Irreducible lateral patellar dislocation: the importance of impaction fracture recognition.

Irreducible lateral patellar dislocation may occur in the older patient with a previous history of patellofemoral arthritis. The only subtle finding on physical examination to suggest this diagnosis will be positioning of the knee in less flexion than a typical lateral patellar dislocation, anterolateral position of the patella and internal rotation of the patella from the coronal plane. That is, the patella is dislocated laterally but the lateral border comes to lie in a position of variable degrees of anterior displacement relative to the medial patellar border. Plain x-rays may reveal the r

Lateral fracture dislocation of the second and third carpometacarpal joints.

We report a case of a rare injury, lateral fracture dislocation of the second and third carpometacarpal joints, which presented late and was managed by fusion of the involved joints.

Surgical outcomes of short-segment fixation for thoracolumbar fracture dislocation.

BACKGROUND: Currently long-segment pedicle instrumentation for a thoracolumbar (T-L) fracture-dislocation is gaining in popularity. Otherwise, short-segment fixation may be chosen as an another treatment method. This article evaluates the efficacy and complications of short-segment fixation for the treatment of thoracic or lumbar spine fracture-dislocation. METHODS: Twenty patients with thoracic or lumbar spine fracture-dislocation were included in the study. The mean follow-up period was 3 years. Clinical, neurologic, radiologic (angle of deformity, displacement percentage, fusion rate), and

Diagnosis and management of acute fracture-dislocation of the carpus.

Acute fracture-dislocations of the carpus are uncommon. If treated inadequately, however, these injuries can lead to wrist pain and dysfunction as a result of progressive traumatic arthritis. Accurate diagnosis and early intervention are essential for optimal recovery. This article presents the anatomy, epidemiology, and mechanisms of injury of the carpus and the diagnosis, treatment, and treatment results of dislocation of the carpus.

Palmar lunate transscaphoid fracture-dislocation caused by palmar flexion injury.

The authors report a rare case of palmar lunate transscaphoid fracture-dislocation resulting from a palmar flexion injury. After performing an open reduction of the lunate, they used a dorsal approach to fix the fractured scaphoid with a Herbert screw. A Kirschner wire fixation was also performed to stabilize the lunotriquetral joint.

Closed talar body fracture with talonavicular dislocation; a case report.

Closed fracture dislocation of the body of the talus is rare. The dislocation of the subtalar and ankle joints (associated with fracture body) has been reported and already described in the current classifications. We report a case of closed fracture of the body of the talus associated with talonavicular dislocation. Open reduction and internal fixation was performed after failed attempt of closed reduction. The fracture has healed and there is no evidence of avascular necrosis after 2 years follow up. We recommend early evaluation of this injury by CT scan or by proper radiographic views unde…

The operative treatment of spinal fracture-dislocation without neurologic deficits.

OBJECTIVE: To evaluate the results of operative treatment of spinal fracture-dislocation without neurologic deficits. METHODS: Eighteen patients with spinal fracture-dislocation were neurologically intact at the time of injury, and all were treated operatively. The fracture sites were: 8 cases in cervical spine, 3 cases in thoracic spine, and 7 cases in lumbar spine. Eight patients with cervical injuries had variant degrees of forward slide and kyphotic deformity. Of the 10 thoracic and lumbar fractures, one had lateral dislocation, 4 cases with kyphotic deformities, 5 cases with spinal canal

Salter-Harris type III fracture-dislocation of the proximal humerus.

Salter-Harris type III fractures of the proximal humerus are rare injuries. We report a Salter-Harris type III anterior fracture-dislocation of the proximal humerus in a 10-year-old boy that was open reduced and internally stabilized. A bone scan performed during the initial hospitalization and at 2-year follow-up revealed devascularization and subsequent revascularization of the humeral head. At 2-year follow-up, the patient had full motion of the shoulder, no pain, and arm strength equal to that of the contralateral side. Four cases of Salter-Harris type III fractures of the proximal humerus

[Irreducible fracture dislocation of the ankle: report of two cases].

The goal of operative treatment of ankle fracture are to obtain an anatomical reduction and rigid fixation to ensure a healed fracture and n … racture with deltoid ligament or posterior tibialis tendon interposition. Type II is an irreducible fracture dislocation of the ankle due to posterior dislocation of the fibula. Type III is a diastatic ankle fracture with extensor tendons entrapped in the distal tibiofibular joint and restricted by extensor retinaculum. The mechanism of the irreducible ankle fracture …

Fracture-dislocation of the shoulder in a child: case report.

This is a case of an 11-year old girl with a delayed diagnosis of a shoulder fracture-dislocation. Shoulder dislocations are rare, and proximal humerus fractures are uncommon. A fracture-dislocation in a child is, to our knowledge, as yet unrepresented in the English-language literature. The child's injury resolved without recurrence.

[Case report of an isolated fracture-dislocation of the trapezium].

Conservative management in case of a rarely observed fracture dislocation of the trapezium is reported: pertinent extension of the thumb in plaster of Paris (POP) for more than six weeks effected consolidation and healing of the unstable fracture.

[Vertebrectomy in treating fracture-dislocation of the spine with paraplegia].

From April 1980 to August 1984, 11 cases of fracture--dislocation of spine at the level of T-11 to L-2 with obvious kyphosis at the site of displacement, with paraplegia, were treated by vertebrectomy through posterior median approach. The operations were performed 2 to 28 weeks, averaging 8 weeks, after injury. 5 of the 9 patients who had complete paraplegia suffered also causalgia in both legs. The other 2 had incomplete paraplegia only. Follow-up study, 4 years and 5 months post-operation on average, revealed complete reduction of the fracture-dislocation in 10 patients and incomplete in on

Bilateral traumatic dislocation of the hips with ipsilateral femoral fracture.

A rare case of bilateral traumatic dislocation of the hip with ipsilateral femoral shaft fractures occurred in a 29-year-old man. Reduction was achieved by manipulation, despite the presence of a femoral shaft fracture on one side. A review of the literature reveals reports of three similar cases.

Mandibular condyle fracture and dislocation into the middle cranial fossa.

A case report is presented concerning a patient who sustained a condyle fracture with superior dislocation into the middle cranial fossa. Although relatively rare, other cases of central condylar dislocation have been presented and will be discussed and compared. Diagnosis has been a problem, and treatments have varied greatly for these cases. This is the first case on record, however, in which such a dislocation occurred in conjunction with a fracture of the condylar head. In the present case, the dura was covered with temporalis muscle and the glenoid fossa was covered using the fragments of

Femoral shaft fracture with ipsilateral hip dislocation in a child.

A child victim of low speed violence was found to have a fractured femur and dislocation of the ipsilateral hip. This combination of injuries in a child is very rare. The dislocation was not initially recognized because of absence of suggestive history, symptoms, and signs. After recognition, open reduction of the hip dislocation was achieved through a posterior approach. One month later an osteotomy at the femur fracture allowed the leg to fall to neutral while maintaining the femoral head in the most desirable position.

A case of cerivco-thoracic anomaly simulating traumatic fracture-dislocation.

A patient who sustained severe head trauma had a simulated C7, T1 fracture dislocation. Anteroposterior tomography of the cervico-thoracic junction disclosed anomalous vertebral-costal articulations. The rotation of the spine and the abnormal posterior articulations were secondary to anterior costal bridging. Tomography of the costo-vertebral facets in patients with unusual rib configurations may help delineate cases of thoracic spine rotation.
