In order to examine the chronic effects of anticancer drug 5-fluorouracil (5-FU) on reproduction and development, we exploited Caenorhabditis elegans as a model system. We demonstrate that 5-FU induces cell-cycle arrest and apoptosis of germline cells and reduces by approximately 30-40% the number of mitotic nuclei per gonad arm when compared to untreated worms. PURPOSE: To evaluate the efficacy and safety of augmented trabeculectomy with 5-fluorouracil (5-FU) and mitomycin C (MMC) compared to 5-FU only for the treatment of pediatric glaucoma.
PATIENTS AND METHODS: In a prospective randomized clinical trial, 8 children (12 eyes) with pediatric glaucoma, either congenital or secondary to: lens aspiration, Sturge-Weber syndrome, or steroids underwent augmented trabeculectomy. Vaginal adenosis and clear cell adenocarcinoma of the vagina occurred in a 44-year-old woman after treatment for condylomata acuminata. She had no known exposure to diethylstilbestrol (DES) in utero. Nineteen women with extensive condyloma acuminatum involvement of the lower female genital tract were treated with seven-day courses of topical 5-fluorouracil. The complete response rate was 68.4%. One patient completed only two courses; she underwent vaginal delivery after cryosurgical destruction of the remaining lesions. Her recurrent lesions in the immediate postpartum period were controlled with additional courses of topical 5-fluorouracil. Only one patient was totally unresponsive to treatment with topical 5-fluorouracil.
