External fixation as a primary and definitive treatment of open limb fractures.

This paper analyses the medical records of 5024 injured persons treated and operated on at the Surgical Clinic of the Clinical Hospital in Osijek, during the war against Croatia (1991 and 1992). There were 1653 (33.0 per cent) admitted, while the rest were treated as out-patients. Of the admitted patients 82.7 per cent (1372) had limb injuries. In 760 (15.0 per cent) of the wounded, 1320 limb fractures caused by explosives were diagnosed, out of which 856 were of the lower and 464 of the upper limbs. One hundred and ninety-two (3.9 per cent) were treated by external fixation. External fixators

External fixation. Technical advances based upon multiplanar ligamentotaxis.

The principle of ligamentotaxis obtained by longitudinal traction is useful in restoring skeletal length to distal radial fractures. Using external skeletal fixation to translate the hand in radial-ulnar and dorsal-palmar directions, ligamentotaxis in two additional planes aligns and tilts the distal radial fragment and its articular surface. Following restoration of palmar tilt by palmar translation, wrist position can be adjusted into neutral or extension to help avoid finger stiffness and carpal tunnel syndrome without compromising fracture reduction.

External fixation of severely comminuted and open tibial pilon fractures.

Twenty patients with severely comminuted fractures about the ankle, either severely comminuted pilon fractures or open pilon fractures (three Grade II, seven Grade III), were managed with the use of a Delta-framed external fixator across the ankle joint. All fractures had open reduction and internal fixation (ORIF) with either screw fixation or small plates to stabilize the articular surface with minimal soft-tissue dissection. Average external fixator time was 2.5 months, and the time to union averaged 4.5 months. All fractures healed. Three delayed unions required bone grafting and two had p

External fixation maintained until fracture consolidation in the skeletally immature.

In a prospective study, we investigated the effects of external fixation on fracture union in skeletally immature patients. Twenty-five fractures of the femur and tibia in 21 patients were fixed externally with the intention of leaving the fixator in place until the fractures consolidated or nonunion was established. The main indications were head injury and multiple trauma. One hundred percent of the fractures consolidated with the fixator in place; 45 of 50 joints had 100% motion, three had 95% motion, and two had 60% motion owing to scarring from traumatic wounds. Eight-four percent of the

Internal and external fixation in complex diaphyseal and metaphyseal fractures of the humerus.

The treatment of diaphyseal and metaphyseal fractures of the humerus is often controversial, especially when these fractures are complex and/or unstable (i.e. comminuted, segmental, or with a butterfly fragment). The authors review the indications, advantages, and disadvantages of various open and closed procedures, concluding that a combination of internal and external fixation is a valid treatment for these fractures. The purpose of the external fixation is immediate stabilization of the fracture, which is difficult if not impossible with intramedullary internal fixation alone. Internal fixa

Unilateral external fixation. Methods to increase and reduce frame stiffness.

Unilateral configurations with stiffness characteristics similar to those of the most rigid one- and two-plane bilateral constructs are easily built. Mechanically, most effective were two-plane unilateral frames with a short bone-rod distance. But single- and double-bar anterior unilateral frames had the best combination of clinical and mechanical features. To put gradual frame builddown on a rational basis, a sequence of stepwise frame reduction was developed. This sequence leads from the most rigid unilateral constructs to frames that allow almost complete force transmission across the fract

[Use of a rod apparatus for external fixation in the treatment of hand injuries].

The authors describe an external fixation rod apparatus and the results of its application in 28 patients with various injuries. The apparatus was used in the patients with fractures of phalanges of the fingers and metacarpal bones for extra-articular fixation after the elimination of contractures and dislocations in the joints, in cases of combined epidermatoplasty, arthrodesis, etc. The method of treatment had to be changed only in 2 patients. In all other cases the effect of the rod apparatus was positive.

[The tibio-calcaneus external fixator in the treatment of fractures of the lower tibia].

Twenty-seven patients with complex fractures of the lower end of the tibia were treated by an external Hoffman fixator between 1977 and 1981. Its use was inspired by the presence of skin lesions and by the anatomical type of fracture when it was felt that other methods of treatment would be ineffective or hazardous. In favour of the method was the absence of iatrogenic infection and anatomical results that were superior to those of conservative treatment. Against it, there was a longer time for union, with eight delayed unions and three non-unions and the fact that it was impossible to correct

External fixation for skeletal stabilization of severe open fractures of the hand.

The successful use of external fixation to stabilize severe open fractures elsewhere in the body has led to its adaptation in the hand. It is particularly useful when extensive wounds prohibit internal fixation and when comminution and bone loss render the fracture difficult to control by other methods. External fixation permits simultaneous wound access and fracture management. It can be used alone or adjunctively with other methods of fixation such as Kirschner wires, screws, or plates for definitive fracture management. Alternatively, external fixation can be used for provisional fracture m

[Metacarpal fractures. Use of an external cemented wire fixation device].

Osteosynthesis for fractures of the metacarpals using a simplified system of Kirschner wires introduced percutaneously and maintained by an acrylic cement bridge has been performed in 40 patients. Consolidation was obtained consistently, within one month on average, with almost no morbidity and virtually without sequelae. This procedure is extremely simple and can be used for all extra-articular fractures of the metacarpals with displacement.

External fixation of severe open tibial fractures with the Hoffmann frame.

Extensive experience with stable external fixation of open tibial fractures has confirmed the observation that this method promotes healing of skin and soft tissue damage, reduces the risk of infection, and facilitates the treatment of patients with multiple injuries. In cases of very severe open fractures the method seems to be the treatment of choice and can reduce the frequency of amputation. Recently, complete long-term follow-up results showed that healing times can be reduced if the period of rigid external fixation is reduced. Functional end results after stable external fixation compar

Guidelines for treatment of open fractures and infected pseudarthroses by external fixation.

There are three phases in the treatment of open fractures and infected pseudarthroses by the Hoffmann external fixator. During the initial phase a careful surgical debridement is performed in the bony tissues, and the external fixator is applied in a manner appropriate to the type and level of the lesion site (double- or triple-frame mounting; additional anchorages; neutralization, compression, or distraction; ligamentotaxis). During the intermediate phase both the nursing of the patient and care of the apparatus are critical for the avoidance of complications; it is during this period that gr

External fixation use in the management of massive upper extremity trauma.

Immobilization of fractures associated with instability and comminution by an external skeletal device facilitates wound care, skin coverage, and ultimate reconstructive surgery. External skeletal fixation was applied in 20 patients who sustained complex open injuries of the upper extremity. All injuries were associated with extensive soft tissue and skin defects. Associated neurovascular injuries included 14 nerve injuries and five major vascular injuries. The Hoffman fixation device was used in the majority of cases. Skin coverage was obtained in all patients by an appropriate method of skin

Temporary Stabilization of Tibia Fractures: Does External Fixation or Temporary Plate Fixation Result in Better Outcomes?

BACKGROUND: Provisional stabilization of high-energy tibia fractures using temporary plate fixation (TPF) or external fixation (ex-fix) prior to definitive medullary nailing (MN) is a strategy common in damage control orthopaedics. There is a lack of comprehensive data evaluating outcomes between these methods. This study compares outcomes of patients stabilized with either TPF or ex-fix, and with early definitive MN only, assessing complications including nonunion and deep infection. METHODS: A retrospective review was performed on adult patients with tibia fractures treated with MN followed

Definitive External Fixation for Anterior Stabilization of Combat-related Pelvic Ring Injuries, With or Without Sacroiliac Fixation.

BACKGROUND: Combat-related pelvic ring injuries frequently lead to placement of a temporizing external fixation device for early resuscitation and transport. These injuries are commonly complicated by concomitant polytrauma and extensive soft-tissue injuries, which may preclude early internal fixation and lead to prolonged use of external fixation. To date, few studies have reported on the outcomes of definitive external fixation for combat-related pelvic ring injuries. QUESTIONS/PURPOSES: (1) In patients treated with definitive external fixation after combat-related pelvic ring injuries, how
