[Malaria and hemoglobin S: interactions in African children].

Among 300 cases of Plasmodium falciparum malaria attacks explored in Gabon, the proportion of homozygous (SS) or heterozygous (AS) sickle-cell patients was 6.2 percent in 206 ordinary attacks and 3.2 percent in 94 cerebral malaria attacks, and 23.2 percent in the general population. On the other hand, asymptomatic carriage, as detected in 98 children by thin blood films in school screening, was as frequent in the SS or AS infantile population as in the general population. These data show that haemoglobin S protects effectively, although not entirely, against severe attacks of P. falciparum mal…

[Current serologic data on viral hemorrhagic fevers in the Central African Republic].

During the years 1984-1985, 1,528 serum samples were taken through out the CAR. Of these sera, 319 (20.8%) contained anti-Filoviridae antibodies (Ebola, Marburg). This figure is higher than those found in Cameroon, Sudan, Gabon and Zaïre. Three zones of the country are particularly exposed, the North-East where the population is in the contact with Sudan Ebola, the South-East where it is in contact with Zaïre Ebola, and the South-West where it is in contact with Sudan Ebola. The authors believe that there could exist either a Central African Ebola strain or an Ebola-Like less pathogenic virus …

Pharmacogene Sequencing of a Gabonese Population with Severe Plasmodium falciparum Malaria Reveals Multiple Novel Variants with Putative Relevance for Antimalarial Treatment.

Malaria remains one of the deadliest diseases in Africa, particularly for children. While successful in reducing morbidity and mortality, antimalarial treatments are also a major cause of adverse drug reactions (ADRs). Host genetic variation in genes involved in drug disposition or toxicity constitutes an important determinant of ADR risk and can prime for parasite drug resistance. Importantly, however, the genetic diversity in Africa is substantial, and thus, genetic profiles in one population cannot be reliably extrapolated to other ethnogeographic groups. Gabon is considered a high-transmis…

Is there a suburban sleeping sickness in Libreville?

BACKGROUND: The transmission of sleeping sickness occurs primarily in rural areas, and exposed populations are those living from rural activities such as agriculture, fishing, animal husbandry or hunting. However, urban and suburban foci are more and more reported in T. b. gambiense areas. In Libreville town, sleeping sickness cases are regularly diagnosed. In order to investigate about the establishment of a transmission cycle of that disease, we have carried out an entomological survey in two quarters in the vicinity of the town. METHODS: Vavoua traps were set out in all suitable biotopes f

[Specific serologic reactions in the cerebrospinal fluid in congenital syphilis and therapeutic implications].

In a study carried out in Gabon, antibodies against the treponema were looked for in the cerebrospinal fluid (CSF) in 13 children with active congenital syphilis (presence of specific IgM antibodies) and in 7 children with positive serologic reactions reflecting transplacental passage of maternal antibodies. Serologic reactions used included the VDRL test, the TPHA test, and the FTA-ABS IgG and IgM tests. Among the 13 children with syphilis, 7 had a positive FTA-ABS IgG test in the CSF; positivity of this test was not correlated with severity of clinical features, CSF protein levels or CSF cyt…

Treatment of recurrent, filarial, Calabar-type oedema with mebendazole.

A case of recurrent, severe filarial Calabar-type oedema, causing acute discomfort in the patient, is described in a male Caucasian. An adult Loa loa worm but no microfilariae were present while Dipetalonema perstans microfilaraemia was extremely low. Treatment with mebendazole (1.5 g per day) for 21 days led to the disappearance of the clinical symptoms and microfilariae, and to a decrease of eosinophilia to normal values.

A new species in the major malaria vector complex sheds light on reticulated species evolution.

Complexes of closely related species provide key insights into the rapid and independent evolution of adaptive traits. Here, we described and studied Anopheles fontenillei sp.n., a new species in the Anopheles gambiae complex that we recently discovered in the forested areas of Gabon, Central Africa. Our analysis placed the new taxon in the phylogenetic tree of the An. gambiae complex, revealing important introgression events with other members of the complex. Particularly, we detected recent introgression, with Anopheles gambiae and Anopheles coluzzii, of genes directly involved in vectorial …

Diagnosing multiple parasitic infections: trypanosomiasis, loiasis and schistosomiasis in a single case.

A case is reported of a 32-year-old traveller with loiasis, schistosomiasis and African trypanosomiasis. The patient had been working in oil exploration in Nigeria and Gabon and presented with Calabar swellings and carpal tunnel syndrome. Serology for all 3 diseases was positive but microfilariae of Loa loa and ova of schistosomiasis were not found. Treatment with diethylcarbamazine and praziquantel was given for loiasis and schistosomiasis respectively. Trypanosomes were isolated from a lymph node aspirate only after repetition of the procedure 2 months later and the patient was treated with …

Distribution of Aedes mosquito species along the rural-urban gradient in Lambaréné and its surrounding.

BACKGROUND: Aedes albopictus and Aedes aegypti are known for their potential as vectors of dengue (DENV) and chikungunya (CHIKV) viruses. However, entomological surveys are mostly carried out during epidemics. In Gabon where outbreaks of both viruses have occurred, there is no vector control program targeting these arboviruses. Therefore, we assessed the presence of Aedes species along a rural-urban gradient in Lambaréné (Gabon) and its surroundings and determined ecological factors associated to their presence. METHODS: An entomological survey was conducted in Lambaréné and its surrounding r…

Economic evaluation of an alternative drug to sulfadoxine-pyrimethamine as intermittent preventive treatment of malaria in pregnancy.

BACKGROUND: Intermittent preventive treatment in pregnancy (IPTp) with sulfadoxine-pyrimethamine (SP) is recommended in HIV-negative women to avert malaria, while this relies on cotrimoxazole prophylaxis (CTXp) in HIV-positive women. Alternative antimalarials are required in areas where parasite resistance to antifolate drugs is high. The cost-effectiveness of IPTp with alternative drugs is needed to inform policy. METHODS: The cost-effectiveness of 2-dose IPTp-mefloquine (MQ) was compared with IPTp-SP in HIV-negative women (Benin, Gabon, Mozambique and Tanzania). In HIV-positive women the co…

Polymorphisms in Plasmodium falciparum K13-propeller in Angola and Mozambique after the introduction of the ACTs.

We report the presence of SNPs in Plasmodium falciparum K13-propeller gene in two African countries, Angola and Mozambique, where malaria is … ion of ACT. The R471R mutation detected in Angola has already been reported in Africa (DR-Congo and Gabon). However, the mutations R575R (Angola) and V494I (Mozambique), have never been reported. V494I is adjacent to the known K13 resistance-associated mutation Y493H, although functional analysis did not predict a deleterious effect on protein function.

TNFalpha-308A associated with shorter intervals of Plasmodium falciparum reinfections.

A mutation at position -308 of the tumor necrosis factor alpha (TNF-308A) gene promoter has previously been associated with particular manifestations of infectious and non-infectious diseases. In a longitudinal study on malariological parameters in Gabon, TNF promoter variants of 98 children initially presenting with severe Plasmodium falciparum malaria, followed by a total of 504 reinfection events within 52 months, and 100 children initially presenting with mild malaria followed by a total of 342 reinfections were analyzed. Symptomatic P. falciparum reinfections occurred more quickly (median…

Tuberculosis of the lower cervical spine (C3-C7) in adults: diagnostic and surgical aspects.

From 1976 to 1992, 160 cases of tuberculosis of the spine were treated at Jeanne Ebori Medical Center (Gabon); among whom 10 were localized to the lower cervical spine (C3-C7). From those 10 cases, 5 concerned children and had a medical treatment, whereas the remaining 5 cases concerned adults and underwent surgery (anterior approach), following an initial three-weeks course of antituberculous therapy. Prominent features of the disease in the 5 adult cases included neck pain and stiffness, neurological deficits and osteolytic erosions with myelographic block. The mean length of follow-up was 3…

Five ways to find a parasite: comparison of five molecular assays for the detection of Strongyloides spp.

UNLABELLED: Strongyloidiasis, caused by nematodes of the genus Strongyloides, is a neglected tropical disease with significant public health implications. Accurate diagnostics are essential for disease surveillance and control, yet traditional stool-based and serological methods have limitations in sensitivity and specificity. Molecular assays offer an alternative diagnostic approach. In this study, we compared four probe-based real-time PCR (qPCR) assays and one loop-mediated isothermal amplification (LAMP) assay using human fecal DNA samples from Gabon and Papua New Guinea with both Strongyl…

Effective endovascular treatment of veins of Galen malformation in a child: A case study.

Vein of Galen aneurysmal malformation (VGAM) is a rare congenital arteriovenous anomaly characterized by abnormal connections between cerebral arteries and the vein of Galen. This case report details the presentation, diagnosis, and management of a 10-year-old boy from Gabon, who exhibited learning difficulties, macro crania, and generalized tonic-clonic seizures. A cerebral CT scan revealed an aneurysmal malformation of the ampulla of Galen with tri ventricular hydrocephalus, necessitating the placement of a peritoneal shunt. Upon referral to our facility, the patient underwent an angioMRI an…

Building the capacity of West African countries in Aedes surveillance: inaugural meeting of the West African Aedes Surveillance Network (WAASuN).

Arboviral diseases such as dengue, Zika and chikungunya transmitted by Aedes mosquitoes have been reported in 34 African countries. Available data indicate that in recent years there have been dengue and chikungunya outbreaks in the West Africa subregion, in countries including Côte d'Ivoire, Burkina Faso, Gabon, Senegal, and Benin. These viral diseases are causing an increased public health burden, which impedes poverty reduction and sustainable development. Aedes surveillance and control capacity, which are key to reducing the prevalence of arboviral infections, need to be strengthened in We…
