Endoscopic management of failed frontal sinus obliteration.

BACKGROUND: Frontal sinus obliteration (FSO) traditionally has represented the final stage in the algorithm for difficult to manage frontal sinus disease. In addition, FSO has been used in selected cases of frontal sinus trauma. However, this procedure has been associated with failure in 5-10% of cases. Advances in surgical instrumentation and image-guided surgical navigation have permitted endoscopic management of these failures. METHODS: Eleven patients presenting with failure of a previously performed FSO were managed endoscopically with the assistance of image-guided surgical navigation.

Orbital wall thickness and the spread of infection from the paranasal sinuses.

Ninety-three human skulls (80 adults and 13 children) have been examined and the extent of thin bone in the party walls between the orbit and the frontal, ethmoidal and maxillary sinuses has been assessed. Translucent bone is most often present in the lateral wall of the ethmoidal labyrinth and least often in the floor of the frontal sinus. In children such bone is present significantly less often in the roof of the maxillary sinus (P less than 0.001) than in adults. Computerized tomography scans and clinical data from 6 patients with orbital cellulitis were reviewed. In one of these an infero

Pott's puffy tumor following an insect bite.

Pott's puffy tumor, a feature of osteomyelitis of the frontal bone, is a rare entity, especially in adults. Sir Percival Pott originally described this condition as a complication of trauma to the frontal bone. This is also a recognized complication of fronto-ethmoidal sinusitis. We present a rare case of Pott's puffy tumor caused by an insect bite presenting initially as a preseptal cellulitis and explore its pathogenesis and management.

Frontal sinus cranialization using the pericranial flap: an added layer of protection.

OBJECTIVES: Extensive fractures involving the anterior and posterior tables of the frontal sinus are treated by frontal sinus cranialization. During this procedure, the disrupted posterior wall of the frontal sinus is removed, the sinus mucosa is drilled away, and the brain and dura are permitted to rest against the repaired anterior wall and sinus floor. Conventionally, the area originally occupied by the frontal sinus is left as dead space or filled with free adipose tissue. We describe a method of cranialization using a pericranial flap and report our experience with this technique. STUDY

[Amaurosis secondary to frontal traumatism].

CASE REPORT: We present the case of a patient diagnosed with amaurosis of the right eye secondary to a right frontal contusion. The energy of the impact was projected from the orbital ceiling to the minor wing of the sphenoid bone. This bone was fractured, thus reducing the optic canal diameter and damaging the optic nerve. DISCUSSION: In our case, we describe a mixed mechanism of injury, that is to say, a frontal contusion indirectly transmitted to the optic canal and a direct lesion of the optic nerve secondary to the movement of the minor wing of the sphenoid bone into the optic canal.

Cathepsin-B activity in otosclerosis.

Cathepsin-B activity was determined fluorimetrically in the otosclerotic stapes footplate, the stapes superstructure, normal temporal cortical bone, and os frontale osteoma. Measurements with a synthetic substrate made determinations in individual samples possible. The cathepsin-B activity in the otosclerotic stapes footplate was one order of magnitude higher than that of the superstructure, which was not affected by the disease. The cortical bone and the superstructure displayed similar activities, as did os frontale osteoma and otosclerosis. The high lysosomal proteinase activity appears to

Congenital Ocular Dystopia from Orbitofrontal Bone Dysplasia.

BACKGROUND: Several patients with ocular dystopia and craniofacial differences have been found to have orbital dysplasia and a frontal bone defect. This deformity is characteristically different from differential diagnoses of encephalocele, sphenoid dysplasia, craniofacial dysostoses, or atypical clefting. METHODS: The authors retrospectively reviewed the craniofacial registries of two pediatric centers for patients presenting with ocular dystopia or orbitofrontal anomalies between 2000 and 2014. The features and treatment of these patients were analyzed. RESULTS: Four patients with congenit

Bridging the Frontozygomatic Gap in Fronto-Orbital Advancement Using Autologous Bone Bead Grafts, a Technical Note.

Fronto-orbital advancement is a standard surgical technique for addressing late-presenting craniosynostosis, particularly in cases involving premature fusion of the metopic or coronal suture. Inherently, this procedure results in a gap at the frontozygomatic pillar due to the advancement of the supraorbital bandeau, which presents a unique challenge for bone grafting because of its limited dimensions and the fact that the bone needs to be kept in a stable position. This technical note introduces a flexible and efficient approach to bridging this gap using autologous bone bead grafts. These gra

Incidence of Odontogenic Disease in Patients With Pott's Puffy Tumor.

PURPOSE: Osteomyelitis with subperiosteal abscess of the frontal bone, or Pott's puffy tumor (PPT), is a rare but life-threatening condition. The relationship of concurrent dental disease to PPT has not been well described. This study sought to delineate the incidence of odontogenic disease in PPT, especially in cases where there is no history of facial trauma or prior frontal sinus surgery. METHODS: A retrospective chart review of patients diagnosed with PPT between 2010 and 2019 was carried out. Demographics, pertinent medical history, surgical procedures performed, and microbial cultures a

A Concept of "Boundary Determination" by the Combination of a Local Flap and Free Tissue Transfer Useful for the Prevention of Postoperative Complications After Complex and Widespread Skull Base Reconstruction.

The transplanted tissue is sutured to the stumps of the bones in skull base reconstruction. Concerning reconstruction by free tissue transfer, it is difficult that the tissue is accurately sutured/fixed to a complex three-dimensional structure. The authors devised a technique in which free tissue transfer is combined with a local flap. A pericranial flap is sutured/fixed onto the irregular border between the cranial and nasal cavities. A free tissue graft is transplanted cranial or caudal to the determined border. Clinical presentation 1 is a patient with carcinoma of the frontal sinus (T4bN0M

Dural Penetration of Cavernous Hemangioma on Skull: Uncommon Clinical Presentation.

BACKGROUND: Cavernous hemangioma (CH) of the bone is a rare, benign neoplasm found usually in the vertebral body. This tumor rarely develops in the calvaria, with predilection to occur in the frontal and temporoparietal bones. CASE DESCRIPTION: A 56-year-old man with a right frontal palpable mass was admitted to our hospital. Magnetic resonance imaging (MRI) revealed a large extra-axial tumor in the right frontal bone. Intraoperatively, the bony mass involved the multilayers composed of extracranium-skull-dura-intradura-cortex. Pathologic examination revealed a typical calvarial CH penetratin

Reverse temporalis muscle flap: treatment of large anterior cranial base defect with direct intracranial-nasopharyngeal communication.

Inadequately managed frontal sinus fracture or sinusitis can pose a major problem of intracranial-nasopharyngeal communication. Life-threatening ascending infection of the intracranium is inevitable unless the intracranium is separated from the nasopharynx. In five patients with frontal bone defect associated with direct intracranial-nasopharyngeal communication, the authors used reverse temporalis muscle flap based on the superficial temporal vessels to obliterate the nasocranial communication. With the authors' method, the nasocranial communication were sealed off permanently, and the ascend

[Effectiveness of the treatment of frontal sinus inflammation and its prevention].

The analysis of intraoperative data obtained at correction of ethmoid bone structure in 130 patients with frontal sinusitis including the removed tissue histology shows that the severity of ostiomeatal complex deformation correlates with actual advance of the destructive alterations in the ethmoid labyrinth by the frontal sinus. Removal of necrobiotic tissues adjacent to the frontal sinus is pathogenetically valid and the most effective treatment of frontal sinusitis which has reduced the number of recurrences to 3.5%.

[Frontal sinus anatomical scanographic study of transgender patients for feminization frontoorbitoplasty surgery].

INTRODUCTION: Little is known about the anatomical elements that allow safe and predictable performance of fronto orbital surgery for facial feminization. The primary objective of this study was to analyze the characteristics (dimensions, pneumatization rate, and anterior wall thickness) of the frontal sinus in MtF transgender patients. The secondary objective was to establish reproducible criteria for CT measurements that could guide preoperative planning of frontal impaction in feminization frontoplasty (FF). MATERIALS: Fifty preoperative facial mass scans of FF surgery patients were includ
