Skinny wire and locking plate fixation for comminuted intra-articular distal humerus fractures: a technical trick and case series.

INTRODUCTION: Intra-articular distal humerus fractures present a challenge to orthopedic surgeons. Stable fixation is difficult to achieve in fractures with articular and metaphyseal comminution and osteoporotic bone. Hence, these fractures are more commonly being managed with total elbow arthroplasty. We describe a novel surgical technique that confers stable fixation, allowing for early range of motion resulting in a high rate of union, a functional range of motion, and excellent patient reported outcome scores without the activity restrictions of total elbow arthroplasty. METHODS: Retrospe

Arthroscopic-assisted plate fixation for displaced large-sized comminuted greater tuberosity fractures of proximal humerus: a novel surgical technique.

PURPOSE: The purpose of the present study was to describe the use of a novel hybrid surgical technique-arthroscopic-assisted plate fixation-and evaluate its clinical and anatomical outcomes in the management of large, displaced greater tuberosity (GT) fractures with comminution. METHODS: From 2009 to 2011, this novel technique was performed in 11 patients [2 men and 9 women; median age, 64 years (range 41-83 years)] with large, comminuted GT fractures, with fragment displacements of >5 mm. The preoperative mean posterior and superior migration of the fractured fragment, as measured on compute

Biomechanical comparison of locking versus nonlocking volar and dorsal T-plates for fixation of dorsally comminuted distal radius fractures.

OBJECTIVES: The purpose of this study was to gain insight into the effect of plate location and screw type for fixation of extra-articular distal radius fractures with dorsal comminution (Orthopaedic Trauma Association Type 23-A3.2). METHODS: Sixteen pairs of cadaver radii were randomized to four plating configurations: dorsal locking, dorsal nonlocking, volar locking, and volar nonlocking. A standard 1-cm dorsal wedge osteotomy was used. Cyclic axial loads were applied for 5000 cycles. Stiffness and fragment displacement were recorded at 500 cycle-intervals. Pre- and postcyclic loading radio

The palmar locking compression plate is biomechanically comparable to the dorsal pi plate for dorsally comminuted, intraarticular wrist fractures.

A clinically appropriate fracture model and testing regimen were used to test the null hypothesis that a palmarly applied locking plate was inferior to a dorsally applied Pi plate in the stabilisation of dorsally comminuted intraarticular wrist fractures. Sixteen standardised fractures of Synbone models of the radius were stabilised using either a palmar locking compression T plate (the experimental group) (n=8) or a dorsally applied Pi plate (the control group) (n=8). The constructs were tested on an Instron materials testing machine. Deformation was monitored during 500 loading cycles to 200

Comminuted femoral-shaft fractures: treatment by roller traction, cerclage wires and an intramedullary nail, or an interlocking intramedullary nail.

In a retrospective study, we attempted to assess progress in the treatment of comminuted fractures of the femoral shaft at Parkland Memorial Hospital from 1978 to 1983. Seventy-nine comminuted femoral-shaft fractures were available for follow-up: thirty-two treated by roller traction, twenty-three treated by cerclage wires and an intramedullary nail, and twenty-four treated by an interlocking intramedullary nail. Using the classification of Winquist and Hansen, Grade-III and IV comminuted fractures accounted for 69 per cent of those treated by roller traction, 68 per cent of those treated by n

Continuous Passive Motion in the Rehabilitation of a Comminuted Patellar Fracture Following Delayed Physiotherapy Initiation: A Case Report.

Comminuted patellar fractures are challenging injuries, and delays in post-operative physiotherapy significantly increase the risk of arthrofibrosis and long-term disability, particularly in resource-limited settings. A 40-year-old male with multiple comorbidities presented 53 days after open reduction and internal fixation (ORIF) for a comminuted patellar fracture, suffering severe knee stiffness. Initial assessment revealed profound functional impairment: knee flexion limited to 10°, a Visual Analog Scale (VAS) pain score of 8/10, a Kujala score of 19/100, and a Lysholm score of 7/100. A 4-w

Ankle arthrodesis with non-vascularized fibular graft for neglected comminuted talar body fracture: A case report.

Talus injuries present a diversified and distinct set of care issues, having a significant impact on the patient's short- and long-term functional outcomes. Talus fractures are extremely uncommon, accounting for only around 1% of all foot and ankle fractures. The degree of initial fracture displacement and the accuracy of fracture reduction are both factors that influence the result. This case report describes a case of a 30 year-old male patient with neglected close fracture talus on the left foot. The patient presented to the outpatient department with chief complaint of progressive pain ove

Rare Case of Open Comminuted Scaphoid Fracture Dislocation: Treatment With Free Bone Graft and Locking Plate.

To the best of our knowledge, there are no reports in the literature of an open comminuted scaphoid fracture dislocation. We present such a rare case. The case report illustrates the case of a 58-year-old right-handed press operator who injured his left wrist when his hand got caught in a press machine. He received initial treatment at another hospital and was subsequently referred to our hospital. Eight days after the injury, surgery was performed under the brachial plexus block. Successful bone fusion was achieved through volar locking plate fixation, primary free bone grafting from the radi

Perforation of the Knee Joint Following Antegrade Intramedullary Nailing of a Comminuted Femoral Diaphyseal Fracture: A Case Report.

A 63-year-old man with a comminuted spiral femoral shaft fracture was treated with closed reduction and internal fixation with a cephalomedullary nail. Two weeks postoperatively, one of the two static distal interlocking bolts began backing out and was removed. The nail ultimately migrated distally and perforated the knee joint at four months postoperatively. The patient was successfully treated with an exchange nail and percutaneous bone graft to the fracture site. A single static distal interlocking bolt may be inadequate to maintain length in a healing comminuted spiral femur shaft. Multipl

Short-term Clinical Outcome of Dual Plate Fixation in the Treatment of Proximal Humerus Fractures with Calcar Comminution.

OBJECTIVE: Calcar comminution has been considered to be the main cause of the failure of internal fixation and fracture nonunion in proximal humerus surgery. Anatomical reduction and increasing the strength of internal fixation is the key to success. The purpose of this study was to investigate the short-term clinical effect of dual plate fixation in the treatment of proximal humeral fractures with calcar comminution. METHODS: The data of 37 patients with proximal humeral fractures with calcar comminution, treated in our departments from July 2018 to April 2020, were retrospectively analyzed.

[Combined convergent elbow dislocation with posterior luxation of the humeroulnar joint and radial head comminuted fracture in adults].

Transposition of the proximal radioulnar joint, so-called convergent elbow dislocation, is an extremely rare injury whereby the proximal radius crosses over the proximal ulnar and converges medially. This injury can occur in isolation or even less frequently in association with a posterior humeroulnar dislocation. Open reduction is almost always necessary. The diagnosis is frequently delayed as the injury is easily overlooked. In the literature, 13 cases in children and 2 in adults have so far been described. This is the first report of a combined elbow dislocation (posterior and convergent) w

Four Screws Diamond Configuration Fixation for Displaced, Comminuted Intracapsular Fracture Neck Femur in Young Adults.

BACKGROUND: Four cannulated cancellous screws (CCS) in diamond configuration have been recommended by some authors for fixation of intracapsular neck of femur (ICNF) fracture with posterior comminution in adults. This is also supported by biomechanical studies. However, the clinical usefulness of this biomechanical advantage is not known. This study evaluates the outcomes of displaced, comminuted ICNF fracture in young adults managed by four CCS fixation in diamond configuration. MATERIALS AND METHODS: 25 patients who met the inclusion criteria were operated during the study period. Four pati

[Endoscopy-Assisted Treatment of Metaphyseal Osteomyelitis Focus in the Distal Tibia after Osteosynthesis of Comminuted Pilon Fracture].

The submitted case study concerns a 36-year-old patient with a ladder fall injury, who suffered a comminuted right pilon fracture type C3 according to the AO classification. The primary treatment consisted in fixing the fracture with an ankle spanning external fixation and subsequent internal osteosynthesis after the recovery of soft tissues. At four weeks, an infection with fistula developed on the anterior face of the distal tibia. The patient was repeatedly treated with excochleation of suspected focus of osteomyelitis, soft tissue drainage by VAC system, application of antibiotics based on

What happens to the posterior comminution in extra-articular fractures of the distal radius treated with volar locking plates?

Extra-articular fractures of the distal radius with posterior displacement are typically treated with volar locking plates. However, this fixation method does not address the posterior comminution, which seems to have no impact on the final result. The purpose of this study was to determine the fate of the posterior comminution. This was a retrospective study of 22 patients over 50 years old with a distal radius fracture. A preoperative computed tomography (CT) scan was performed to evaluate the comminution. All fractures were fixed with a volar locking plate. All patients underwent a bone den
