Neuroaxial versus general anaesthesia in geriatric patients for hip fracture surgery: does it matter?

The influence of the mode of anaesthesia on outcome of geriatric patients with hip fractures is a controversial issue in the medical literature. In the light of an ageing society, a conclusive answer to this question is of growing importance. The purpose of this review was to assess the effect of neuroaxial and general anaesthesia on mortality and morbidity in geriatric patients sustaining a hip fracture. Following a current literature search within the Pubmed and Cochrane database (1967-2010), 34 randomised controlled trials, 14 observational studies and eight reviews/meta-analysis publicatio

Malignant hyperthermia during general anesthesia for a surgery of idiopathic scoliosis: A case report.

Malignant hyperthermia is a potentially fatal hypermetabolic syndrome characterized by skeletal muscle rigidity and hyperpyrexia. We had an 11-year-old female presented for a surgery for idiopathic scoliosis of the thoracic vertebra. Generalized muscle rigidity and tachycardia were revealed 1 hour 35 minutes after isoflurane administration. Then the body temperature was risen to 43degrees C within 10 minutes and the arterial blood gas analysis showed a severe respiratory and metabolic acidosis. Under the suspicion of malignant hyperthermia, the patient was treated by discontinuing all anesthet

General anaesthesia versus local anaesthesia for carotid surgery (GALA): a multicentre, randomised controlled trial.

BACKGROUND: The effect of carotid endarterectomy in lowering the risk of stroke ipsilateral to severe atherosclerotic carotid-artery stenosis is offset by complications during or soon after surgery. We compared surgery under general anaesthesia with that under local anaesthesia because prediction and avoidance of perioperative strokes might be easier under local anaesthesia than under general anaesthesia. METHODS: We undertook a parallel group, multicentre, randomised controlled trial of 3526 patients with symptomatic or asymptomatic carotid stenosis from 95 centres in 24 countries. Participa

[Real-time intraoperative monitoring of recurrent laryngeal nerve in thyroid surgery under general anesthesia].

OBJECTIVE: To evaluate the clinical significance and application value of the real-time intraoperative monitoring (RTIM) technique in preventing recurrent laryngeal nerve (RLN) from injury during operation. METHODS: The RTIM of RLNs by nerve integrity monitor were used in 40 cases who underwent thyroid surgery under general anesthesia from Nov. 2002 to May 2005. The endotracheal intubation with laryngeal electrode sited on the tube nerve integrity monitor were adopted for general anesthesia. The RLNs in 7 cases who underwent thyroid gland lobectomy were exposed and explored and not exposed in

Ciprofol versus propofol for general anesthesia during thoracoscopic surgery: a randomized double-blind trial.

BACKGROUND: Ciprofol is a novel intravenous anesthetic with potentially improved hemodynamic stability and fewer injection-related adverse effects than propofol. This study compared ciprofol and propofol for general anesthesia in patients undergoing video-assisted thoracoscopic surgery (VATS). METHODS: In this prospective, randomized, double-blind trial, 120 adults undergoing elective thoracoscopic surgery were randomized to ciprofol (n = 60) or propofol (n = 60). MAP, HR, and BIS were assessed at eight predefined perioperative time points using linear mixed-effects models. The repeated binar

Predictors of Postacute Care Discharge in Older Adult Emergency General Surgery: A Nationwide Analysis.

INTRODUCTION: Older adults undergoing emergency general surgery (EGS) represent a particularly high-risk population. We aimed to identify predictors of discharge to postacute care facilities in older adult EGS patients. METHODS: We used the 2013-2019 American College of Surgeons National Surgical Quality Improvement Program database to identify all EGS patients ≥ 65 y of age presenting directly from home. The primary outcome was nonroutine discharge, defined as discharge to a postacute care facility. Nonsurvivors and patients transferred from health care facilities were excluded. Bidirectiona…

Coloproctology practice during General and Digestive Surgery training. Survey of the Galician Coloproctology Group.

INTRODUCTION: In general surgery (GS), coloproctology (CP) constitutes one of the cornerstones in specialist training; however, it is unclear whether this training is sufficiently comprehensive and how trainees perceive it. In the same way, it's interesting to assess whether advanced accreditation of units may have an influence on this. Our objective was to determine the quality of this training in our community and to compare the results between hospitals with advanced colorectal unit and those without accreditation. MATERIALS AND METHODS: A cross-sectional study was conducted using the resu…

Artificial Intelligence for Predicting Length of Stay for General Surgery Patients: A Systematic Review.

BACKGROUND: Length of stay (LoS) prediction is vital for optimizing surgical patient flow and resource allocation. Artificial intelligence (AI) and machine learning (ML) models may enhance LoS prediction, yet their validity and clinical utility in General Surgery remain unclear. METHODS: Embase, Medline, Scopus, and Web of Science were systematically searched to June 2025. Studies employing AI or ML to predict LoS or discharge timing in adult General Surgery cohorts were included. Data on model development, predictor and target variables, performance, and validation were extracted. Risk of bi…

Spinal Anesthesia: A Safe and Effective Alternative to General Anesthesia for Infantile DDH Surgery.

BACKGROUND: Operative hip reductions for infantile DDH are traditionally performed under general anesthesia (GA). This introduces potential safety concerns, as risks related to GA are highest in children below 1 year. While surgery can be temporarily delayed to reduce these risks, this decreases the chances of a successful reduction via closed methods alone and also compromises valuable remodeling potential. This pilot study sought to evaluate the safety and efficacy of spinal anesthesia (SA) in infants undergoing operative hip reduction. METHODS: This is a multicenter, retrospective cohort s

Incidence and Associated Factors of ST-Segment Changes in Noncardiac Surgery Under General Anesthesia.

BACKGROUND: Intraoperative cardiac complications may result from an imbalance between myocardial oxygen supply and demand or from plaque rupture. Electrocardiographic (ECG) alterations in the ST-segment observed during surgery or anesthesia can serve as indicators of myocardial ischemia. Numerous factors influence ST-segment variability. This study aimed to investigate the incidence of ST-segment changes and the factors associated with their occurrence. METHODS: This prospective observational study enrolled adult patients (aged ≥ 20 years) undergoing noncardiac surgery under general anesthesi
