[The importance of magnification angiography as an investigation for the arterial supply of the femoral head after medial neck fracture (author's transl)].

In observing the development in 20 patients who had been operated upon for fracture of the neck of the femur a significant interrelation between the angiographic result and the formation of a necrosis of the head of the femur has been ascertained. The magnification angiography enables the terminal arterial region of the femoral vessels to be assessed with a still higher degree of certainty and thus appears to be a valuable diagnostic measure for the prognostic assessment of the medial fracture of the femoral neck.

Alterations of bone mineral density of the femurs in hemiplegia.

We evaluated the bone mineral density (BMD) of the bilateral femurs in 112 patients with hemiplegia using dual-energy X-ray absorptiometry in order to elucidate the effect of disuse and immobilization. BMD of the paretic side was significantly reduced compared with that of the non-paretic side in hemiplegic patients (femoral neck 0.582 +/- 0.014 g/cm2 versus 0.623 +/- 0.014 g/cm2 and total femur 0.645 +/- 0.02 g/cm2 versus 0.702 +/- 0.017 g/cm2; mean +/- SEM, P < 0.01, respectively). Femoral BMD in both the paretic and nonparetic limb had significantly (P < 0.01) lower values than in age- and

Finite Element Evaluation of the Femoral Neck System as Prophylactic Fixation to Prevent Contralateral Hip Fractures.

INTRODUCTION: Hip fractures cause significant morbidity and mortality for geriatric patients, and incidence is increasing as the population ages. Following a primary hip fracture, up to 20% may suffer a contralateral hip fracture within 5 years despite fracture risk reduction measures, including fall prevention and osteoporosis pharmacologic treatment. The aim of this study is to assess whether insertion of the Femoral Neck System (Depuy Synthes, West Chester, PA) into the contralateral proximal femur may strengthen the bone and decrease the incidence of contralateral hip fractures. A Controlled Variable Study of the Biomechanical Properties of the Proximal Femur before and after Cancellous Bone Removal.

OBJECTIVE: The biomechanical characteristics of proximal femoral trabeculae are closely related to the occurrence and treatment of proximal femoral fractures. Therefore, it is of great significance to study its biomechanical effects of cancellous bone in the proximal femur. This study examines the biomechanical effects of the cancellous bone in the proximal femur using a controlled variable method, which provide a foundation for further research into the mechanical properties of the proximal femur. METHODS: Seventeen proximal femoral specimens were selected to scan by quantitative computed to

Lower preoperative Hounsfield unit values are associated with intra-operative fractures in cementless bipolar hemiarthroplasty.

UNLABELLED: The use of Hounsfield units (HU) from CT scanning to assess regional BMD has been described. Therefore, we evaluated whether HU could be used to identify osteoporosis of the proximal part of the femur. The results showed that HU assessment is associated with the presence of intra-operative fracture during arthroplasty. INTRODUCTION: The aim with this study was to determine the association of preoperative Hounsfield unit (HU) in hip computed tomography (CT) with intra-operative osteoporotic fracture during cementless hemiarthroplasty. METHODS: In this retrospective study, we revie

Monostotic fibrous dysplasia in the femoral neck. A clinicopathologic study.

In eight patients with monostotic fibrous dysplasia in the femoral neck, the lesion was virtually limited to the neck region without any extension into the shaft. In one patient with symptoms of 40 years' duration and another with a recurrent lesion, there was a deformity in the neck of the femur. A pathologic fracture, including a minor one, was identified in two patients. Roentgenographically, the lesion should be distinguished from various entities producing a localized central region of rarefaction in the proximal femur. The histologic features of fibrous dysplasia are characteristically d

[Traumatology in the elderly. Problems exemplified by femoral neck fractures].

The most frequent complications in response to a fracture of the femoral neck, a typical injury of the elderly, are necrosis of the head of femur and pseudoarthrosis of the femoral neck. Only the around dressed down fracture can be conveyed to conservative treatment. The classification of AO takes these problems into consideration. One possibility to preserve the femur head is by operative insertion of a dynamic hip screw (DHS). Specific problems of the diverse types of fracture and their surgical details are taken into special account.

The effect of pre-fracture proximal femur geometry on hip fracture type in elderly patients.

This study aimed to analyze the relationship between fracture type by determining data on the geometry of the proximal femur in the pre-fracture period in patients over 65 years of age who had hip fractures as a result of low-energy trauma. A total of 127 patients who were admitted to the hospital for reasons other than hip pathology within 1 year before the occurrence of hip fracture and who had an anterior-posterior pelvic X-ray were included in the study. Measurements were made to evaluate the proximal femur geometry, neck shaft angle, central edge angle, femoral head diameter, femoral neck

Simultaneous Bilateral Femoral Neck Stress Fracture in a Young Stone Mason.

Unilateral stress fractures of the femoral neck are very uncommon and bilateral involvement is even rarer. They commonly occur in athletes, military recruits, older persons, or individuals with underlying metabolic disorders and very seldom in normal individuals. We present a rare case of simultaneous bilateral fracture neck of femur in a 25-year-old man who came with complaints of pain in bilateral groin for 1 month. There was no history of trauma or history suggestive of excessive activity prior to the onset of pain, but there was history of lifting heavy weights daily. On evaluation with MR

Total hip arthroplasty after failed treatment of proximal femur fracture.

INTRODUCTION: Total hip arthroplasty (THA) is a good option as a salvage procedure after failed treatment of proximal femur fracture. The anatomy of the proximal femur, however, makes this surgery complicated and challenging. The purpose of this study was to evaluate the radiographic and clinical outcomes of THA after failed treatment of proximal femur fractures. MATERIALS AND METHODS: We retrospectively analysed 50 consecutive THAs (42 women, 8 men; mean age 77 years) after failed treatment of a proximal femur fracture. Mean postoperative follow-up was 58.1 months. Preoperative diagnoses wer

Morphological study on distribution of nutrient foramina in femoral neck in relation to retinacula of weitbrecht with its surgical implications.

BACKGROUND: Now a day's many surgeries are done around the head and neck of femur like femoral head resurfacing arthroplasty for managing advanced hip-joint degeneration. Thus, it is vital for an orthopaedic surgeon to exactly understand the vascular supply of the neck of femur. The knowledge of distribution of foramina and their relation to the retinacula of Weitbrecht will help orthopaedic surgeons to choose their management technique for various injuries. So, this study was undertaken to study the number, size and distribution of nutrient foramina in all the surfaces of neck of femur and th

Variation in bone mineral density between different anatomical sites in a normal local population.

Bone mineral density (BMD) measured using dual energy X-ray absorptiometry (DXA) can be expressed in terms of standard deviations, above or below mean young adult T-scores and above or below age-matched Z-scores. The differences between the left neck of the femur (LN), right neck of the femur (RN) and lumbar spine (L2-L4) were statistically significant, irrespective of whether expressed as T-scores or Z-scores. Therefore skeletal status assessment should involve as many sites as practically possible. Considering the low radiation dose and short scanning time, it is suggested that at least the

A TRAUMATIC BILATERAL FEMORAL NECK FRACTURES IN A HEALTHY PREMENOPAUSAL FEMALE WITH HYPOVITAMINOSIS D.

Bilateral atraumatic femur neck fractures in young adult are very rare. We report a young premenopausal female with osteormalacia secondary to vitamin D deficiency and spontaneous bilateral femur neck fractures. Patient had no reported risk factors for osteomalacia but hypovitaminosis D was noted upon evaluation. Osteomalacia secondary to low serum levels of vitamin D may lead to stress and fragility fractures. Identification and treatment of such patients may decrease the incidence of stress fractures and- possible complications.

The biomechanical effect of notch size, notch location, and femur orientation on hip resurfacing failure.

For hip resurfacing, this is the first biomechanical study to assess anterior and posterior femoral neck notching and femur flexion and extension. Forty-seven artificial femurs were implanted with the Birmingham hip resurfacing (BHR) using a range of notch sizes (0, 2, and 5 mm), notch locations (superior, anterior, and posterior), and femur orientations (neutral stance, flexion, and extension). Implant preparation was done using imageless computer navigation, and mechanical tests measured stiffness and strength. For notch size and location, in neutral stance the unnotched group had 1.9 times

Lag screw with DHS (LSD) for vertical angle femoral neck fractures in young adults.

Vertical Pauwels Type III fractures of the neck of femur in young patients are difficult fractures to treat. These are usually as a result of high energy trauma which are associated with a significant degree of fracture comminution in the neck of femur. This makes the fracture reduction and fixation difficult. Many fixation methods and implants have been described for use in these fractures but are not without reported complications such as non-union and varus collapse. In this article, we highlight the incidence of fracture comminution in both the anterior and posterior cortices of the femora
