From Ebola to COVID-19: emergency preparedness and response plans and actions in Lagos, Nigeria.

BACKGROUND: Lagos state is the industrial nerve centre of Nigeria and was the epicentre of the 2014 Ebola outbreak in Nigeria as it is now for the current Coronavirus Disease (COVID-19) outbreak. This paper describes how the lessons learned from the Ebola outbreak in 2014 informed the emergency preparedness of the State ahead of the COVID-19 outbreak and guided response. DISCUSSION: Following the Ebola outbreak in 2014, the Lagos State government provided governance by developing a policy on emergency preparedness and biosecurity and provided oversight and coordination of emergency preparedne

Scalable, sustainable cost-effective surgical care: a model for safety and quality in the developing world, part I: challenge and commitment.

BACKGROUND: With an estimated backlog of 4,000,000 patients worldwide, cleft lip and cleft palate remain a stark example of the global burden of surgical disease. The need for a new paradigm in global surgery has been increasingly recognized by governments, funding agencies, and professionals to exponentially expand care while emphasizing safety and quality. This three-part article examines the evolution of the Operation Smile Guwahati Comprehensive Cleft Care Center (GCCCC) as an innovative model for sustainable cleft care in the developing world. METHODS: The GCCCC is the result of a unique

Reframing pandemic preparedness: an intelligence-led solidarity model for multi-layered emergencies.

The convergence of infectious disease outbreaks, climate shocks, and conflict is exposing fundamental limitations in existing global health preparedness systems, which remain largely fragmented and disease-specific. Epidemics increasingly occur within multi-layered emergencies, where overlapping risks interact to overwhelm fragile health systems and complicate response efforts. This perspective examines structural gaps in current preparedness approaches, including siloed financing, limited integration of multi-sectoral intelligence, and constrained institutional autonomy. Drawing on a structur

Breast cancer management in middle-resource countries (MRCs): consensus statement from the Breast Health Global Initiative.

In middle resource countries (MRCs), cancer control programs are becoming a priority as the pattern of disease shifts from infectious diseases to non-communicable diseases such as breast cancer, the most common cancer among women in MRCs. The Middle Resource Scenarios Working Group of the BHGI 2010 Global Summit met to identify common issues and obstacles to breast cancer detection, diagnosis and treatment in MRCs. They concluded that breast cancer early detection programs continue to be important, should include clinical breast examination (CBE) with or without mammography, and should be coup

The Italian Twin Project: from the personal identification number to a national twin registry.

The unique opportunity given by the "fiscal code", an alphanumeric identification with demographic information on any single person residing in Italy, introduced in 1976 by the Ministry of Finance, allowed a database of all potential Italian twins to be created. This database contains up to now name, surname, date and place of birth and home address of about 1,300,000 "possible twins". Even though we estimated an excess of 40% of pseudo-twins, this still is the world's largest twin population ever collected. The database of possible twins is currently used in population-based studies on multip

[Female prostitution and HIV/AIDS--review of the literature on epidemiology and personal observations in a contact center for drug dependent patients].

Epidemiological studies show that intravenous drug use represents the main risk for HIV/AIDS infection among female prostitutes in Germany. According to current information, HIV/AIDS infections are not more prevalent among prostitutes who are not intravenous drug users than in the general population. To establish effective preventive measures against HIV/AIDS infection, we must differentiate between two specific target groups: 1) female intravenous drug users who engage in prostitution to finance their drug addiction, and 2) female prostitutes who are not intravenous drug users. Instead of man

A Comparative Analysis of AI HRM Governance Approaches Across African Countries: Continental Patterns and Divergences.

BACKGROUND: The governance of artificial intelligence within human resource management across the 54 sovereign states of Africa presents distinctive challenges that differ markedly from those observed in Euro-American contexts. African organizations have adopted AI-driven workforce tools at annual growth rates exceeding 25% since 2018; however, governance frameworks remain in their nascent stages and are characterized by fragmentation. Current comparative scholarship has largely overlooked innovative governance developments within the African continent, often positioning Africa as a recipient

Out-of-pocket payment for surgery in Uganda: The rate of impoverishing and catastrophic expenditure at a government hospital.

BACKGROUND AND OBJECTIVES: It is Ugandan governmental policy that all surgical care delivered at government hospitals in Uganda is to be provided to patients free of charge. In practice, however, frequent stock-outs and broken equipment require patients to pay for large portions of their care out of their own pocket. The purpose of this study was to determine the financial impact on patients who undergo surgery at a government hospital in Uganda. METHODS: Every surgical patient discharged from a surgical ward at a large regional referral hospital in rural southwestern Uganda over a 3-week per

Policy analysis on drug-resistant tuberculosis in ASEAN member countries using a governance framework approach: a scoping review.

Drug-resistant tuberculosis (DR-TB) heavily contributes to the global challenge of antimicrobial resistance (AMR), resulting in increased morbidity among tuberculosis cases. The Association of Southeast Asian Nations is home to approximately 10% of the world's population, accounting for almost 15% of the annual global TB incidence. Thus, this study aimed to evaluate the existing national policies on DR-TB in the region using the governance framework approach for AMR and the WHO Health System Building Blocks. A thorough search for DR-TB-related national policies was performed on the websites of

The hospital costs of treating necrotizing fasciitis.

BACKGROUND: Management of necrotizing fasciitis places significant demands upon hospital and medical resources. A successful management usually requires extensive surgical intervention and an adjunct hyperbaric oxygen treatment. The cost impact on the health care system has not been well characterized. We have, therefore, analysed the cost of treating this disease at an Australian tertiary referral hospital with extensive case experience and well-developed financial costing systems and have compared this with the current casemix-based government funding arrangements applying in Victoria, Austr

The politics of 'The Natural Family' in Israel: state policy and kinship ideologies.

Israel is the only country in the world that provides nearly unlimited, universal state funding for fertility treatments. This exceptional policy has been widely understood as symbolising the state's pronatalism. In this paper I probe the policy and assess medical experts' practice to show how a specific modality of pronatalism--enhancing 'the natural family' concept--is being construed through legislation and practice. Policy analysis discloses how the relatively efficient and safe technology of donor insemination has been devalued as a last resort solution to male infertility, to be applied

The impact of the single contract on health service delivery in the Democratic Republic of the Congo (DRC): findings from a quasi-experimental study.

The Democratic Republic of the Congo (DRC) initiated a contract (single contract) among the Provincial Ministry of Public Health, Provincial Health Divisions (PHDs), and development partners to strengthen the function of PHDs. Using a quasi-experimental research design, we assessed the impact of the single contract on primary health care (PHC) delivery in the country. We found that the single contract had a no direct impact on PHC services. When combined with the performance-based financing (PBF) program, the single contract demonstrated a more favorable impact on delivering PHC services. The

Mpox in Sierra Leone and the Mano River Union: an epidemic unfolding in real time.

The 2025 mpox outbreak in Sierra Leone represents an unprecedented public health crisis, escalating rapidly from minimal prior cases to over 4400 confirmed infections within 6 months. This surge challenges previous assumptions about mpox epidemiology in West Africa and highlights critical vulnerabilities in disease surveillance, diagnostics and health system preparedness-challenges shared across the Mano River Union (MRU) region, including Liberia, Guinea and Côte d'Ivoire. Despite global commitments to equitable vaccine access, Sierra Leone and its neighbours have faced significant delays and

The Use of the CDC Preventive Health and Health Services Block Grant to Address Social Determinants of Health to Advance Health Equity.

OBJECTIVE: State public health departments use federal funding to examine and address social determinants of health (SDOH) within their communities to improve health and advance health equity. Yet, most federal funding is categorical (ie, funding used for a specific program or narrow purpose), which can create barriers to addressing social and structural drivers of inequity. The Preventive Health and Health Services Block Grant (PHHS Block Grant) is a flexible funding mechanism that provides health departments the latitude to identify and fund initiatives that address local public health needs

Cost-Benefit Analysis and Assessment of Quality of Care in patients with Hemophilia undergoing treatment at National Rural Health Mission in Maharashtra, India.

BACKGROUND: Hemophilia is a genetic disorder with high health care burden. In India, most patients with hemophilia seek care through self-purchasing factor concentrate and incur huge out-of-pocket (OOP) expenditure. In March 2013, the government of India launched a pilot hematology program through the National Rural Health Mission for providing free treatment services to patients with hemophilia in the state of Maharashtra. OBJECTIVES: To estimate the benefit-cost ratio of the program from a patient perspective, to estimate reduction in OOP expenditure of the patients and their families, and

If rollbacks go forward, up to 14 million children could become ineligible for public or subsidized coverage by 2019.

In spring 2015 Congress passed legislation to extend funding for the Children's Health Insurance Program (CHIP) through the end of fiscal year 2017. This two-year extension pushes to 2017 the question of whether CHIP funding will end, allowing states to end their separate state CHIP programs. Also, when the Affordable Care Act's maintenance-of-effort requirements expire after 2019, states will be allowed to roll back Medicaid- and CHIP-eligibility thresholds to minimum levels allowed by federal law. This study investigated the potential health insurance options available to low-income children
