Femoral shaft fractures following hip arthroplasty can be difficult to manage, particularly in elderly patients. Nine patients were treated with open reduction and rigid internal fixation using cement augmentation for screw fixation in the distal shaft. The stability achieved allowed all patients to be mobilized in the immediate postoperative period. No incidences of reduction loss or implant failure occurred, and all fractures had healed by the time of review. The results obtained support this method of treatment for ipsilateral femoral shaft fractures when prosthetic stability has not been c

A case of bilateral fractures of the femoral neck resulting from high-voltage electric injury is reported. Surgeons caring for patients with electrical injuries must be aware of the possibility of this injury as well as other skeletal injuries which may result from muscle contraction or falls related to electric shock. Without vigilance for these injuries, diagnosis may be delayed.

The morbidity and mortality of 11 femoral neck fractures were analyzed to compare operative and conservative management of femoral neck fractures in dialysis patients. All fractures occurred in older men with severe cardiac, pulmonary, gastro-intestinal, and neurologic conditions and with advanced renal osteodystrophy. Six of the seven operated patients survived the surgery and achieved varying degrees of ambulation. Stability of the operated hip was excellent in each case. Post-operative complications included transient confusional state related to narcotics, pneumonia, decubitus ulcers, and

Débridement of the wound and immediate reamed nailing were performed on sixty-seven patients who had an open fracture of the femoral diaphysis. Forty-two of these patients were followed for a minimum of twelve months (average, twenty-three months). Using the classification system of Gustilo and Anderson, there were fifteen (36 per cent) Grade-I, nineteen (45 per cent) Grade-II, and eight (19 per cent) Grade-III soft-tissue injuries. All of the fractures healed within four months after injury. The average flexion of the knee was 127 degrees and that of the hip, 129 degrees. At least 90 degrees

In a series of over 1000 elderly patients with femoral neck fracture, who were routinely screened by iliac crest bone biopsy, a 2% prevalence of osteomalacia was found. This low figure suggests that osteomalacia is not a significant predisposing factor in the development of these fractures and that routine screening is not cost-effective.

Sixteen young adults with neglected femoral neck fractures were treated from 1974 to 1981. The ages ranged from 16 to 43 years. The periods of delay after fracture were three months to two years. Most of the patients were initially treated by a "Chinese bone setter" with subsequent development of leg shortening, upward displacement of the distal fragment, and a variable degree of absorption of the femoral neck or head. Treatment included skeletal traction followed by open or closed reduction, internal fixation, or angulation osteotomy with or without bone graft. Two to eight years later, there

In 94 patients with femoral neck fracture the vitality of the femoral head was determined pre- and postoperatively with 99mTc-MDP-scintimetry. In half of the fractures a Thornton nail was used, in the other half three Scand hip pins. Irrespective of the displacement, no difference was noted in the two groups. In half of the operations in each group the fracture was impacted by hammer at the end of the operation. In displaced fractures, impaction caused a decrease in femoral head vitality.

Fractures of the distal femur comprise 4 per cent of all femoral fractures. The records of a consecutive series, from 1969 to 1976, of 135 adult patients with 137 fractures, not caused by malignant disease, were reviewed for age, sex, previous disease, type of fracture, treatment and end-result. Eighty-four per cent of the patients were over 50 years of age, 19 per cent had other general factors predisposing them to fracture, and 42 per cent had previous or current disease of the fractured leg. Eight-three fractures were caused by moderate and 52 by severe trauma. Unicondylar fractures were se…

Femoral neck fractures are of great interest for challenging several problems to indicate surgical procedures. Even though ages and general or local conditions of the patient, and types of fracture or some other influencing factors are sufficiently taken into consideration, it is very difficult to foresee the fate of femoral head on the radiogram at the time of injury. This viability of fractured femoral head is the most important factor to choose the method of treatment, but no adequate method of assessing the viability has been established. One hundred and twelve femoral heads removed in the

Recognition of ligamentous injuries of the knee in the acute phase is essential for optimum treatment and prognosis. A multitraumatized patient with a fractured femur and occult knee ligament damage presents a difficult diagnostic challenge. This paper retrospectively reviews 52 patients with 54 midshaft femoral fractures. The high incidence (48%) of ipsilateral knee ligament damage, particularly of a severe nature (30%) evident at followup (mean 24.5 months) is alarming. In the 26 cases of knee ligament damage, the mean time from injury to documentation of instability is 12.8 months. Motor ve…

Six healthy young men sustaining femoral shaft fractures in traffic accidents were treated in pins-in-plaster. Complications included pintract infection (3/6), shortening, angulation (1), malrotation (2). Recasting and rehospitalization was frequent. These adverse experiences with this method of treatment of femoral shaft fractures should discourage its further use.

The results of the Deyerle method of pin plate fixation for intracapsular fracture of the neck of the femur are less satisfactory than the results in the original literature. We treated 143 intrascapsular neck fractures of the femur by the Deyerle method during the period from 1969 to 1975. Seventy-four patients had follow-ups of one year or had a major complication leading to failure prior to one year for evaluation. The failures were the result of avascular necrosis in 17.6% of the patients, non-union in 27% of the patients, and pin breakage in 1.35% of patients. Eight cases with combined av

In 136 fatigue fractures in military recruits, 58 were in the femoral shaft and 43 were in the tibia. Fatigue fractures in the long bones of the lower limb can be classified on the extend of the periosteal reaction. Most were grade 1, showing periosteal reaction only on one or two cortices as seen on the anteroposterior and lateral projections of the roentgenograms. Physical activity was discontinued immediately when fatigue fracture of the femur or tibia was suspected on clinical grounds alone. Early clinical diagnosis the long bone fatigue fractures in all patients except one. The injured so

We report two rare cases of femoral neck fracture resulting from osteonecrosis of the femoral head (ONFH) that was undiagnosed at the patients' initial visits. The patient in the first case had sequential bilateral displaced femoral neck fractures. Because no osteonecrosis of the femoral head was visible on X-ray film and the data of liver function tests were normal, ONFH was not diagnosed. In addition, because the patient was a 55-year-old man with normal everyday functioning, closed reduction with cannulated screws was performed at both visits. Nine months later, he came to our outpatient de

Cannulated screw fixation is the main therapy for femoral neck fractures, especially in young patients. The traditional surgical procedure uses C-arm fluoroscopy to place the screw freehand and requires several guide wire adjustments, which increases the operation time and radiation exposure. Repeated drilling can also cause damage to the blood supply and bone quality of the femoral neck, which can be followed by complications such as screw loosening, nonunion, and femoral head necrosis. In order to make fixation more precise and reduce the incidence of complications, our team applied robot-as

We report the case of a 36-year-old man, who presented to us five months after the initial trauma. He had been treated elsewhere with a cephalomedullary femoral nail. He described severe pain in his right thigh and groin that confined him to a wheelchair. He had shortening of the right lower limb and painful restriction of movements of the right hip. Radiographs demonstrated hypertrophic callus with a gap at the femoral shaft while the neck fracture was in varus malalignment with bone resorption; the neck fracture been fixed using two hip screws that were missing the nail. The patient was mana

The incidence of displaced femoral neck fractures among elderly patients is increasing as the population ages.Historically, the preferred treatment for displaced femoral neck fractures in elderly patients has been hemiarthroplasty with use of cemented fixation of the implant. However, there is evidence that this technique may be associated with fat embolization and subsequent cardiopulmonary arrest in the early postoperative period. Cementing techniques are also associated with increased operative time when compared with cementless techniques. There is increasing evidence that, among elderly pa

A prospective followup study of 36 femoral neck fractures according to the assessment of femoral head perfusion using dynamic magnetic resonance imaging is reported. Patients were divided into three groups based on the dynamic magnetic resonance imaging findings (dynamic curve pattern and relative enhancement ratio) that were conducted within 48 hours of the injury. Traction was used to achieve anatomic reduction and to prevent additional damage to vascularity until minimally invasive internal fixation could be done. The fractures of all 17 patients whose femoral head perfusion was normal (Typ

A follow-up study regarding the occurrence of segmental collapse of femoral head has been done in 53 patients of united femoral neck fracture including a periodic examination by 99mTc-MDP scintimetry. The results showed that the sequential variations in the nuclide uptake ratio of the femoral heads could reflect the histological repairing process and metabolic level in the femoral head. A sustainedly high nuclide uptake in 12 months after operation often denotes that there are defects in the repairing process of the ischemic femoral head, and that the segmental collapse of the femoral head wil
