Acid-base balance at high altitude in lowlanders and indigenous highlanders.

High-altitude exposure results in a hyperventilatory-induced respiratory alkalosis followed by renal compensation (bicarbonaturia) to return arterial blood pH (pHa) toward sea-level values. However, acid-base balance has not been comprehensively examined in both lowlanders and indigenous populations-where the latter are thought to be fully adapted to high altitude. The purpose of this investigation was to compare acid-base balance between acclimatizing lowlanders and Andean and Sherpa highlanders at various altitudes (∼3,800, ∼4,300, and ∼5,000 m). We compiled data collected across five indepe

[Andes virus aboard the MV Hondius: Transnational public health challenges in ocean-going cruise travel].

This article analyzes the cluster of Andes virus infections that occurred aboard the expedition vessel MV Hondius during a transatlantic crossing. Drawing on publicly available information released by the World Health Organization, the European Centre for Disease Prevention and Control, and the US Centers for Disease Control and Prevention, it examines the key characteristics of this event: probable exposure in a natural reservoir area in South America before embarkation, limited human-to-human transmission in a closed maritime environment, difficulties in early case recognition, and the compl

[Chronic alcoholic enteritis in the North].

Altogether 107 patients with chronic enterocolitis were examined. They were divided into 3 groups with relation to their attitude to alcohol: the 1st group-alcoholics, the 2nd risk group-hard drinking, the 3rd group-non-alcoholics. Severe disorders of the intestinal enzymatic activity (suppression of the intraduodenal amylolytic and proteolytic and intraluminal lactase activity) were revealed, the character of parietal digestion was changed. Atrophic duodenal changes among these patients were noted twice as more frequently as among the patients who did not consume alcohol or consumed it modera

Relation of timing of cardiac catheterization to outcomes in patients with non-ST-segment elevation myocardial infarction or unstable angina pectoris enrolled in the multinational global registry of acute coronary events.

We assessed whether timing of catheterization is associated with the type of non-ST-segment elevation acute coronary syndrome and/or outcome in patients who were enrolled in the Global Registry of Acute Coronary Events. Overall, 8,853 patients who had unstable angina pectoris or non-ST-elevation myocardial infarction were categorized according to timing of catheterization: expeditive (<24 hours), early (24 to 48 hours), and delayed (>48 hours). Patients in the delayed group were older, more frequently had previous myocardial infarction or stroke, and had a higher risk score compared with those

Preserved circadian variation in cortisol and androgens during a ski traverse of Antarctica in summer.

Antarctic expeditions present extreme physiological challenges due to cold temperatures, high physical exertion, and 24-hour daylight. This observational study evaluated endocrine adaptation in nine participants (six men, three women) during a 47-day, 1,000 km unassisted ski traverse. Detailed salivary sampling was conducted before, during and after the expedition, corroborated by blood and hair sampling before and after the expedition. Cortisol, testosterone, and androstenedione were measured using mass spectrometry, and thyroid hormones via immunoassay. Diurnal cortisol, androstenedione and

Everest Physiology Pre-2008.

When Edmund Hillary and Tenzing Norgay reached the summit of Mt. Everest in 1953, it was the culmination of many attempts beginning in 1921. Alexander Kellas had actually predicted as early as 1920 that the mountain could be climbed, but the extreme altitude of 8848 m with the consequent oxygen deprivation had foiled previous attempts. One reason for the success of the 1953 expedition was the work done by the British physiologist Griffith Pugh in 1952 when he studied many of the physiological factors at high altitude including the oxygen requirements. Seven years later, Pugh and Hillary teamed

[C.P. Thunberg and his voyage to Japan (4). Mercury water therapy for syphilis introduced into Japan by Thunberg].

During his 16-month stay in Japan from August 1775 to December 1776, Thunberg taught mercury water therapy to Japanese medical doctors and interpreters in order to help them treat syphilis, which was prevalent in Japan at that time. Kohgyu Yoshio, a Japanese-Dutch interpreter who was taught this therapy by Thunberg, recorded Thunberg's teaching in his "Kohmoh Hijiki." According to this book, the mercury water therapy that Thunberg introduced to Japan had been tested successfully by van Swieten, a Dutch doctor, about 20 years previously in Europe and used corrosive sublimate (mercuric chloride)

Pre- to postexpedition changes in the energy usage of women undertaking sustained expeditionary polar travel.

This paper reports the metabolic energy changes in six women who made the first unsupported traverse of Antarctica, covering a distance of 1,700 km in 61 days, hauling sledges weighing up to 80 kg. Pre- and postexpedition, measurements of energy expenditure and substrate utilization were made on all six members of the expedition over a 36-h period in a whole body calorimeter. During the study, subjects were fed an isocaloric diet: 50% carbohydrate, 35% fat, and 15% protein. The experimental protocol contained pre- and postexpedition measurement, including periods of sleep, rest, and three peri

It's Time to Expand Chlamydia Treatment for Gay and Bisexual Men.

Expedited partner therapy involves prescribing sexually transmitted infection (STI) treatment for a patient's partner(s) without seeing the partner. It is approved for heterosexual partners of patients with chlamydia in most states. However, the Centers for Disease Control and Prevention recommends against expedited partner therapy in men-who-have-sex-with-men (MSM), citing limited data in this population and concerns that expedited partner therapy could discourage comprehensive STI testing, thereby driving increased HIV transmission. In this piece, we describe the case of a 33-year-old gay ma

When Pasteurian Science Went to Sea: The Birth of Marine Microbiology.

In the late nineteenth century, French naturalists were global leaders in microbial research. Louis Pasteur advanced sterilization techniques and demonstrated that dust particles in the air could contaminate a putrefiable liquid. Pasteur's discoveries prompted a new research program for the naturalists of the Talisman and Travailleur expeditions: to recover uncontaminated water and mud samples from the deep sea. French naturalists Adrien Certes and Paul Regnard both independently conducted experiments to address the question of whether microorganisms inhabited the oceans and whether organic ma

Alexandre Yersin's explorations (1892-1894) in French Indochina before the discovery of the plague bacillus.

Alexandre Yersin, the great French discoverer of yersinia pestis, was a keen explorer of unknown lands. At the age of 30, a member of the French Colonial Health service, he set off to fulfil his intimate dream and explore other continents. For almost two years and three long expeditions, he journeyed through widely unknown regions in the province of the French Indochina, in southeast Asia, territories of Vietnam, Cambodia and Laos. This article presents vignettes from his explorations. During his difficult travels, he carefully planned and noted his itineraries; designed new routes, but also o

Early Morbidity but Not Mortality Increases With Surgery Delayed Greater Than 24 Hours in Patients With a Periprosthetic Fracture of the Hip.

BACKGROUND: Studies have identified a possible morbidity and mortality benefit with expedited time to surgery after a native hip fracture. This association after hip periprosthetic fractures (PPF) has been less clearly delineated. The purpose of this study is to assess the effect of time to surgery on rates of 30-day complications. METHODS: The National Surgical Quality Improvement Program registry was used to identify all patients who underwent surgical intervention for hip PPF between 2005 and 2016. Patients were stratified into 2 cohorts based on time from hospital admission to surgery, ei

Sleep apneas and high altitude newcomers.

Sleep and respiration data from two French medical high altitude expeditions (Annapurna 4,800 m and Mt Sajama 6,542 m) are presented. Difficulties in maintaining sleep and a SWS decrease were found with periodic breathing (PB) during both non-REM and REM sleep. Extent of PB varied considerably among subjects and was not correlated to the number of arousals but to the intercurrent wakefulness duration. There was a positive correlation between the time spent in PB and the individual hypoxic ventilatory drive. The relation between PB, nocturnal desaturation, and mountain sickness intensity are di

Edward Leicester Atkinson (1881-1929): Antarctic explorer, scientist and naval surgeon.

Edward Leicester Atkinson qualified at St Thomas's Hospital in 1906 and joined the Navy in 1908. He was a doctor and parasitologist on Captain Scott's Terra Nova expedition to the Antarctic and had to take charge of the expedition when Scott died on his return from the South Pole. After the expedition he went to China and discovered the cause of schistosomiasis, returning at the start of the First World War in which he served with distinction, winning a DSO and Albert Medal but also being severely injured. After the war he served in various naval posts and became the youngest Surgeon Captain i
