxamine (45-75 mg/kg body weight, 6-7 time/week), were seen in our centre during the last 8 years. Serum ferritin levels ranged from 976 to 4115 micrograms/l. There was a progressive decline in growth velocity in these patients 2-3 years before the appearance of rickets-like radiological lesions. All patients underwent surgery to correct genu valg

The authors have seen 41 instances of chondrolysis following slipped capital femoral epiphyses. Most of the cases were followed-up for more than five years. The main clinical features are reviewed. There is joint stiffness and generalised narrowing of the joint space during the year following the initial lesion. The evolution of the lesion is unpredictable. A number of possible radiological changes may develop including renewal of the joint space, modifications in the femoral head with a tendency towards protrusio, and osteophyte formation at the lateral margins of the acetabulum. In five case

UNLABELLED: Core biopsies of the proximal femoral growth plate from three patients with slipped capital femoral epiphysis were compared with three normal growth plates from patients of similar ages. The growth plates of patients with slipped capital femoral epiphysis had a normal resting zone, a loose and fibrillated proliferative zone, and a very disarranged and thickened hypertrophic zone. The proliferative zone stained strongly with alcian blue, suggesting the presence of abundant proteoglycans in the cartilage matrix. The thickened hypertrophic zone was strongly positive to periodic acid-S

Slipped capital femoral epiphysis (SCFE) occurs in preadolescent and adolescent children. The exact pathomechanism of the slip is not known. Several risk factors have been identified including hormonal derangement. Of the classification systems, stability and severity classification systems are most relevant to everyday practice. There is a strong agreement regarding the management of a mild slip. Controversies exist in the treatment of moderate and severe slips and also regarding the prophylactic pinning of the opposite side. Discrepancies exist amongst countries and surgeons as to the number

CASE: A 22-year-old man with worsening hip pain was found to have chronic complete slippage of a previously pinned slipped capital femoral epiphysis (SCFE) and subsequent pseudoarthrosis between the femur and ilium. The deformity was successfully treated with total hip replacement. We report 13 months of postoperative follow-up with adequate restoration of leg length and symptomatic relief.
CONCLUSION: This rare case demonstrates the importance of careful preoperative planning to ensure appropriate alignment of rotation of the hip and restoration of leg length in the setting of a chronic SCFE

BACKGROUND: In Sweden, most children with slipped capital femoral epiphysis (SCFE) are operated on with a single smooth pin or a short-threaded screw, allowing further growth of the femoral neck. Using the Swedish Pediatric Orthopaedic Quality registry, SPOQ, we investigated whether angular remodelling occurs adjacent to the proximal femoral epiphysis after fixation of SCFE using implants, allowing continued growth of the femoral neck.
METHODS: During 2008-2010 a total national population of 155 children were reported to the SPOQ registry. Following our strict inclusion criteria, radiographs

BACKGROUND: In slipped capital femoral epiphysis (SCFE), the femoral head slides in an anterosuperior direction along the growth plate. The femoral head remains in the acetabulum. The pathogenesis of SCFE is multifactorial. An important predisposing factor is obesity.
PROBLEM: Epiphysiolysis can compromise the blood supply to the epiphysis and osteonecrosis of the femoral head may result.
DIAGNOSTICS AND PROGNOSIS: Conventional radiography represents the first diagnostic step. The long-term prognosis of the disease depends on the residual deformity of the femoral head and in the worst case m

OBJECTIVE: The purpose of this study was to compare outcomes between orthopaedic trainees using various preoperative training platforms (physical simulation [PS], virtual reality [VR], and reading/videos) in a slipped capital femoral epiphysis model.
METHODS: Participants were randomly assigned to one of the three groups: (1) reading/video control group (n = 7), (2) VR group (n = 7), or (3) PS group (n = 7). Participants in the VR group completed a VR slipped capital femoral epiphysis module while participants in the PS group practiced the placement of a screw in the physical module before ev

BACKGROUND: Avascular necrosis (AVN) is a well-known complication of unstable slipped capital femoral epiphysis (SCFE) and its cause is multifactorial. Higher AVN rates have been reported with surgery undertaken between 24 hours to 7 days from the onset of symptoms. The current evidence regarding time to surgery and AVN rate remains unclear. The aim of our study was to investigate the rate of AVN and time to surgery in unstable SCFE.
METHODS: A literature search of several databases was conducted. Eligibility criteria included all studies that reported AVN rates and time to surgery in unstabl

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is a hip disorder frequently occurring in adolescence. In adults it is rare and so far very few cases have been documented.
CASE PRESENTATION: This report presents a 25-year-old patient diagnosed with an anterior fossa giant chondroma, hypogonadotropic hypogonadism, and SCFE. The patient underwent surgical and hormonal therapy. His symptoms revealed, and he became a father.
CONCLUSIONS: Every patient diagnosed with SCFE in adulthood should undergo endocrinological assessment based on physical examination and laboratory tests.
