Necrotizing fasciitis is an acute soft-tissue infection rarely seen in the head and neck area. The case of a 29-year-old female with necrotizing cervical fasciitis of unknown origin is presented. These infections usually result from dental infections; this patient, however, denied any associated dental problems. The patient presented with an erythematous swelling over the anterior neck associated with a choking sensation and an elevated WBC suggestive of acute thyroiditis. After three days of hospitalization the skin over the anterior neck area became necrotic. Surgical debridement of the area…

Necrotizing fasciitis is a rapidly spreading infection of the subcutaneous tissue and fascia; diabetes mellitus appears to be the most frequent underlying disease. Early diagnosis and immediate aggressive surgical therapy are paramount to curtail morbidity and mortality, but diagnosis is often difficult and unnecessarily delayed. We describe a case of necrotizing fasciitis precipitating diabetic ketoacidotic coma where correct diagnosis was not made until the 14th hospital day. We stress the fact that physicians caring for critically ill patients should be keenly aware of the possibility of ne…

Eosinophilic Fasciitis is a syndrome characterized by exertion related scleroderma-like skin changes, peripheral eosinophilia, hypergammaglobulinemia and diffuse faciitis. Controversy exists as to the precise classification of the syndrome, i.e., whether it is a distinct entity or a variant of scleroderma. We describe a patient with eosinophilic faciitis but with several unique features: 1) progressive skin changes unresponsive to corticosteroid therapy; 2) elevated anti-DNA antibodies; 3) hypocomplementemia; and 4) a followup biopsy showing sclerodermatoid skin changes. These features and oth…

Serum immune complexes were measured in 22 patients with eosinophilic fasciitis, 8 of whom had serial determinations. Elevated levels were found by Raji cell radioimmunoassay in 14 (64%) patients, by agarose gel electrophoresis in 13 (59%), and by C1q agglutination-inhibition in 9 (41%). Elevated levels by Raji cell assay were present more frequently at times of active disease [17 of 22 sera (77%)] than at times of inactive disease [2 of 24 (8%) (P less than 0.0005)] and were more closely correlated with disease activity than were eosinophilia, hypergammaglobulinemia, or increased erythrocyte …

Two cases of necrotizing fasciitis in intravenous cocaine abusers are presented to alert the medical community to the possibility of these severe infections in such addicts. Antimicrobial therapy should include agents effective against Bacteroides species, streptococci, and Gram-negative aerobes; therapy directed only against staphylococci and Gram-negative aerobes is not sufficient.

A detailed report is made of the clinical, histological, biological manifestations of eosinophilic fasciitis, i. e. the Shulman's syndrome, about a 53-year-old man. An extreme induration of sub-cutaneous tissues from arms, legs and trunk, without involvement of the face and extremities, was associated with severe thickening of deep peri-muscular fascias. Raynaud's phenomenon was absent, as were morpheas and visceral involvement. Results of biopsies studied by standard, electron and I. F. microscopy, revealed sclerosis and cellular infiltrates (lymphocytes, plasma cells, histiocytes and eosinop…

We report a case of a 61-year-old immunosuppressed man with rheumatoid arthritis on etanercept and past intravenous drug use who presented with intractable hiccups, an unreported feature of cervical necrotizing fasciitis (CNF). He was initially diagnosed with a viral syndrome, but later developed severe sepsis with neck swelling, dysphagia, and ligneous induration. The absence of skin changes, probably secondary to etanercept-induced immunosuppression, hindered diagnosis. Contrast-enhanced computed tomography (CT) scan of the neck indicated CNF accompanied by descending necrotizing mediastinit…

BACKGROUND: Nodular fasciitis (NF), also known as pseudosarcoma fasciitis, is defined as a benign, self-limiting, vascular, or fibrovascular proliferation. It is commonly found on the trunk and upper extremities. NF is rarely found in the orbit and periorbital regions, with only a few case studies reported since it was initially described by Font et al. in 1966. NF usually presents as a rapidly growing solitary tumor and is often mistaken for a sarcoma due to its high cellularity and increased mitotic activity.
CASE PRESENTATION: We present a case of NF in a 41-year-old Saudi man with a 1-yea…

BACKGROUND: To describe the presentation and management of patients with periorbital necrotizing fasciitis (PONF) through an observational retrospective case series. The clinical notes of twelve consecutive patients managed by the Oculoplastic and Orbital Service and Maxillofacial Service of the Manchester University NHS Foundation Trust between 2018 and 2023 were reviewed. Five of these patients were contactable and gave informed consent for inclusion in the study.
METHODS: Retrospective review of patient characteristics, risk factors, clinical findings, Laboratory Risk Indicator for Necroti…

INTRODUCTION: Necrotizing Fasciitis (NF) is a severe and aggressive disease with rapid progression associated with systemic diseases, its presentation is rare in the head and neck region.
MATERIALS AND METHODS: We present the case of a 69-year-old patient who consulted urgently for a 3-day history characterized by right molar pain with subsequent increase in right facial volume and periorbital cellulitis, with uncontrolled diabetes mellitus. The condition showed a cyclical evolution influenced by existing comorbidities, with the patient undergoing both surgical and medical treatment with a mu…

Periorbital necrotizing fasciitis (NF) is a devastating bacterial infection associated with irreversible inflammatory destruction of soft tissues. Outcomes include disfigurement, vision loss, septic shock, and death within hours to days. We describe two cases of periorbital NF that presented to our unit within a three-month period. We aim to highlight the key clinical features of periorbital NF, demonstrate the rapid progression of the disease, and the need for prompt identification and decisive intervention. Both patients presented with fever and left-sided periorbital swelling and showed rap…

Eosinophilic fasciitis (EF) is a rare connective tissue disorder characterized by painful edema and induration of the limbs and trunk, likely associated with hypereosinophilia and hypergammaglobulinemia. EF causes arthralgia and range of motion limitation, leading to significant functional impairment and poor quality of life. Since its description by Shulman in 1974, over 300 cases have been reported. We present here a review of the latest diagnostic, pathophysiological and therapeutic developments in this disease. Magnetic resonance imaging appears useful to guide diagnosis and biopsy. Diagno…

Nodular fasciitis (NF) is a myofibroblastic proliferation that is uncommonly present in pediatric patients. These benign neoplasms can masquerade as more insidious sarcomatous proliferations on both clinical exam and initial histopathologic review, often prompting undue concern in patients, parents, and providers. While immunohistochemical analysis of NF can be variable, adding to the diagnostic uncertainty, molecular analysis documenting ubiquitin-specific protease 6 (USP6) gene rearrangement can help confirm the diagnosis as an association between NF and USP6 overexpression was first identif…

PURPOSE: To determine whether severity of periorbital necrotizing fasciitis can be predicted based on premorbid patient characteristics.
METHODS: Records of 10 consecutive patients with periorbital necrotizing fasciitis presenting at a single center, treated by one attending ophthalmic plastic surgeon, were retrospectively reviewed. Demographic information and medical history were used to determine a Charlson Comorbidity Index (CCI) score for each patient. Other variables included presenting visual acuity, number of surgical debridements performed, infectious organism (if known), and visual a…
