Selective Reduction Using Intravascular Potassium Chloride Injection after Laser Surgery for Twin-Twin Transfusion Syndrome.

Selective reduction (SR) via intravascular potassium chloride (KCl) injection is contraindicated in monochorionic twins due to the presence of placental vascular communications, which may serve as a conduit for inter-twin passage of KCl or allow exsanguination of the living twin into the demised twin. After successful selective laser photocoagulation of communicating vessels (SLPCV) for twin-twin transfusion syndrome (TTTS), the twins' circulatory systems are rendered independent. Theoretically, intravascular KCl injection into one twin after successful SLPCV should not result in passage of th…

Evaluation of vascular delivery methodologies to enhance rAAV6-mediated gene transfer to canine striated musculature.

A growing body of research supports the development of recombinant adeno-associated viral (rAAV) vectors for delivery of gene expression cas … sculature of canines. A simple procedure incorporating acute (i) occlusion of limb blood flow, (ii) exsanguination via compression bandage, and (iii) vector "dwell" time of <20 minutes, markedly enhanced the transduction of limb muscles, compared with a simple bolus limb infusion of vector. A complementary method whereby vector was infused into the jugular vein …

A decade's experience with temporary intravascular shunts at a civilian level I trauma center.

BACKGROUND: A 10-year review of temporary intravascular shunts (TIVS) at a regional trauma center. METHODS: Retrospective chart review of a … e of 73%. TIVS have an established role primarily in patients requiring either "damage control" for exsanguination or temporary vascular conduits during stabilization of Gustilo IIIc fractures. Truncal injuries are associated with the highest mortality likely due to accompanying multisystem trauma.

Iatrogenic vitamin K deficiency and life threatening coagulopathy.

A man was admitted with abdominal pain. Treatment for acute diverticulitis was instituted with intravenous antibiotics and oral limitation. Imaging demonstrated a complex inflammatory mass. Prothrombin time (PT), activated partial thromboplastin time (APTT) and fibrinogen were within normal limits. However, repeat preoperative clotting studies demonstrated a severe unexpected coagulopathy to have developed since admission that could have caused fatal intraoperative exsanguination. Direct assays showed severe, isolated deficiency of vitamin K dependent clotting factors, and mixing studies norma…

Primary pheochromocytoma extending into the right atrium: report of a case and review of the literature.

Pheochromocytoma rarely extends locally into the vena cava or the right atrium. We report a case of malignant pheochromocytoma with growth i … o-laparotomy. If feasible, a combination of cardiopulmonary bypass, hypothermia, cardiac arrest and exsanguination procedures should be used. In case of local of tumour remnants after surgery or distant metastases treatment options are secondary surgery, tumour embolization, or treatment with radioactive labelled drugs, including(131)I-MIBG.

Sutureless liver repair and hemorrhage control using laser-mediated fusion of human albumin as a solder.

BACKGROUND: Major liver trauma has a high mortality because of immediate exsanguination and a delayed morbidity from septicemia, peritonitis, biliary fistulae, and delayed secondary hemorrhage. We evaluated laser soldering using liquid albumin for welding liver injuries. METHODS: Fourteen lacerations (6 x 2 cm) and 13 nonanatomic resection injuries (raw surface, 8 x 2 cm) were repaired. An 805-nm laser was used to weld 53% liquid albumin-indocyanine green solder to the liver surface, reinforcing it by welding a free autologous omental scaffold. The animals were heparinized and hepatic inflow …

Recognition and management of catheter-induced pulmonary artery rupture.

BACKGROUND: Catheter-induced pulmonary artery rupture is a well-recognized complication of invasive monitoring, but the risk has not diminished. Although commonly associated with cardiopulmonary bypass, injuries also occur in intensive care. Definitive proof requires pulmonary angiography or autopsy. Many cases are never reported, and lesser injuries are probably under-diagnosed. METHODS: Seven cases fulfilling accepted diagnostic criteria discovered over 2 years are described in four groups illustrating the common modes of presentation: hemoptysis with hypoxemia, exsanguination, delayed recu…

The use of VEPs for CNS monitoring during continuous cardiopulmonary bypass and circulatory arrest.

Cerebral function was monitored with the use of visual evoked potentials (VEPs) in 16 infants (mean age 9.9 +/- 4.3 months) during surgery for congenital cardiac anomalies. While hypothermia was employed in all patients, half (8) remained on continuous cardiopulmonary bypass (CCB) while the rest were cooled to lower temperatures before the induction of circulatory stasis and venous exsanguination (CA), i.e., profound hypothermic circulatory arrest (PHCA). VEPs were recorded before, during and after surgical intervention. Latency changes occurred in both the N100 and P145 components of the VEP …

Brazilian guidelines on diagnosis and management of traumatic vascular injuries.

Trauma is a leading cause of death, permanent disability, and health care cost worldwide. The young and economically active are the most affected population. Exsanguination due to noncompressible torso hemorrhage is one of the most frequent causes of early death, posing a significant challenge to trauma and vascular surgeons. The possibility of limb loss due to vascular injuries must also be considered. In recent decades, the approach to vascular injuries has been significantly modified. Angiotomography has become the standard method for diagnosis, endovascular techniques are currently incorpo…

The advent of artificial ventilation was largely sparked by the popular belief that drowning and other causes of asphyxia could induce death … ial ventilation for the treatment of asphyxia over alternative resuscitation measures like heat and exsanguination. In addition, Goodwyn's remarkable dissertation contains the first account of a reflex known as diving bradycardia, and possibly the first vehement refutation of claims by his contemporaries that pulmonary circulation stopped during exhalation. Specific complications of monochorionic twin pregnancies: twin-twin transfusion syndrome and twin reversed arterial perfusion sequence.

Monochorionic twins are subjected to specific complications which originate in either imbalance or abnormality of the single placenta servin … tion in one twin hazardous since the spontaneous death of one twin exposes the co-twin to a risk of exsanguination into the dead twin and its placenta. The latter is responsible for the death of the co-twin in up to 20% of the cases and in ischemic sequelae in about the same proportions in the survivors. Although the symptoms of all these complications are very …

Two unusual stab injuries to the neck: homicide or self-infliction?

A 31-year-old woman was found dead by her daughter, lying in the living room which showed a large pool of blood, secondary droplets and stains from arterial blood spatter, dropping and contact. This bloody scene and two puncture wounds at the anterior aspect of the neck, one of them transecting the left common carotid artery leading to exsanguination, arose suspicion of homicide. However, the wound morphology including notches and a parallel skin incision as well as microradiography demonstrated that the two puncture wounds had been produced by glass. At the scene, a broken wine glass with two…

[Effect of extrahepatic cholestasis on amino acid metabolism in the animal experiment].

UNLABELLED: The knowledge of pathobiochemical processes in extrahepatic cholestasis is one of the prerequisites for the application of infusion solutions adjusted to meet the requirements. Early changes of amino acid metabolism in rats after bile-duct ligation, which indicate disorders in the urea cycle are reported. For this purpose 60 male Wistar rats were laparotomized and the common bile duct was twice ligated under narcosis with pentobarbital. Exsanguination of the animals from the aorta abdominalis occurred after 3, 6, 12, 24, 48 and 72 hours. Analysis of the free amino acids in serum wa…

Suicides by starter's pistols and air guns.

We report the case of a 25-year-old depressed woman who committed suicide with a starter's pistol loaded with CS tear-gas ammunition. The propellant gases of the contact shot entered her chest through the left sixth intercostal space. Exsanguination was caused by perforations of the pericardium and apex of the heart. Autopsy did not reveal any metallic or other foreign bodies that might have originated from the propellant, the cartridge, or any bulletlike material. Her injuries were thus caused by the propellant alone. 0.5 mg L-1 of the CS degradation product cyanide was detected in the cardia…
