Primary adrenal high-grade undifferentiated pleomorphic sarcoma (UPS) is an exceptionally rare malignant mesenchymal neoplasm, and presentation with brain metastasis as the initial manifestation is even more uncommon. This report highlights the diagnostic evaluation and multidisciplinary management of a patient presenting with an undifferentiated brain metastasis accompanied by an adrenal mass. A 75-year-old woman presented with speech disturbance, cognitive slowing, memory decline, gait instability, and right-sided weakness. Brain computed tomography and contrast-enhanced magnetic resonance i

TEMPI syndrome (TEMPI) compounds telangiectasias and polycythemia with elevated erythropoietin levels, monoclonal gammopathy, perirenal fluid collections, and intrapulmonary shunt. Although the pathophysiology of this syndrome remains unclarified, prior research has been established that it is a plasma cell neoplasm, often containing less than 10% bone marrow plasma cells. 18F-FDG PET serves as a valuable instrument for initial staging and treatment monitoring in multiple myeloma management. Thus, 18F-FDG PET can be legitimately applied for TEMPI assessment. Here, we present the first 18F-FDG

Typically, myeloma cells express a monoclonal immunoglobulin (Ig), either heavy or light chain. Here, we present a case of multiple myeloma with clonal dual expression of kappa and lambda light chain in a 74-year-old woman. Awareness of rare biphenotypic myeloma is important for proper diagnostic workup. A 74-year-old woman underwent hip replacement with an incidental finding of 20% plasma cells in the femoral head. Subsequent bone marrow biopsy also showed about 30% of plasma cells negative for CD20, CD56, and CD117. Immunohistochemistry (IHC) and in situ hybridization studies showed a mixtur

BACKGROUND: The optimal surgical treatment for patients presenting with (impending and complete) pathological proximal femoral fractures is predicated on prognosis. Guidelines recommend a preoperative biopsy to exclude sarcomas, however no evidence confirms a benefit.
OBJECTIVE: This study aimed to describe the diagnostic accuracy, morbidity and sarcoma incidence of biopsy results in these patients.
MATERIAL AND METHODS: All patients (n = 153) presenting with pathological proximal femoral fractures between 2000 and 2019 were retrospectively evaluated. Patients after inadvertent surgery (n =

PURPOSE OF THE STUDY: Pluridisciplinary management of patients with metastasis to the femur is well defined, but the choice between palliative surgery or abstention must be decided on the basis of a few evaluated prognostic criteria. We report a series of 24 cases of metastasis to the weakened or fractured femur which was evaluated with the Tokuhashi score and treated by surgery.
MATERIAL AND METHODS: Sixteen women and eight men, mean age 71 years (58-89) underwent centromedullary nailing of the femur. These patients had metastases from breast cancer (n = 13 of the 16 women). Twenty of the 24

We report a new technique to create an effective lower extremity weight-bearing stump for two patients who had extensive segmental loss of femoral bone proximal to the distal femoral condyles. One patient had previously had complete resection of the proximal part of the femur because of an infection following the insertion of a custom femoral replacement and hip arthroplasty prosthesis. The other patient had had débridement of the femur from the subcapital line to the femoral condyles because of post-traumatic osteomyelitis after failure of a reconstruction with a massive allograft. Both patie

INTRODUCTION: Erdheim-Chester disease (ECD) is a rare disease that belongs to the group of Dendritic and histiocytic neoplasms. Only 2000 cases have been reported worldwide. It can present with a wide range of symptoms, making a differential diagnosis especially difficult. The primary and most important diagnostic tool is a biopsy of the affected organ/tissue. Nowadays the analysis of different mutations affecting the BRAF and MAPK pathways makes it possible to use targeted treatments, such as vemurafenib, dabrafenib, or cobimetinib.
OBJECTIVE: Our aim is to present the results of three male

Giant cell tumor of the bone (GCTB) is a locally aggressive benign neoplasm with a significant propensity for local recurrence following intralesional surgery. Isolated involvement of the lateral condyle of the distal femur is uncommon. Extended intralesional curettage with chemical adjuvants and polymethylmethacrylate bone cementing is the accepted limb-salvage treatment. Management of recurrent disease, particularly with repeat joint-preserving surgery, and long-term outcomes beyond 10 years following revision procedures remain underreported in the literature. INTRODUCTION: Giant cell tumor (GCT) of bone is a locally aggressive, benign neoplasm, accounting for approximately 20% of all bone tumors. While the distal femur and proximal tibia are the most common locations for GCTs, with the majority arising in the epiphyseal regions, their occurrence in the proximal femur is relatively rare, representing only 5.5% of cases. These tumors pose significant management challenges due to their tendency to cause pathological fractures, aggressive local behavior, and involvement of critical weight-bearing bones. Effective treatment requires careful consideratio

Records were reviewed for 96 patients who received distal femoral osteoarticular allografts to reconstruct femurs after wide resections of benign and malignant bone tumors. Thirteen of these were excluded from the study because of death or local recurrence. The mortality rate for all patients was 11%; for those with osteosarcomas it was 27%. The local recurrence rate was 3%. Eighty-three patients were subjected to further analysis. Their complications included fractures (14%), nonunions (12%), arthritis (10%), instability (7%), infections (6%), and resorption (6%). There was a significant diff

Metastatic lesions in the proximal femur can reduce hip strength and lead to pathologic fracture. However, current methods for identifying patients at risk of pathologic fracture are inadequate. We hypothesized the percentage of intact proximal femoral strength remaining after formation of a simulated lytic defect within the femoral neck or at the level of the lesser trochanter depends on defect location within the respective region. Computed tomography scan-based finite element models of 12 cadaveric proximal femora were used to evaluate the effect of 20-mm-diameter spherical voids at various

Currently, there is no proven sensitive or specific method for predicting pathological fracture of the femur. The clinical management of lytic femoral metastases is based on geometric measurement of the bone, of the defect, or both. However, the mechanical behavior of a structure depends on both its material and geometric properties. Our hypothesis is that a change in bone structural properties as the result of tumor induced osteolysis determines the fracture risk in bones with skeletal metastases. We developed a method of QCT (Quantitative Computed Tomography) combined with engineering beam a

The senior author has used 41 femoral recon nails of one design in 41 consecutive patients with 30 impending and 11 pathologic femur fractures. All nails were inserted with minimally invasive surgical technique. The hospital stay was shorter (average, 5.6 days) and discharge to home was more likely (77%) after stabilization of impending pathologic fractures compared with fixation of completed fractures (length of hospital stay averaged 7.8 days, with a 36% discharge-to-home rate), as patients with completed fractures required a higher level of postoperative care. Fixation prior to fracture occ

The diagnosis of capsular retraction of the hip is based on measurement of the capacity of the joint cavity on arthrography: it is reduced by at least 25% (normally 15 ml +/- 2 ml). The opaque area is only visible reduced in the more severe forms with a capacity of 5 ml or less. The arthrographic image is therefore not the key to the diagnosis. The major clinical sign is restriction of joint movement, especially in abduction and rotation. Secondary, "surgical" capsular retraction of the hip is the most common form. It is associated with synovial chondromatosis in more than one half of cases. T

The authors present 5 cases of monoarthritis which revealed adjacent bone tumours: a bony metastasis, a fibrosarcoma and three bone lymphomas. In three cases, the synovial histology showed lesions of non-specific synovitis without neoplastic invasion. A review of the literature demonstrates the rarity of these mono- or oligo- arthrites adjacent to bone tumours and two types can be distinguished: neoplastic invasion of the synovium or reactive non-specific synovitis without invasion.

The author reports two cases of Ewing's sarcoma with diffuse neoplastic invasion of the bone marrow causing panmyelopoietic failure. This condition must be differentiated from the medullary aplasia which may occur as a complication of treatment of antitumoral chemotherapy in patients affected by Ewing's sarcoma.

Three patients with foregut (bronchial), hindgut (rectal) or (ovarian) carcinoid tumors had symptomatic bone metastasis with abnormal 99m Tc pyrophosphate bone scans and bone roentgenograms. Six patients with midgut (small intestine or caecal) carcinoid) carcinoid tumors who had no symptoms of bone metastasis had no evidence of bone metastasis on bone scan or bone roentgenographic examination. This study supports the clinical impression that patients with midget carcinoid tumors have a low incidence of bone metastasis.

Atypical femoral fractures (AFFs) occur in osteoporosis patients receiving long-term bisphosphonate. Atypical femoral fractures also occur in cancer patients receiving long-term bisphosphonate or denosumab, but the prevalence is low. We describe a 53-year-old woman with a history of medullary thyroid cancer and skull metastasis who was prescribed bisphosphonate for 6 years and denosumab for 1.5 years, consecutively. Bone scintigraphy performed because of spontaneous groin pain showed uptake in the lateral aspect of the left femur, which was confirmed as impending AFF. In oncological patients r

The orthopedic surgical treatment of metastases is very important in the treatment of osteophile cancers. The surgical option is always proposed after a multidisciplinary decision. The main risk of the metastases is the pathological fracture. This risk has to be evaluated and a preventive treatment can often be performed. The surgical options are multiple: such as preventive osteosynthesis, pathological fracture treatment, reconstruction with prosthesis, carcinological excision… The choice of the treatment is support on the life expectancy and the functional risk of the metastatic bone lesion.
